Diabetes mellitus remains a major risk factor for developing cardiovascular diseases, resulting in increased morbidity and mortality associated with cardiovascular complications. Given the burden of diabetes-related cardiovascular complications, there is a need to identify strategies, safe and effective therapeutic agents that could effectively prevent and control diabetes. Presently, many patients living with diabetes depends on traditional medicines as an alternative cure. Channa striatus (Haruan) is a freshwater fish traditionally used to treat wounds, inflammations, and pains. Several pharmacological investigations have supported the folkloric claims of C. striatus extracts, including hypoglycemic, hypolipidemic, antioxidant, anti-inflammatory, and pro-platelet aggregation activities. The therapeutic potentials of C. striatus were demonstrated to be associated with the presence of high content essential amino acids and good fatty acids known to improve cell growth and facilitate wound healing. Therefore, C. striatus bioactive compounds have great potentials to serve as lead candidates in developing novel therapeutic agents for the management of diabetes and related cardiovascular diseases. This review aims to provide a comprehensive overview of the pharmacological properties and therapeutic potentials of C. striatus for the management of diabetes and associated cardiovascular complications.

Wound healing remains a major issue in surgery. None of the existing treatment modalities in caring for wounds can yet claim to be the holy grail of wound management. Channa striatus, locally known in Malaysia as Haruan, is a freshwater air-breathing carnivorous fish that is proven to influence the different phases of wound healing. As a medicinal fish, not only does Haruan have a high content of amino and fatty acids, which are essential in collagen fibre synthesis during wound recovery, it also abounds in arachidonic acid and polyunsaturated fatty acids that promote prostaglandin synthesis, a vital component of the healing process. Moreover, its antinociceptive effects could potentially reduce wound pain, an important factor in wound healing. Proteomic studies show that a quarter of the total protein detected in freeze- and spray-dried C. striatus extract are actin, myosin and tropomyosin – all molecules that play a role in the wound healing process. Proteomic profiling also reveals that Haruan possesses two types of collagen namely collagen type-I and type-II that confer tensile strength during the healing process. It is proven that collagen along with other components of the extracellular matrix form the granulation tissue which, when contracted, closes the wound and concomitantly aligns the collagen fibres in the extracellular matrix. Hence, it is inferred that Haruan promotes the maturation of granulation tissue, thereby expediting the wound healing process itself. Consequently, it could mediate a faster recovery from surgical wound coupled with a lower incidence of wound infection due to an improved and accelerated wound healing process. Additionally, Haruan has demonstrated its ability in promoting angiogenesis and cell proliferation in wound bed preparation for skin grafting. Furthermore, a Haruan aerosol concentrate can act as a wound dressing at the donor site thereby enhancing the healing process while simultaneously exhibiting some antinociceptive properties. Haruan’s exceptional ability in promoting wound healing together with its potential use in skin grafting would be instrumental in the field of surgery. In essence, the cumulated benefits from all the processes involved would translate into a significant reduction of hospitalisation cost; that would immensely benefit not only the patient, but also the government.

