Dietetics has changed substantially; a mixed-methods project was undertaken to: (i) gauge interest in the profession history since 1993, (ii) identify preferred format(s), (iii) identify possible topics, and (iv) identify possible key informants. An online bilingual survey was conducted in 2018, with follow-up phone interviews among interested respondents. Survey content was organised as 12 major topics. Respondents were invited via a Dietitians of Canada (DC) newsletter, Facebook groups, and at the DC national conference. Survey data, including respondent-generated topics of interest and interview content, were descriptively analyzed. The online survey garnered 360 responses; 332 (92%) completed more than 10% of the survey and were interested in history. Detailed responses were analyzed (296 English; 36 French); 51 were interviewed. An online timeline was the most preferred format (79%). Review of the rise in technology and obesity, aging, supermarket registered dietitians (RDs), the local/organic movement, Practice-based Evidence in Nutrition (PEN), the changes in training models and scope of practice, public awareness of the profession, and advocacy and unique career paths were of most interest (≥ 50% of respondents). These results confirm interest in the recent history of the profession among RDs and provide guidance on preferred format and topics for further work.

Abstract Background In Japan, there has been a private-sector initiative to register dietitians in pharmacies. There is not yet an adequate amount of data on the attitudes of dietitians in pharmacies regarding their work. Objectives To assess the attitudes of pharmacists and dietitians working in community pharmacies in Japan about their work, and particularly about demonstrating expertise and overall job satisfaction. Methods We administered a web-based questionnaire to pharmacists and dietitians working in pharmacy branches with registered dietitians at two pharmacy chains that agreed to cooperate. We used our own 7-item questionnaire and compared the scores for each item between pharmacists and dietitians. Results A total of 61 persons (22 pharmacists and 39 dietitians) answered the questionnaire. The item with the highest mean score (standard deviation) for pharmacists was "I feel that I am helpful to patients," at 3.50 (0.74), and for dietitians it was "I have colleagues at my workplace whom I can talk to when I have a problem," at 3.51 (1.00). Conversely, the items with the lowest mean score (standard deviation) were "I am satisfied with my current job" for pharmacists, at 3.14 (0.83), and "I can grow sufficiently as a professional" for dietitians, at 2.41 (0.97). As a result of factor analysis, the seven-item question could be divided into two factors: "demonstrating expertise" and "overall job satisfaction.” The factor-score for "demonstrating expertise" was significantly lower for dietitians than for pharmacists (p<0.01), but there was no significant difference in overall job satisfaction between the two professions (p=0.36). Further research is needed to understand why dietitians find “demonstrating expertise” difficult in pharmacies.

Carbohydrate counting is essential for well-controlled blood glucose in people with type 1 diabetes, but to perform it precisely is challenging, especially for Thai foods. Consequently, we developed a deep learning-based system for automatic carbohydrate counting using Thai food images taken from smartphones. The newly constructed Thai food image dataset contained 256,178 ingredient objects with measured weight for 175 food categories among 75,232 images. These were used to train object detector and weight estimator algorithms. After training, the system had a Top-1 accuracy of 80.9% and a root mean square error (RMSE) for carbohydrate estimation of &lt;10 g in the test dataset. Another set of 20 images, which contained 48 food items in total, was used to compare the accuracy of carbohydrate estimations between measured weight, system estimation, and eight experienced registered dietitians (RDs). System estimation error was 4%, while estimation errors from nearest, lowest, and highest carbohydrate among RDs were 0.7, 25.5, and 7.6%, respectively. The RMSE for carbohydrate estimations of the system and the lowest RD were 9.4 and 10.2, respectively. The system could perform with an estimation error of &lt;10 g for 13/20 images, which placed it third behind only two of the best performing RDs: RD1 (15/20 images) and RD5 (14/20 images). Hence, the system was satisfactory in terms of accurately estimating carbohydrate content, with results being comparable with those of experienced dietitians.

