Efforts in Disaster Risk Reduction (DRR) are widely geared towards integrating indigenous knowledge and science. Several conceptual frameworks have thus evolved towards co-creating knowledge and co-designing DRR measures from the standpoint of the communities-at-risk. This is claimed to foster optimization and sustainability of measures. This study tests the effectiveness of this standpoint argument based on the case of floods in the Rwenzori, western Uganda, where a mismatch is noted between research, policy, and action. A protocol was developed to stimulate dialogue on knowledge co-creation and co-designing of DRR measures among participants from three stakeholder groups: scientists, policymakers, and communities-at-risk. Beyond convergence on some measures among participants, equitable deliberations were observed among the different stakeholders. This enabled three processes: coalescing some of the proposed measures, the emergence of hybrid worldviews, and co-design of alternative options. The co-designed options fall within the contemporary conceptualization of nature-based solutions and sustainability. This meant that they are adoptable and optimizable over time by communities-at-risk. This constructive knowledge integration and co-design of DRR options were favored by three attributes: coalescing overlaps in theorizations of processes, embracing diversity in ontological values, and self-critiques among policymakers. Lessons are drawn on how these attributes facilitate bridging gaps between science, policy, and action in DRR.

Abstract Background Effective management of solid waste is one of the most serious environmental problems confronting urban governments in developing countries due to insufficient financial resources and institutional capacity to provide basic solid waste management infrastructure, impoverished urban populations, low rates and coverage of collection, and rising food consumption rates. Composting has been touted as the most economical solution for reducing organic fraction of urban waste volumes while releasing vital nutrients for the soils. However, there is a paucity of information on utilization and associated factors of urban wastes in Uganda. This study aims to assess the level of utilization of Municipal Solid Waste (MSW) compost produced from Clean Development Mechanism (CDM) plants and associated factors among farmers in Fort Portal and Mbarara Municipalities, western Uganda. Methods Data was collected using a cross-sectional survey from 359 and 361 randomly selected farmers in Mbarara and Fort Portal, respectively. A semi-structured questionnaire was employed to collect quantitative data and analyzed statistically using SPSS and STATA statistical programs. Probit model was used to determine factors that influence farmers’ decisions to utilize MSW compost as a soil conditioner. Results The findings revealed a very low level of utilization in both municipalities, 1.7% in Fort Portal and 2.2% in Mbarara. The results indicated that annual farm income, access to other soil conditioners, experience with the use of fertilizers, membership to a farmers’ group and the cost of MSW compost significantly influenced farmers’ decision to use MSW compost. Conclusions The findings call for government investment in policies aimed at increasing the level of utilization of the MSW compost by improving accessibility to soil conditioners, creating opportunities for maximizing household incomes, farmer-to-farmer experience sharing, dissemination of information through extension programmes and other innovative communication approaches, operationalization of farmers’ groups and introduction of subsidy schemes on the price of compost through operational tax waivers in urban areas of Uganda. Finally, to guarantee quality and to improve the adoption of compost generated at the CDM plants, there is a need for research to assess the quality of MSW compost, undertake a supply chain analysis and cost–benefit study and set a price commensurate with the quality, and develop guidelines and rates of application of the MSW compost.

Abstract Background: S. aureus is a skin and mucosal bacterial commensal of both humans and animals which has evolved as an important pathogen implicated to cause various infections. High levels of antibiotic use have resulted into multi-drug resistance MRSA, especially among HA-MRSA, CA-and LA - MRSA. Awareness on coexistence and diversity of MRSA clones among humans and household Livestock particularly cattle and swine in our region is limited. We used spa typing method to determine spa diversity, distribution and coexistence in outpatients, household contacts and respective livestock (cattle and swine) in Kabale region, south western Uganda. Methods: This was a cross sectional study by design consisting of outpatients, household contacts and livestock. Outpatients (n =100) colonized with MRSA were traced back to their respective homesteads where household members, domestic cattle, and, swine were tested for S. aureus and subsequently MRSA colonization. High-resolution DNA melting analysis was used to determine spa types among MRSA isolates. Overlap of MRSA isolates among humans and livestock was based on the presence of similar spa types.Results: A total of 3371 S.aureus isolates were collected from outpatients (n =376), household contacts (n = 1531), Cattle (n = 1159) and Swine (n = 305), among which 482 had mecA gene where 27% (100/376) and 8% (123/1531) were outpatients and household contacts respectively while 11% (132/1159) and 42% (127/305) were cattle and swine respectively. Twenty different spa types were identified; t034, t4677, t108, t1451, t9377, t1081, t040, t701, t041, t002, t044, t037,t121, t127, t922, t032, t019, t018, t012 and t030, among which t034 (109/482), t4677 (53/482), t9377 (63/482) and t1081 (53/482) were most prevalent and distributed among human and livestock. All the MRSA isolates were multidrug resistant to antibiotics tested. Conclusion: In Kabale region, there is high diversity of spa types among MRSA. Presence of similar spa types was found circulating among humans and their respective livestock which demonstrates a possible bidirectional transmission. Presence of MDR - MRSA highlights the need for effective prevention and control of MRSA among livestock and in the community using One Health approach.

