Plasmodium vivax malaria elimination requires radical cure with chloroquine/primaquine. However, primaquine causes hemolysis in glucose-6-phosphate dehydrogenase-deficient (G6PDd) individuals. Between February 2016 and July 2017 in Odisha State, India, a prospective, observational, active pharmacovigilance study assessed the hematologic safety of directly observed 25 mg/kg chloroquine over 3 days plus primaquine 0.25 mg/kg/day for 14 days in 100 P. vivax patients (≥ 1 year old) with hemoglobin (Hb) ≥ 7 g/dL. Pretreatment G6PDd screening was not done, but patients were advised on hemolysis signs and symptoms using a visual aid. For evaluable patients, the mean absolute change in Hb between day 0 and day 7 was −0.62 g/dL (95% confidence interval [CI]: −0.93, −0.31) for males (N = 53) versus −0.24 g/dL (95%CI: −0.59, 0.10) for females (N = 45; P = 0.034). Hemoglobin declines ≥ 3 g/dL occurred in 5/99 (5.1%) patients (three males, two females); none had concurrent clinical symptoms of hemolysis. Based on G6PD qualitative testing after study completion, three had a G6PD-normal phenotype, one female was confirmed by genotyping as G6PDd heterozygous, and one male had an unknown phenotype. A G6PDd prevalence survey was conducted between August 2017 and March 2018 in the same region using qualitative G6PD testing, confirmed by genotyping. G6PDd prevalence was 12.0% (14/117) in tribal versus 3.1% (16/509) in nontribal populations, with G6PD Orissa identified in 29/30 (96.7%) of G6PDd samples. Following chloroquine/primaquine, notable Hb declines were observed in this population that were not recognized by patients based on clinical signs and symptoms.

Background and Hypothesis:  Social determinants of health (SDOH) directly affect health outcomes and indirectly limit access to resources needed to maintain individual health. In an effort to address the negative impacts of SDOH on the urban communities of Indianapolis, four congregations have employed Site Connectors to directly form relationships with neighbors, learn about their health journey, and connect them to resources addressing expressed needs. It is our hypothesis that through research of similar models and discussion with community partners, an optimized model for fostering relationships and assessing the health and social needs of neighbors can be developed for use by the Site Connectors. Project Methods: A search was performed for examples of health and social needs assessments utilized by established care providers, and thirteen were identified. The items within these assessments were then organized into six groups based on SDOH topic (ie. Housing, Transportation, etc.). Additionally, six interviews with community partners performing similar work were conducted utilizing a fixed set of questions. Results: It was determined that the best format for our assessment would not be a survey, as in the example assessments, but rather a visual aid resembling a concept map. This model lends itself more to the nature of relationship-building by guiding Connector-Neighbor conversations rather than dictating them, with three starter questions at the center and six offshoots covering each of the SDOH topics. A post-encounter checklist was also developed for Connectors to retroactively record priority items from their conversations. Potential Impact: It is yet to be determined whether our model will be useful in practice, as the Connectors begin their work in August. However, it is our hope that we have developed a novel format for assessing needs that more holistically addresses the impacts of SDOH through respecting the vulnerability and energy required for relationship-building.

Abstract Aims A discharge summary is a permanent record of a patient’s hospital visit and the primary means of handover between care providers. Studies show they often lack precision and omit important information. This may compromise quality and continuity of care yet they are frequently written by the most junior clinicians on a ward with little guidance or formal education on how to write one. The aim of this study was to develop some specific guidelines to improve the quality of discharge summaries in a busy neurosurgical unit. Methods A survey was designed to identify the challenges faced by junior medical staff in writing discharge summaries. The essential components of a good neurosurgical discharge summary were identified by group of senior neurosurgeons. Summaries were retrospectively audited against these components. We then designed a simple visual aid and placed it above computer stations in the junior doctors’ offices. Formal departmental teaching session followed. After three months we re-audited the discharge summaries retrospectively to measure any effect of our intervention. Results Half of the neurosurgical team rated summaries as below expectations. Challenges included poor ward round documentation and a lack of clear expectations regarding structure and essential components. After the intervention, ward round documentation and discharge summary quality improved dramatically. Conclusions Although various recommendations about writing good discharge summaries exist, they are generally vague and not specific to neurosurgical practice. The development of a simple specialty specific discharge summary guide can improve discharge summary quality and should be encouraged in all specialties.

