Sarcoid granulomas can be found in a wide range of diseases and differentiating sarcoidosis from a sarcoid-like reaction may be a challenge. We present a woman with erythematoviolaceous papulonodular lesions located on the ears where piercings were placed. A skin biopsy showing an infiltrate of sarcoid and focal tuberculoid granulomas did not exclude sarcoidosis. There was a slight increase in the level of angiotensin-converting enzyme. Systemic involvement due to sarcoidosis was excluded. Epicutaneous tests performed revealed a strong positive reaction to palladium and nickel, supporting the diagnosis of granulomatous contact dermatitis. There are only a few reports of granulomatous contact dermatitis to palladium with piercings as the source of sensitization. The formation of sarcoid granulomas can represent either a sarcoid-like reaction or a form of cutaneous sarcoidosis, and patch tests are essential in order to establish the diagnosis.

Abstract European Baseline Series (EBS) of haptens (contact allergens) is used throughout Europe, containing patch tests essential for the diagnosis of contact allergy (Tables I and II) [1]. Exposure to environmental allergens is constantly evolving and the EBS must be adapted accordingly in order to guarantee its relevance of the molecules tested, in particular by the addition of emerging allergens [1].

Background: Skin disorders among hairdressers and beauticians have been recognized as a common problem, owing to the prolonged exposure to a variety of irritants and allergens. Objectives: The aim was to study the prevalence of dermatoses among hairdressers and beauticians in Srinagar, the capital city of Kashmir, India. Methods: The study was conducted on 100 respondents from 57 salons and parlors located within the borders of the city. Detailed history taking and complete examination were performed during personal visits. Patch tests were performed with the Indian standard series on all participants. Results: The mean age of the study sample was 24.3 ± 6.3 years. 54% of the studied cases were found to suffer from one or more skin disorders. Hand eczema was the most commonly encountered dermatosis, observed in 32% of the cases. Three patterns of hand eczema were recognized: classic (14%), interdigital (6%), and threading (8%). Callosities were seen in 19% of the cases. We employed the term scissor nodule for the typical pattern of a callosity caused by the finger rings of scissors. Nail staining was encountered in 19%. The most common allergens leading to a positive patch test were found to be paraphenylenediamine (PPD) in thirteen cases, followed by a fragrance mix in nine, nickel sulfate in nine, thiuram in three, and formaldehyde in one. Conclusion: Skin disorders are common among hairdressers and beauticians. Raising awareness of these disorders and methods of their prevention among this group is imperative.

Introduction: Chromium has been responsible for occupational skin disease, both irritant and/or allergic contact dermatitis. In order to minimize this problem, regulatory measures limiting its use have been implemented. The impact of these regulations should be monitored over time. Material and Methods: A retrospective study from 2009 to 2018 was carried out in the Department of Dermatology to assess the temporal trend of chromium sensitization evaluated by patch testing with potassium dichromate 0.5% pet, particularly considering age, sex and its relationship with occupational activity. Results: 3277 individuals were studied, 2369 females (72.29%) and 908 males (27.71%). Of those 621 (18.95%) fulfilled criteria for occupational contact dermatitis. Chromium allergy was found in 118 (3.60%) patients, 64 females (54.4%) and 54 males (45.76%). We found that male patients were significantly more frequent (45.76% vs 27.03%; p<0.001), as well as occupational dermatitis (31.35% vs 18.47%; p<0.001), hand dermatitis (53.54% vs 23.52%; p<0.001) and age above 40 years (70.34% vs 56.76%; p=0.003). Discussion/Conclusion: The results show a high prevalence (3.60%) of chromium positive patch tests. In our study, there has not been a significant decrease in sensitization to chromium over the years. Interestingly cases classically related to this allergy (cement in the building industry) are no longer the main cause of the problem. We found other sources such as leather as a main cause of allergic sensitization, suggesting a change of the panorama.

