Introduction: Ectopic pregnancy is a pregnancy problem when the embryo attaches itself outside the uterus. To limit the development of treatments to enhance the outcome. Conclusion: Although it is not uncommon for an ectopic pregnancy to exceed the first trimester, it does occur occasionally. As a result, in any emergency, abdominal surgery during pregnancy, Today, early intervention saves lives and reduces morbidity, but ectopic pregnancy still kills 4 to 10% of pregnant women and results in a higher ectopic pregnancy rate following pregnancy.

Introduction: Extra uterine pregnancy is a first-trimester pregnancy problem that affects 1.3–2.4 percent of all pregnancies. The key signs of the ectopic pregnancy are abdominal discomfort and vaginal bleeding along with Sharp, dull, or cramping pains may be experienced around 50% of the women who are suffering from ectopic pregnancy. The neglected ectopic pregnancy may results in the fallopian tube can burst, internal abdominal bleeding, shock, and serious blood loss and later complication is septicemia. As a health care professional it’s very important to manage certain complication with medical and surgical management. Main Symptoms and/or Important Clinical Findings: A 20 years old female with post operative case of Exploratory Laparotomy admitted in A.V.B.R.H. on 14/02/2021.with chief complaints of the after undergone certain investigation she  has diagnosed as Exploratory Laparotomy with septicemia as post-op complication. The Main Diagnoses, Therapeutic Interventions, and Outcomes: A 20 years old female with post-operative Exploratory Laparotomy for Ruptured Ectopic Pregnancy operated case with septicemia, with chief complaints of acute abdominal pain, and vaginal bleeding the doctors manage her initially with I.V. fluids, antibiotics, Zonac suppository and adequate nursing management. Nursing Perspectives: The nursing interventions initiated for managing present case are fluid replacement therapy, monitoring vital sign per hourly, monitoring the CBC reports and other investigations like USG abdomen, blood glucose levels. Maintained TPR Chart, I/O Charting, abdominal girth charting. Conclusion: In the Present case the patient of A  20 year old female with post operative exploratory laparotomy  for  ruptured ectopic pregnancy with  septicemia it has been managed with  the therapeutic and surgical   treatment ,right now the patient condition then  patient has discharged on dated 22/2/21.

Abstract Aim The Pan-surgical Intercollegiate eLogbook represents the world’s largest repository of training and operative data. ASiT have recently published concerns from a nationwide survey demonstrating widespread inaccuracies in eLogbook operative case reporting. This study aimed to add to this work by identifying opportunities to improve the way eLogbook data is used and interpreted. Method An online, self-administered, voluntary questionnaire was created on Google Forms (Google Inc, Menlo Park, CA) to explore patterns of use of the eLogbook, and highlight areas for improvement. The survey was disseminated to surgical trainees across the UK and Ireland using specialty networks and deanery training forums. Results Fifty trainees across the country from all ten surgical specialties completed the questionnaire. 54% of respondents reported using the Mobile eLogbook Application to record procedural data and 62% reported doing this on the same day of the procedure. 52% reviewed their eLogbook data less frequently than monthly. Just 16% thought the current presentation of eLogbook data were ‘very useful’ or ‘useful’. 72% were happy to record more detailed procedural data. 88% were interested in anonymously comparing procedural data with peers, and four-fifths reported that hospital or regional level data would influence their application choices. 96% reported that access to trainer’s supervision data would influence planning of their daily clinical activity. Conclusions This survey has identified several solutions for improving engagement and utility of the Intercollegiate eLogbook. Trainees report a desire to access enhanced analysis of individual, local, regional and national operative data.

Background General surgery residents (GSRs) must develop operative autonomy skills to practice independently after graduation. We aim to investigate perceived confidence and operative autonomy of GSR physicians in order to identify and address influential factors. Methods A 28-question anonymous online survey was distributed to 23 United States general surgery residency programs. Multivariable logistic regression was used for calculating the adjusted odds ratio (aOR) for binary outcomes. Significance was defined as P-values ≤ .05 or 95% confidence intervals (CIs) >1 or <1. Results There were 120/558 (21.5%) GSR respondents. General surgery residents with >200 overall operative case volume reported significantly higher confidence with minor cases (P = .05) and major cases (P = .02). General surgery residents that performed both minor and major surgeries reported higher confidence with minor cases at 85.7% compared to GSRs that performed mostly minor surgeries (64.7%) and mostly major surgeries (62.5%). General surgery residents who performed >50 minor surgeries during their PGY 1 and 2 were less confident with major cases than GSRs who performed <50 minor surgeries (aOR: 19.98, 95% CI: 1.26, 318). General surgery residents from community teaching hospitals reported higher confidence with major and minor cases than GSRs from university teaching hospitals and combined programs. Conclusion Increased case volume, predominant case type, early surgical experience during PGY 1 and 2 years, and training at community teaching hospitals were identified as the most important factors that positively influence perception of operative confidence and autonomy among GSRs. These may have important implications in the development of future surgeons.

Within the Assembly to Order (ATO) production strategy, the common approach is to produce the parts to assemble with a Push-Make to Stock policy.In recent decades, the effects of the modern Just in Time (JIT) moved to a Pull-Make to Order policy. Assembled parts characterized by wide variety and huge storage space utilization are critical, and a proper Push/Pull production policy definition is required. An appropriate balance of storage space utilization and setup times leads to the optimization of the production policy. The aim of this paper is to define a bi-objective mathematical optimization model to assign the most suitable production policy to the parts within the production mix in an ATO industrial context. A numerical simulation and an operative case study showcases the model application, proving the industrial relevance of this research.
