The Warden procedure has been described for the repair of a partial anomalous pulmonary venous connection that is inserted high in the superior vena cava. One long-term concern remains, which is related to the narrowing of the superior vena caval anastomosis. In fully grown teenagers and adults, we have modified the procedure by using an interposition graft that consists of a descending thoracic aortic homograft. It usually matches the size of the superior vena cava and avoids the need for anticoagulation. We also insert the homograft into the free wall of the right atrium rather than into the atrial appendage. We present the technique in a 16-year-old girl through a vertical right axillary thoracotomy.

Children with malignant mediastinal masses have increased thrombotic events (TE). We examined primary thromboprophylaxis in reducing TE. Eligible subjects were started on enoxaparin thromboprophylaxis and compared to a cohort without. There were 15 TEs among 76 subjects for an incidence of 19.7%. Mediastinal compression directly led to TE, (M-TE) in 9.2% of subjects requiring 2-fold longer duration of therapeutic anticoagulation. Primary thromboprophylaxis revealed a trend towards reduction in M-TE although not statistically significant. The M-TE subjects had greater superior vena caval compression at diagnosis (p=0.033). We conclude that strategic thromboprophylaxis guided by monitoring dynamic vascular compression can improve TE outcomes.

Acute haemorrhagic pancreatitis is a severe form of pancreatitis often encountered in ethanol abuse. Extensive venous thrombosis resulting in pulmonary embolism is a rare presenting clinical entity of acute haemorrhagic pancreatitis. Here, we report a young male with an extensive deep vein thrombosis involving superior vena caval territory associated with haemorrhagic pancreatitis presented with pulmonary embolism managed supportively. Prompt recognition and appropriate intervention of this rare complication would improve the outcome in patients with acute pancreatitis.