Essential oils possess antiaging properties due to their antioxidant activity. This study aims to determine the antiaging activities of four main Indonesian essential oils and their irritation potential on the skin. The spot yeast and in vivo rat skin with UVB exposure methods were used to analyze the antiaging activity of essential oils on aging triggered by endogenous and exogenous factors, respectively. Meanwhile, patch tests and clinical evaluations were used for the skin irritation potential analysis. The antiaging activity results from the endogenous factor showed that the use of clove, patchouli, nutmeg, and citronella oils increased yeast viability at concentrations of 20, 40, 60, and 100 µg/mL, respectively. Furthermore, nutmeg, cloves, citronella, and patchouli oils decreased the wrinkle score on rat skin after UVB exposure (exogenous factor). The skin irritation potential results of patchouli, nutmeg, citronella, and clove oils were none (0), slightly (0.02), moderately (0.09), and very irritating (0.39), respectively.

Abstract Background The primary purpose of the present prospective study was to consecutively analyse the outcomes of the return to sports activity of patients with positive patch tests undergoing a medial mobile-bearing titanium niobium nitride (TiNbN) unicompartmental knee arthroplasty (UKA). The secondary purpose was to ascertain if a higher grade of physical activity leads to a reduction in the body mass index (BMI) of the patients. Material and methods Forty-one patients with positive skin patch tests were included in this prospective study. The clinical evaluation consisted of the University of California, Los Angeles (UCLA) activity scale and the High-Activity Arthroplasty Score (HAAS). Each patient was evaluated the day before surgery (T0), after 12.37 ± 0.70 months (T1), and on the day of the final follow-up, after 67.03 ± 18.2 months (T2). Furthermore, the BMI of each patient was analysed before surgery and during the final follow-up. Results The UCLA and HAAS mean preoperative values ranged from 3.68 ± 1.1.7 and 6.15 ± 0.76 to 6.1 ± 0.76 and 10.34 ± 1.3, respectively, at T1 (p < 0.0001) and to the final values of 6.34 ± 0.62 and 11.0 ± 8.9, respectively, at T2 (UCLA: T2 versus T1: p = 0.132; T2 versus T0: p < 0.0001; HAAS: T2 versus T1: p = 0.0027; T2 versus T0: p < 0.001). BMI ranged from a preoperative value of 27.97 ± 3.63 to a final value of 26.84 ± 3.11 (p < 0.0001). The only differences within the subgroups concerned patients with BMI ≥ 28, showing a superior HAAS at each follow-up (p < 0.05). A positive correlation was found between BMI and HAAS at T0 and T2 (p < 0.05). Conclusions This is the first study to evaluate the rate of the return to sports activities and change in BMI following hypoallergenic UKA. The majority of patients reduced their weight following UKA and improved their physical activity, showing outcomes that were comparable to the standard cobalt–chrome (CoCr) prostheses, regardless of gender, age, BMI and implant size. AbstractLime (Citrus aurantiifolia) is a plant belonging to the family of Rutaceae and to the genus Citrus. The fruit is widely used in the United States, Mexico, Southeast Asia, Latin America, but is increasingly widespread all over the world. It is used as a fresh fruit, in the preparation of foods, sweets and drinks and its oils are used in the cosmetic and pharmaceutical industry. The main adverse reactions to lime seem to be represented by contact dermatitis, allergic and phototoxic type. In the context of allergic forms, several allergens have been identified in the citrus family, the main one being limonene, but no noteworthy cross-reactivity has been identified. However, a case of fruit protein contact dermatitis has been described, showing sensitization to other fruits, such as kiwi, avocado, pineapple and apple. There are several molecules responsible for phototoxic reactions and mainly belonging to the coumarin and furocoumarins families. Reactions related to ingesting the fruit or inhaling pollen from the tree appear to be rare, as there are no known cases reported in the literature. The increasing diffusion of lime in Europe must pay attention to possible adverse reactions due to contact with this fruit, which seem destined to increase in future years. Further importance must be placed on patch tests and on the possibility of using alternative extracts to classic fragrance mixes.
