AbstractPromotion of “skilled birth attendants” (SBAs) in global maternal health policy has prompted a range of policy responses to “traditional birth attendants” (TBAs). In Indonesia the response has been to develop a national policy of partnership between SBAs (bidan) and TBAs (dukun bayi). This policy aims to ensure the presence of an SBA at every birth yet offers a role for TBAs. In this chapter I examine the development of a district regulation on partnership, promoted within the context of decentralization policies enacted in Indonesia from 1999. The district regulation aimed to strengthen the national policy in a location in West Java where TBAs remain popular. Drawing on 10 months of fieldwork from 2012 to 2013 at a district health office and on observations of its outreach programs, I elucidate how the regulation on partnership was promoted through the policy entrepreneurship of certain key figures in the district health office. They argued that the partnership regulation was the fastest means to improve maternal health. But casting a spotlight on the relationship between SBAs and TBAs diverted attention away from other health system challenges including under-resourced medical facilities and a weak referral system. Three contexts played into this process of bringing the partnership issue to the fore: global policies promoting SBAs and sidelining TBAs; pressure to achieve the Millennium Development Goal (MDG) on maternal mortality; and the limited financial power and decision space afforded to districts under decentralization in Indonesia. In this context, the regulation offered a viable path for demonstrating commitment to improving maternal health outcomes, yet one that failed to address broader constraints in the health system that contribute to persistent high maternal mortality rates.

Prioritization of health problems is carried out using the USG method based on the criteria of Urgency, Seriousness, Growth and fi nding the root of the problem using the fi shbone diagram method. The increase in HIV/AIDS cases with an USG score of 128 has become a top priority health problem in Pacitan District. An increase over the last  4 years with the highest number of cases in 2019, which was 39 cases. The fishbine diagram shows the root of the HIV/AIDS problem, namely the lack of public knowledge about HIV/AIDS, the lack of public knowledge about HIV/AIDS, the lack of awareness of people at risk for conducting an HIV test, this makes the community less aware of information about HIV/AIDS, causing public stigma. which results in people being closed / unwilling to check themselves at the puskesmas orhospital. The increase in HIV/AIDS cases is one of the problems in Pacitan district. To reduce the incidence, health workers need to optimize the dissemination of information about HIV/AIDS, especially risk factors, causes, prevention, symptoms and treatment. Medication review is an important service in optimising medicine use and improves clinical outcomes. This study aims to assess the effectiveness of pharmacist-led medication review on patients’ knowledge and adherence. For this prospective study, 480 patients were randomly recruited in six primary healthcare clinics in Petaling District Health Office. Patients were interviewed with a questionnaire and validated medication adherence scale during recruitment and at follow-up visit to assess their medication knowledge and adherence. The data was analysed using Chi-square tests and paired t-tests to determine the correlation between medication knowledge and adherence with patient demographics. Among 408 patients that had completed the follow-up, 16.9% of patients showed medication knowledge deficits on recruitment. However, there is a significant improvement in the medication knowledge indices during the follow-up session (p < 0.001). Elderly patients were found to benefit from medication review with better medication knowledge and adherence postmedication review. The study found that the number of good adherers increased by 29.3% after the medication review. A further study demonstrating the effectiveness of medication review in cultivating knowledge retention and sustained adherence in the longer-term is warranted. Future work shall also focus on measuring the cost-effectiveness of pharmacistled medication review implementation in primary healthcare settings. Pharmacist-led medication review is an essential and effective service in primary health care facilities for patients to enhance their knowledge on their medications, and adherence especially in elderly patients on chronic medications.
