PurposeThis study investigates the impact of the degree of process maturity on the degree of patient orientation in the context of radical process changes. The study is based on a sample of healthcare providers in Ukraine which experiences a fundamental transformation of its healthcare system.Design/methodology/approachThe investigation was conducted among the full population of the chief physicians from 53 medical institutions (hospitals, general practitioners centers, dental clinics, and maternity clinics) in one of the largest cities in Ukraine. We investigated the maturity of the process of interaction with patients as perceived by these top managers. We applied variance-based structural equation modeling (SmartPLS3).FindingsThe study shows that each stage of process maturity predetermines the following one. With regard to the impact of each stage of process maturity on patient orientation, all stages show a positive and significant relationship toward patient orientation, i.e. even the lowest stage of maturity is critical for patient orientation. A further contradictory finding to extant literature is, that based on the set of indicators, the process appears to be in different stages at the same time. This speaks against the regular sequence-based approach toward process maturity.Originality/valueAlthough it has been assumed that higher degrees of process maturity are associated with higher customer (patient) orientation, this work shows that the relationship holds also for each stage of process maturity separately. This research is based on a very unique sample – the almost complete set of chief physicians and their deputies of practically all medical institutions of a large city.

Almost half of patients referred to orthopedic surgeons for Total Joint Arthroplasty (TJA) do not require TJA at that time or are not appropriate surgical candidates. The objective was to explore patients’ perspectives of a Surgical Readiness Interview Tool and its potential utility in the Osteoarthritis (OA) management process. Semi-structured qualitative interviews were conducted with a convenience sample of patients from arthroplasty clinics in Alberta. All interviews were digitally recorded and transcribed verbatim. Analysis was conducted iteratively, applying a constant comparison method. Nine patients were interviewed. Participants found the interview tool to be relevant and comprehensible. Suggestions were made on how to improve tool clarity and administration processes. Patient orientation versus that of the doctor, and expectation management emerged as salient factors in the meaningful application of the tool. As a result of the interviews, a revised tool was developed which incorporated the participant suggestions. Patients were positive about the interview tool and felt that it would lead to better care provision, particularly with incorporation of participants’ suggestions. The data suggest that the interview tool could improve the conversation on surgical readiness, conservative management, and addressing modifiable risk factors prior to TJA.

AbstractIn this interview, Werner Dorfmeister, a Vice President at DasLab and a former Global Sales Director at Karl Storz, reports on the current developments of digital technologies in the field of medical instruments and services. It is a sector where digitalization affects all processes within hospitals as well as pre- and post-surgical activities. Due to the many actors involved, who use different information systems, and the strong regulation of the entire healthcare sector, crafting digital solutions is complex, but nevertheless promising and necessary. Among the key areas mentioned in the interview are telemedicine, hospital and patient management and a sector-wide digital infrastructure. platforms are described as important enablers to address current inefficiencies and to leverage future potentials towards patient-orientation, such as digital patient pathways.

X-ray is a highly demanded therapy method with wide clinical indications. The high operational efficiency of such equipment makes it possible to ensure the accessibility of medical care. Taking into account the current healthcare trend towards patient-oriented principles of work, it is necessary to evaluate medical organizations and their units for compliance with these principles. Purpose of the study: to assess the implementation of patient-oriented principles in the work of the X-ray therapy service of the City Mariinsky Hospital. Objectives of the research: to analyze the organizational conditions of the X-ray therapy service, to study the structure and dynamics of patients and make conclusions about the implementation of the principles of patient-oriented principles of work. Material and methods. The activity of the X-ray therapy was assessed from the patient-oriented approach. The methods of bibliographic and informational search of sources in document databases, as well as semantic search on the Internet were used. Results: The hospital’s X-ray therapy service demonstrates high rates of operational efficiency, providing in a short time high rate of access to the X-ray therapy for patients with different nosology. It’s work meets most of the criteria for patient-centered work - the proximity of medical care to the patient, orientation towards respectful attitude to the values and needs of patients, rational use of available technical, time and human resources. For further improvement of work in accordance with the principles of patient orientation, it is necessary to introduce formalized questionnaires of the quality of life and assess the quality of medical services. Conclusion. The presented analysis is the first comprehensive multicriteria study of the patient-centered work of the X-ray therapy service in Russia.

One of the main factors for the increase in the incidence of skin cancer in Brazil today is exposure to solar radiation. The main means of prevention is through photoprotection, together with factors such as solar incidence in the region, the habits of the population, and the skin phototype. The relationship between photoprotection and vitamin D is fundamental for patient orientation since photoprotection is a practice widely used today for all people, both those who have already suffered some type of skin cancer and others who are at greater risk or not. to develop it. On the other hand, some studies suggest that the photoprotection of the skin would jeopardize the development of Vitamin D, which may cause its deficiency, and may subject patients to future changes in bone mineralization, increasing the risk of bone deformities and fractures since the vitamin D is essential for bone tissue and its production is stimulated by skin exposure to ultraviolet B radiation (UVB), with natural sources limited through the diet. The discussion about the relationship between photoprotection and vitamin D is essential to establish the right conditions for each patient.