Abstract Objective: To determine the efficacy of Channa striata extract on serum albumin level and wound healing after radical hysterectomy in cervical cancer patients in Mohammad Hoesin Hospital Palembang.Methods: A clinical trial was conducted in Mohammad Hoesin Hospital Palembang during period of January – September 2019. Samples were cervical cancer patients undergoing radical hysterectomy. Serum albumin level was measured before surgery, after surgery, and after the administration of Channa striata extract capsule. Efficacy of the supplement was analyzed with SPSS version 20 using paired t-test.Result: Twenty-eight cervical cancer patients undergoing radical hysterectomy who fulfilled inclusion criteria were obtained. Majority of patients were aged between 40-49 years old (89.3%), normoweight (39.3%), and lived in rural area. Mean duration of surgery was 154.46 ± 40.47 minutes. Serum albumin level before surgery, after surgery and after the administration of Channa striatus extract were 3.4 ± 0.61 g/dL, 2.91 ± 0.42 g/dL, and 3.11 ± 0.49 g/dL, respectively. There was a statistically significant difference between serum albumin level before and after the surgery (p=0.000). However, no statistically significant difference was found between serum albumin level after surgery and after administration of Channa striata extract capsule (p=0.750).Conclusions: There was no significant difference between serum albumin level after surgery and after administration of Channa striata extract capsule.Keywords: cervical cancer , channa striatus extract , radical hysterectomy, serum albumin level. Abstrak Tujuan: Untuk mengetahui efikasi ekstrak Channa striata terhadap kadar albumin serum dan penyembuhan luka pasca histerektomi radikal pada pasien kanker serviks di RS Mohammad Hoesin Palembang.Metode: Uji klinis dilakukan di RS Mohammad Hoesin Palembang selama periode Januari – September 2019. Sampel adalah pasien kanker serviks yang menjalani histerektomi radikal. Kadar albumin serum diukur sebelum operasi, setelah operasi, dan setelah pemberian kapsul ekstrak Channa striata. Khasiat suplemen dianalisis dengan SPSS versi 20 menggunakan uji-t berpasangan.Hasil: Didapatkan 28 pasien kanker serviks yang menjalani histerektomi radikal yang memenuhi kriteria inklusi. Mayoritas pasien berusia antara 40-49 tahun (89,3%), normoweight (39,3%), dan tinggal di daerah pedesaan. Durasi rata-rata operasi adalah 154,46 ± 40,47 menit. Kadar albumin serum sebelum operasi, setelah operasi dan setelah pemberian ekstrak Channa striatus berturut-turut adalah 3,4 ± 0,61 g/dL, 2,91 ± 0,42 g/dL, dan 3,11 ± 0,49 g/dL. Ada perbedaan yang signifikan secara statistik antara kadar albumin serum sebelum dan sesudah operasi (p=0,000). Namun, tidak ditemukan perbedaan yang signifikan secara statistik antara kadar albumin serum setelah operasi dan setelah pemberian kapsul ekstrak Channa striata (p=0,750).Kesimpulan: Tidak terdapat perbedaan yang bermakna antara kadar albumin serum setelah pembedahan dan setelah pemberian kapsul ekstrak Channa striata.Kata kunci: ekstrak Channa striatus, histerektomi radikal, kadar albumin serum, kanker serviks.

The local Channa species used in this research were Channa striatus (haruan) and Channa micropeltes (toman). Both samples were extracted using soxhlet extraction to procure the fish oil and then examined on its physico-chemical properties. The C. micropeltes species shows the highest yield of extracted fish oil (%, w/w) with dry weight basis (9.51 ± 1.20%). The physico-chemical results of the fish oil from haruan and toman were as follows respectively; colour: yellowish dark red for both species, specific gravity: 0.93 ± 0.10, 0.91 ± 0.03; AV: 21.78 ± 2.30 mg KOH/g, 5.55 ± 1.64 mg KOH/g; FFA: 10.95 ± 1.16%, 2.79 ± 0.83% and PV: 18.00 ± 6.56. meq./kg, 14.67 ± 3.79 meq./kg. Both fish oils showed a relatively comparable fatty acid profile. The availability of seven major enriched fatty acid constituents in both fish oils namely dodecanoic acid, 9-hexadecenoic acid, hexadecanoic acid, linoleic acid, oleic acid, octadecanoic acid, docosahexaenoic acid andaninfinitesimal amount of arachidonic acid (the main precursor for wound healing process) may perhaps berequisite for therapeutic consumption.

A field study was carried out to investigate the livelihood patterns the socio-economic conditions of the small-scale traditional fishermen of Kharingcha baor at Chaugachha upazilla in Jashore, Bangladesh. The survey was conducted on 100 peoples from 70 families of the fishermen in the study area. It was found from the study that 94% fishermen were comprising of males, while 6% were of females. Among them, the majority was Hindus (88%) and the minority was Muslims (12%). During the suvey, the proportion of semi paka houses were documented to be 88%, while kancha were 12%, and all the families had electricity facilities. It was found that 20% of fishermen had no education, 6% had passed SSC-HSC examinations, 14% had passed 6-10 classes and 24% had passed 1-5 classes. It revealed that 84% of fishermen were married and 16% were unmarried. Throughout the study, 33% of the fishermen were found to be dependent on village doctors, 40% on upazilla hospital and 27% on MBBS doctors. Several types of local boats and dingi nouka as well as nets such as tana jal (surrounding net), set bagnet, khapla jal (cast net), thela jal (push net) etc. were used for the harvesting of different fish species (Labeo rohita, Catla catla, Cyprinus carpio, Cirrhinus mrigala, Heteropneustes fossilis, Channa striatus, C. punctatus etc.). Averagely, 35-80 kg fish per day was caught by a group of fishermen using a boat. The majority of the fishers sold their harvested fishes upon the fulfillment of their demands. The monthly mean income of fishermen was around BDT. 5,000-15,000 in the baor area. Among the fishermen, 56% of them were observed to lend money from money lenders and 44% from the NGOs for their fishing-related activities and livelihoods. It revealed from the present findings that the traditional fishermen of Kharingcha baor require more technical, organizational and institutional helps for the improvement of their socioeconomic condition and sustainable livelihood.
