Ego-documents are at the heart of historical sociolinguistics. Contrary to what a label such as ego-document may suggest, Early and Late Modern ego-documents constitute a heterogenous group of genres comprising, among others, private letters, diaries and travel journals. Empirical studies have shown that there are important linguistic differences between private letters on the one hand, and diaries/journals on the other. The latter often seem surprisingly standard-like or formal. Theoretical models of register variation and conceptual orality can partially explain the differences, without however offering a full explanation of the surprising formality of diaries/journals. We argue that it is crucial to take into account recent work by social historians concerning diaries/journals in the eighteenth and nineteenth centuries. Diary-writing was an inherently reflexive practice allowing authors to reflect on their lives, and to create a textually fixed point of reference. Authors of diaries had a variable and multilayered audience in mind of known and unknown readers. We introduce the observee’s paradox: while creating private texts for themselves in which they were their own observers and observees, authors of diaries also reckoned with unknown readers in a possibly distant future, which prompted them to shift into a more formal or standard-like register.

Abstract Background Spinal anaesthesia (SA) is the preferred anaesthesia technique for Caesarean section. Hypotension and bradycardia are the most common side effects encountered and are more pronounced in pregnant patients, the incidence being as high as 52.6% and 2.5% in normal patients. The occurrence of hypotension can be dangerous as it compromises placental circulation and can have a detrimental effect on the foetus. Objectives The purpose of this study is to assess the efficacy of ondansetron versus dexamethasone in decreasing incidence of spinal induced hypotension in parturient undergoing cesarean surgery. Patients and Methods This study was conducted at Ain Shams University Hospitals between March 2019 till September 2019. After approval is obtained from the research ethics committee of faculty of medicine, Ain Shams University & patients’ informed consents consent from each patient after full explanation of the procedure possible side effects and complications., 75 healthy parturient, (ASA)physical status I and II undergoing elective cesarean delivery under spinal anesthesia. Results 8 mg ondansetron is more effective than 8 mg dexamethasone in the attenuation of post spinal hypotension. Also, that Dexamethasone 8 mg was as effective as ondansetron 8 mg in providing a simple, safe, cheap, and effective intra and postoperative nausea and emesis prevention method with the advantage of being cheaper decreasing the economic burden. Conclusion The present study demonstrated that, among patients who received spinal anesthesia with bupivacaine and fentanyl as adjuvant for elective for cesarean section, prophylactic intravenous 8 mg ondansetron compared to dexamethasone significantly decreases hypotension, HR fluctuation, yet the need for rescue doses for ephedrine were significantly higher in ondansetron.

The US Geological Survey’s ShakeMap is used domestically and globally for post-earthquake emergency management and response, engineering analyses, financial instruments, and other decision-making activities. Recent developments in the insurance, reinsurance, and catastrophe bond sectors link payouts of potentially hundreds of millions of dollars to ShakeMap products. Similarly, building codes, post-earthquake building damage forensic evaluations, and geotechnical evaluations often rely on estimated peak response-spectral values for site-specific evaluations that may lead to costly analyses, retrofits, or other expenditures. Given such activities, financial, engineering, and other technical users demand processing specifications and a metadata trail for actuarial, escrow, and forensic purposes for each significant earthquake. Recent inquiries include how and why maps change with time, how to interpret metadata, and how to obtain the creation and update history of various map layers. Similarly, the collection of ShakeMap scenarios and historical ShakeMaps—either created in earlier versions or rerun as part of the latest version of the ShakeMap Atlas—warrant a full explanation of the inputs, processing, and archiving given their contribution to fragility curve development and loss model calibration. For these reasons, in addition to event-specific ShakeMap metadata and a comprehensive online ShakeMap Manual, we have crafted this practice paper to answer several of the most frequently asked technical questions. We also describe an application programming interface (API) for accessing site-specific shaking metrics and their uncertainties for earthquake forensic purposes in a consistent fashion. In all, we describe the advantages of employing ShakeMaps for these critical purposes as well as describe their limitations and uncertainties, offering an extensive set of instructions and disclaimers that can be referenced by ShakeMap users.

Breast cancer is the most common female malignancy in Taiwan, while conventional clinical and pathological factors fail to provide full explanation for prognostic heterogeneity. The aim of the study was to evaluate the feasibility of targeted sequencing combined with concurrent genes signature to identify somatic mutations with clinical significance. The extended concurrent genes signature was based on the coherent patterns between genomic and transcriptional alterations. Targeted sequencing of 61 Taiwanese breast cancers revealed 1036 variants, including 76 pathogenic and 545 likely pathogenic variants based on the ACMG classification. The most frequently mutated genes were NOTCH, BRCA1, AR, ERBB2, FANCA, ATM, and BRCA2 and the most common pathogenic deletions were FGFR1, ATM, and WT1, while BRCA1 (rs1799965), FGFR2 (missense), and BRCA1 (rs1799949) were recurrent pathogenic SNPs. In addition, 38 breast cancers were predicted into 12 high-risk and 26 low-risk cases based on the extended concurrent genes signature, while the pathogenic PIK3CA variant (rs121913279) was significantly mutated between groups. Two deleterious SH3GLB2 mutations were further revealed by multivariate Cox’s regression (hazard ratios: 29.4 and 16.1). In addition, we identified several significantly mutated or pathogenic variants associated with differentially expressed signature genes. The feasibility of targeted sequencing in combination with concurrent genes risk stratification was ascertained. Future study to validate clinical applicability and evaluate potential actionability for Taiwanese breast cancers should be initiated.