The overall goal of this study was to employ direct-to-consumer advertisements (DTCAs) as a teaching tool in a Doctor of Pharmacy (PharmD) curriculum. The objectives of this pilot study were to investigate the following questions: 1. Do DTCAs generate student curiosity about the advertised drug and associated disease? 2. Can DTCAs help students understand and reinforce various pharmacological aspects of the drug? 3. How do students perceive DTCAs? A DTCA-based teaching tool was employed in a pharmacology course taken by P2 (second professional year) PharmD and final year (U4) Bachelor of Science (BS) in Pharmacology–Toxicology students. A voluntary online survey was administered to students to determine the effectiveness of this tool. Survey data were analyzed quantitatively and qualitatively. 70–85% of responding students indicated that this teaching tool was an effective visual aid for learning pharmacology and correlating the drug to disease state, mechanism of action, and adverse effects. Moreover, themes identified from the qualitative analysis suggest that this teaching tool may be useful to enhance patient counseling skills in students. The initial implementation of this DTCA-based teaching tool proved to be successful, and a similar approach can be easily implemented in other pharmacotherapy and laboratory courses. Further studies are needed to determine if this approach can improve patient counseling skills.

AbstractThe description of the “Nine Provinces” (Jiu zhou 九州) found in the Rong Cheng shi 容成氏 (Mister Rong Cheng?, late fourth century b.c.e.) manuscript from the Shanghai Museum Bamboo Slips Collection (Shangbo cangjian 上博藏簡) is the only manuscript version of it known to date. Its discovery immediately raised the question of its relation to the cluster of descriptions on the “Nine Provinces” transmitted from the late Warring States to the early Western Han periods. There is general consensus that the manuscript description of the “Nine Provinces” has close affinity with the transmitted descriptions, as well as with a wide spectrum of transmitted early Chinese texts in general. It is distinguished by the eclectic combining of known spatial concepts, rather than manifesting any radically new or specifically Chu traits. In this study I reassess this impression with respect to the reference to the Han River in the manuscript, which up to now has been noted only in passing as an unsolved puzzle. I argue that the Han River is referred to here as the central axis that divides terrestrial space into southern and northern halves, something that implies a shifting of the mapped area to the South and thus conveys a Chu view of space. Together with philological analysis of the descriptions of terrestrial space, I apply an innovative method of investigation of these descriptions through landmarks, using as a visual aid traditional Chinese historical maps. In addition, I explore the predominance of waters as the distinguishing feature of the representation of terrestrial space in the Rong Cheng shi manuscript and demonstrate its difference from the structuring of terrestrial space proceeding from mountains to waterways to be seen in the majority of transmitted early Chinese texts.

This research explores Algerian writer Kamel Daoud’s 2018 non-fictional text, Le peintre dévorant la femme. The text addresses questions relating to religious extremism, the meaning of art, death and eroticism, and the relationship between l’Occident and l’Orient through the visual aid of Picasso’s 1932 Année érotique. Central to this research is the notion of the hybridized public intellectual (Daoud) entering hybridized public spheres (Franco-Algerian and beyond). The consequences of operating within a plural readership suggest that Daoud, subconsciously or not, speaks to particular sectors of his western-French audience more so than the Muslim-Algerian ones, risking an imbalance. This research unpicks how Daoud negotiates the relationship between aesthetics and politics in his non-fictional writing, showing how his public move to an essai in 2018 can be read as facilitating a conversation with more a bourgeois, and potentially more republican, French audience. It also analyses Daoud’s representations of Picasso, Paris, the museum, and the gendered body in western and Muslim societies. By doing so, it attempts to highlight how although Daoud appears to offer a ‘double-edged’ critique of Algeria since independence and French neo-colonialism, his tendency to make generalizations about Islam sometimes unwittingly plays to French (and more widely, western) Islamophobic assumptions.

Despite being a cornerstone of the science of behavior analysis, researchers and practitioners often rely on tradition and consensus-based guidelines, rather than empirical evidence, to make decisions about single-case designs. One approach to develop empirically-based guidelines is to use Monte Carlo simulations for validation, but behavior analysts are not necessarily trained to apply this type of methodology. Therefore, the purpose of our technical article is to walk the reader through conducting Monte Carlo simulations to examine the accuracy, Type I error rate, and power of a visual aid for AB graphs using R Code. Additionally, the tutorial provides code to replicate the procedures with single-case experimental designs as well as with the Python programming language. Overall, a broader adoption of Monte Carlo simulations to validate guidelines should lead to an improvement in how researchers and practitioners use single-case designs.