Agricultural innovation is considered paramount in solving poverty, food insecurity and malnutrition in the global south and notably in the East-African highlands. However, processes of change surrounding innovation in agriculture, and potential gender differences in their impacts, are often poorly understood. This paper resorts to principles from Farming Systems Research (FSR) and social gender analysis to study agricultural innovation processes and increase the understanding of the differential ways men and women engage with and are impacted by agricultural innovation(s). We analyze qualitative data from six Focus Group Discussions conducted in each of the two study communities located in Central and Western Uganda. These data focus on the most important agricultural innovations as perceived and assessed by men and women in their community. We list and discuss these most important innovations and further zoom in on one innovation per site: “Use of herbicides in maize production” in Central Uganda and “New agronomic practices for intensified highland banana production” in Western Uganda. Results clearly show that women's and men's domains are not separated as superficially might appear. Women and men have both separate and joint interests and adoption of an innovation by one gender, will affect the other too. The effects are multifold, with positive and negative elements. Women's ability to innovate is constrained as compared to men because gender norms limit women's agency in relation to mobility and financial independence amongst others. The two innovations studied were found to alter some gender roles and relations but did not unambiguously contribute to increasing gender equality.

Objective: The study aimed to investigate the relationship between mental health with the level of education, relationship status, and awareness on mental health among low-income earners in Western Uganda.Methods: This was a cross-sectional descriptive study carried out among 253 participants. Anxiety, anger, and depression were assessed using a modified generalized anxiety disorder (GAD-7), Spielberger's State-Trait Anger Expression Inventory-2, and Beck Depression Inventory item tools, respectively.Results: The majority of our respondents were male (n = 150/253, 59.3), had a secondary level of education (104/253, 41.1), and were single (137/253, 54.2). No formal education and primary education (r2 = 47.4% and 6.4%, respectively) had a negative correlation with awareness of mental health care. In addition, no formal education had a positive correlation with anger and depression (r2 = 1.9% and 0.3%, respectively). Singleness in this study had a negative correlation with awareness of mental health care, anger, and depression (r2 = 1.9, 0.8, and 0.3%, respectively), and a positive correlation with anxiety (r2 = 3.9%).Conclusion: It is evident that education and relationship status influenced awareness on mental health care and mental health state among low-income earners in Western Uganda during the first COVID-19 lockdown. Therefore, policymakers should strengthen social transformation through the proper engagement of low-income earners in this COVID-19 era.

Objective The study aim was to evaluate the prevalence and predictors of hypertension among an urban adult population in Mbarara city, Western Uganda. Methods We evaluated blood pressure measurements, social demographic and clinical parameters of adults living in Mbarara city, Uganda. These parameters were extracted from medical records of adults who participated in the Uganda World Kidney Day 2020 health screening activities. A total of 302 adults were evaluated for hypertension using the American College of Cardiology/American Heart Association 2017 (blood pressure threshold 130/80 mmHg) and International Society of Hypertension 2020 guidelines (threshold 140/90 mmHg). Results The mean age of the participants was 42.5 years (standard deviation: 15.1) and majority were male 195/302 (64.6%). Using American College of Cardiology/American Heart Association 2017 guidelines, 156/302 (51.7%) adults were newly diagnosed with hypertension compared to 68/302 (22.5%) newly diagnosed with hypertension using International Society of Hypertension 2020 guidelines. Only 23/302 (7.6%) were on treatment. Based on American College of Cardiology/American Heart Association 2017 guidelines, age ≥40 years and overweight/obesity were statistically significant predictors of hypertension (p < 0.05 for all) at multivariate analysis. Using the International Society of Hypertension 2020 guidelines, age ≥40 years predicted hypertension. Discussion The prevalence of hypertension is high among this urban adult population irrespective of the guidelines used, highlighting the need for hypertension prevention interventions.