The low-saturated fat (Swank) and modified Paleolithic elimination (Wahls) diets have shown promise for MS symptoms; however, due to their restriction of specific foods, inadequate intake of micronutrients is concerning. Therefore, as part of a randomized trial, weighed food records were collected on three consecutive days and were used to evaluate the intake of micronutrients among people with relapsing remitting MS adapting these diets. After randomization to either the Swank or Wahls diets, diet education and support was provided by registered dietitians at baseline and throughout the first 12 weeks of the intervention. Usual intake of each micronutrient was estimated and then evaluated with the EAR-cut point method. At 12 weeks, the Swank group had significant reductions in the proportion with inadequate intake from food for vitamins C, D, and E, while the Wahls group had significant reductions for magnesium and vitamins A, C, D, and E. However, the proportion with inadequate intake significantly increased for calcium, thiamin, and vitamin B12 in the Wahls group and for vitamin A in the Swank group. Inclusion of intake from supplements reduced the proportion with inadequate intake for all micronutrients except calcium among the Wahls group but increased the proportion with excessive intake for vitamin D and niacin among both groups and magnesium among the Swank group. Both diets, especially when including intake from supplements, are associated with reduced inadequate intake compared to the normal diet of people with relapsing remitting MS.

Abstract Background Registered dietitians are rarely employed at community pharmacies in Japan, even though dietetic advice might benefit some patients. Objective To clarify the present status of dietetic consultation provided by registered dietitians and their collaboration with pharmacists in community pharmacies. Methods We conducted a cross-sectional questionnaire-based survey of pharmacists and registered dietitians who work in community pharmacies. The surveyed items were: frequency of dietetic consultation, awareness of one’s knowledge and ability to conduct dietetic consultation, concerns, pharmacists’ recognition of the need for nutritional support at community pharmacies, and cooperation between registered dietitians and pharmacists. Results Sixty-six registered dietitians, 53 pharmacists in pharmacies with registered dietitians/dietitians, and 110 pharmacists in pharmacies without registered dietitians/dietitians responded. The frequency of dietetic consultation regarding obesity and hypertension was significantly higher for registered dietitians than for pharmacists. The ability to conduct dietetic consultation regarding diseases/conditions such as kidney disease not requiring dialysis, hyperuricemia, gout, obesity and hypertension was also significantly higher for dietitians than pharmacists. More than 70% of pharmacists recognized the importance of nutritional support at community pharmacies, while 56.1% of registered dietitians noted that they were not able to fully utilize their occupational abilities. Registered dietitians were divided into two groups: registered dietitians who answered that they were able to utilize their occupational abilities and those that answered they were not. The former group was more likely to ask pharmacists about patients’ medication for dietetic consultation and to be asked to provide dietetic consultation to patients. The latter group was more likely to find difficulty in scheduling dietetic consultation. Conclusion Our results suggest that registered dietitians in community pharmacies have a greater explanatory ability than pharmacists concerning nutritional and dietary management for patients. It may be important for pharmacists to improve cooperation with registered dietitians by providing more opportunities for dietetic consultation.

The National Collegiate Athletic Association passed legislation in 2014 to allow for unlimited meals and snacks for college athletes. This policy change created a need for full-time registered dietitians (RDs) to ensure student-athletes are properly fueled and monitor their dietary habits. RDs are now considered to be a key member of the college athlete holistic care team, and this has created a unique professional niche for RDs. There is a lack of research exploring the experiences and challenges of this emerging profession in college athletic departments. Nine RDs working in NCAA Division I athletic departments participated in semi-structured interviews to examine their career experiences. Three themes were constructed from the data analysis: (a) professional transition; (b) nutritional education; and (c) (dis)respect. The respondents discussed their participation in sport or volunteer experience was what pushed them to pursue a career as a sports dietician. One of the primary functions of their position is to build trust with the student-athletes and provide them with nutritional education so they will develop healthy eating habits. The respondents also struggled with a lack of acceptance of the importance of their position on the interprofessional care team. Some had positive experiences with administrators, but most had negative experiences and a general lack of understanding of the professional expertise they had.