Problem setting. The active development of the service sector contributes to the growth of scientists’ attention to the concept of marketing relations, the center of which is a customer-oriented approach. This concept is especially relevant for the field of medical services, as the quality of these services not only forms aspects of patient satisfaction, but also determines their quality of life and longevity. The Ukrainian model of health care and the provision of medical services has recently been in the process of reform. The changes affect key elements of the functioning of the medical sector: transformation of the structure of the medical services market, aimed at increasing the share of the private sector, changing the principles and conditions of financing, improving the system of medical education and quality of health care. The insurance mechanism in the system of medical services is being reformed and the system of legal regulation of state medical institutions is being changed. One of the key tasks of the reform is to ensure the orientation of the health care system to meet the needs of patients in treatment. This necessitates scientific research in the interaction of two key aspects of medical service – the patient (recipient of medical services) and the medical worker (institution of medical services). Recent research and publications analysis. Ukrainian researchers have devoted their work to analyzing the needs of patients in the system of patient-medical institution and finding effective mechanisms for managing medical institutions to meet these needs. The analysis of patients’ opinions on the availability, satisfaction with the provision of medical services and health care reform was conducted by scientists Shkrobanets, V. Galkin researched the issue of improving outpatient therapeutic care as an urgent task of health care. English scientists S. Cavanagh and K. Chadwick identified the basic needs of public health. They analyzed the needs, aspirations and goals: welfare, quality of life and public policy in the field of medicine, and conducted applied research on the quality of life. However, given the realities of the development of the medical services market in Ukraine, patient orientation issues need to be explored in more depth. Highlighting previously unsettled parts of the general problem. Ukrainian patients face significant difficulties in obtaining medical services. In addition to physiological problems, there are psychological problems associated with the implementation of the choice among the complex processes of diagnosis and treatment. The greatest responsibility for health care processes lies with physicians, heads of health care facilities, and other health care providers who are aware of patients’ needs and are responsible for the functioning of the system. Their ability to identify problems and respond appropriately to public demand is fundamental to working in terms of their competencies. Thus, the competence of health professionals is a systemic factor that contributes to the patient’s receipt of high-level medical services. Paper main body. Modern market relations are subject to economic laws that are specific to management and are in conflict with the methods of administration. Medical care within market relations is a specific type of intangible production, filled with economic characteristics. In recent years, Ukraine’s medical sector has achieved significant results in overcoming the crisis that arose during the establishment of independence in all spheres of society, and, as a result of global social, political and economic changes, continues to be reformed. The essence of the current stage of health care reform in Ukraine is characterized by objective conditions under which medical services become a commodity, and the relationship between doctor and patient in its content begins to be defined as marketing with the predominant role of the patient. The introduction of economic criteria in the activities of the medical sector in modern market relations is determined by the need to develop the health care system. At the same time, the reform of health care delivery is being transformed into a system that regulates supply and demand in the health care market. Conclusions of the research and prospects for further studies. According to the results of the research, it is established that Ukrainian consumers average the patient’s focus on medical services. Among the key, unresolved issues that hinder the introduction of patient orientation are the insufficiently comfortable conditions of patients’ stay in inpatient facilities and treatment, insufficient effectiveness of treatment in general and the results obtained. Weaknesses include low awareness of health services in public health facilities, lack of focus on patient health, lack of psychological support for patients during treatment and rehabilitation, late delivery of services, and problems with non-compliance. This creates a field for new research to address the identified problems.

Background: Cleft is a common congenital abnormality in the lip and palate. Cleft palate or palatoschisis is a congenital abnormality occured during pregnancy, resulting in cleft of the palate that does not coalesce into the nasal cavity area, so there is a relationship between the nasal cavity and mouth. Various surgical techniques for cleft palate correction can be used and each method has advantages and disadvantages. Patient and Method: Here, we report a case of palatoschisis reconstruction using Von Langenbeck technique and hemostatic absorbable gelatin that was carried out at Aliyah 3 Hospital, Kendari. Result: After initial diagnosis, the patient underwent surgery about two weeks later. Von Langenback palatoplasty was performed to close the palate defect. The defect then closed primarily with 4/0 optime and 6/0 corolene. Conclusion: Optimal surgical planning, good patient orientation and surgical techniques that are carefully considered are essential for optimal results. Herein, we report a case of palatoschisis and reconstructive surgery using von Langenback technique performed to restore the cleft of palate.