AbstractIn this paper, we explore different possible explanations for research misconduct (especially falsification and fabrication), and investigate whether they are compatible. We suggest that to explain research misconduct, we should pay attention to three factors: (1) the beliefs and desires of the misconductor, (2) contextual affordances, (3) and unconscious biases or influences. We draw on the three different narratives (individual, institutional, system of science) of research misconduct as proposed by Sovacool to review six different explanations. Four theories start from the individual: Rational Choice theory, Bad Apple theory, General Strain Theory and Prospect Theory. Organizational Justice Theory focuses on institutional factors, while New Public Management targets the system of science. For each theory, we illustrate the kinds of facts that must be known in order for explanations based on them to have minimal plausibility. We suggest that none can constitute a full explanation. Finally, we explore how the different possible explanations interrelate. We find that they are compatible, with the exception of explanations based on Rational Choice Theory and Prospect Theory respectively, which are incompatible with one another. For illustrative purposes we examine the case of Diederik Stapel.

Exemption clauses have traditionally been frowned upon because they have been misused, often to the detriment of consumers, and the courts have responded by repeatedly looking for ways to cut them down. In recent years the Unfair Contract Terms Act 1977 has given the courts much stronger powers and the Unfair Terms in Consumer Contracts Regulations have strengthened the position of consumers. These statutory controls have recently been radically overhauled in the Consumer Rights Act 2015 and this chapter provides a full explanation of these complex developments.

The theoretical assumption of Sustainable Development (SD) is gradually beginning to be applied in various industrial segments. One on the segments is industrial energy, which is significantly utilized and intuitively comprehended even though there is no full explanation of its phenomena. The main aim of this research contribution is to discuss the theoretical assumptions of SD and Industrial Energy (IE). This paper considers critically all the issues of permanence since it incorporates the environmental and social needs for ecological development. In addition, overall characterized, the major segments of these theoretical assumptions are based on critical analysis of the literature source.

Crossover (CO) is a constraint on anaphoric dependencies, according to which, quantifier scope can feed pronominal anaphora unless the anaphoric expression precedes the quantifier. We demonstrate that effects reminiscent of CO arise with presupposition as well, and propose to generalise CO as follows: Projective content (quantifier scope, presupposition projection, etc.) feeds semantic dependencies (pronominal anaphora, presupposition satisfaction), unless the semantically dependent expression precedes the trigger of the projective content. We call this generalisation, Generalised Crossover (GCO). Although we cannot offer a full explanation for GCO in this paper, we will discuss its implications for recent theories of CO.

Recently, owing to the discovery of graphene, porous carbons experienced a revitalization in their explorations. However, nowadays, the focus is more on search for suitable energy advancing catalysts sensing, energy storage or thermal/light absorbing features than on separations. In many of these processes, adsorption, although not emphasized sufficiently, can be a significant step. It can just provide a surface accumulation of molecules used in other application-driving chemical or physical phenomena or can be even an additional mechanism adding to the efficiency of the overall performance. However, that aspect of confined molecules in pores and their involvement in the overall performance is often underrated. In many applications, nanopores might silently advance the target processes or might very directly affect or change the outcomes. Therefore, the objective of this communication is to bring awareness to the role of nanopores in carbon materials, and also in other solids, to scientists working on cutting-edge application of nonporous carbons, not necessary involving the adsorption process directly. It is not our intention to provide a clear explanation of the small pore effects, but we rather tend to indicate that such effects exist and that their full explanation is complex, as complex is the surface of nanoporous carbons.

A typical scientist has no responsibility other than to explain how a natural event occurred. However, when a philosopher asks about the conditions under which a scientist’s explanation is true, he is, in fact, raising an ultimate question, the concept which Karl Popper used for the first time. Answering this question requires that no elements are neglected in the explanation, and no significant factors in the explanation are overlooked. In other words, in explainingphenomenon, at any level of its explanation, there should be no remainder. These requirements can be achieved through the full explanation. In the present article, by drawing on concepts such as theory-ladenness of observation, underdetermination of theory by evidence, and the role of models and metaphors in developing a scientific theory, it is illustrated thatcomplete explanation includes both a scientific explanation and personal explanation. A personal explanation comprises mental properties such as belief, desire, and intention, which are irreducible to physical properties. Therefore, we cannot provide a personal explanation while restricting ourselves to scientific methods. Consequently, it is argued in this article first, the personal explanation is irreducible to a scientific explanation. Second, the personal explanation is inevitable in order to provide a full explanation. Third, (methodological) naturalists claim that the ultimate judgment of what is natural and unnatural is possible only by scientific inquiry. Finally, accepting these three premises entails the inability of a naturalist to answer ultimate questions.
