Objective: To explore the relationship between white matter changes and olfactory ability among patients with mild cognitive impairment (MCI) and to develop a tool to predict the development of Alzheimer’s disease among patients with MCI.Methods: The Montreal Cognitive Assessment (MoCA) was used for cognitive assessments, and the 70% isopropanol test paper was used to evaluate olfactory function. Tract-based spatial statistics, based on the diffusion tensor imaging technology, were used to obtain relevant parameters, and behavioral and imaging results were compared between patients with MCI (n = 36) and healthy older adults (n = 32).Results: The olfactory ability of MCI patients was lower overall, which was positively correlated with the MoCA score. Fractional anisotropy (FA) changes significantly of all parameters. Lower FA regions were mainly located in the corpus callosum, the orbitofrontal gyrus, and the left occipital lobe. The olfactory score was significantly correlated with the FA value of the orbitofrontal gyrus. Fibrous connections in several brain regions, such as the entorhinal cortex, were stronger in patients with MCI.Conclusion: The olfactory ability of MCI patients in our group was positively correlated with the neuropsychological scale results. Impairment in olfactory function was superior to memory deficits for predicting cognitive decline among cognitively intact participants. The fibrous connections in several brain regions, such as the entorhinal cortex, were higher in patients with MCI, which suggested that there may be a compensatory mechanism in the olfactory pathway in MCI patients. The decline in olfactory function may be a significant and useful indicator of neuropathological changes in MCI patients and an effective marker for the development of cognitive decline and dementia.

Individuals in healthy romantic relationships gain significant benefits to their psychological wellbeing and physiological health. Notably, the majority of relationship research has focused on how adult attachment influences these relationship outcomes while the role of olfaction remains an emerging research focus. The aim of the current study was to bring together these seemingly unrelated factors–attachment and olfaction–in an online quasi-experimental design. The participants were 401 undergraduate students, predominantly females, ranging in age from 17 to 70 years. Participants completed a battery of questionnaires that evaluated their attachment tendencies, olfactory ability and experiences in romantic relationships. Results indicated that attachment insecurity, across both attachment anxiety and avoidance, was associated with decreased olfactory functioning for females. These findings provide preliminary evidence that olfaction is related to romantic relationship maintenance and suggests that body odors could be fundamental for evoking the attachment system. These findings also elicit enticing new avenues of research which can assist psychologists to provide targeted treatments to individuals with olfactory deficits and insecure attachment tendencies.

Epilepsy is a debilitating and potentially life-threatening neurological condition which affects approximately 65 million people worldwide. There is currently no reliable and simple early warning seizure-onset device available, which means many people with unstable epilepsy live in fear of injury or sudden death and the negative impact of social stigmatization. If anecdotal claims that untrained dogs anticipate seizures are found to be true, they could offer a simple and readily available early warning system. We hypothesized that, given the extraordinary olfactory ability of dogs, a volatile organic compound exhaled by the dog’s epileptic owner may constitute an early warning trigger mechanism to which make dogs react by owner-directed affiliative responses in the pre-seizure period. Using 19 pet dogs with no experience of epilepsy, we exposed them to odours that were deemed to be characteristic of three seizure phases, by using sweat harvested from people with epilepsy. The odours were delivered to a point immediately under a non-epileptic and seated pet dog owner’s thighs. By altering the alternating odours emerging from sweat samples, captured before seizure, during a seizure and after a seizure, and two nonseizure controls, we were able to record the response of the 19 pet dogs. Our findings suggest that seizures are associated with an odour and that dogs detect this odour and demonstrate a marked increase in affiliative behaviour directed at their owners. A characteristic response of all 19 dogs to seizure odour presentation was an intense stare which was statistically significant, (p < 0.0029), across the pre-seizure, seizure and post-seizure phases when compared to control odours of nonseizure origin.

AbstractThe Delta Smelt (Hypomesus transpacificus) is a small, semi-anadromous fish native to the San Francisco Bay-Delta Estuary and has been declared as critically endangered. Their olfactory biology, in particular, is poorly understood and a basic description of their sensory anatomy is needed to advance our understanding of the sensory ecology of species to inform conservation efforts to manage and protect them. We provide a description of the gross morphology, histological, immunohistochemical, and ultrastructural features of the olfactory rosette in this fish and discuss some of the functional implications in relation to olfactory ability. We show that Delta Smelt have a multilamellar olfactory rosette with allometric growth. Calretinin immunohistochemistry revealed a diffuse distribution of olfactory receptor neurons within the epithelium. Ciliated, microvillous and crypt neurons were clearly identified using morphological and immunohistochemical features. The olfactory neurons were supported by robust ciliated and secretory sustentacular cells. Although the sense of smell has been overlooked in Delta Smelt, we conclude that the olfactory epithelium has many characteristics of macrosmatic fish. With this study, we provide a foundation for future research into the sensory ecology of this imperiled fish.

Importance Olfactory deficits are common in aging and associated with several conditions linked to inflammation. A few studies suggest that increased concentration of pro-inflammatory biomarkers may be related to olfactory deficits, but these associations are understudied in population-based samples. Objective To investigate the association between serum concentrations of C-reactive protein (CRP) and olfactory identification level as well as rate of change in aging. Methods We included 1,721 participants (mean age 70.5 years; 61.9% female) with at least two olfactory assessments across the 12-year follow-up. Baseline level and change in odor identification were estimated with linear mixed models as a function of CRP levels, derived from blood plasma at baseline. Results Results indicated a negative dose–response association between CRP level and odor identification scores at baseline, after adjustment for demographic, cognitive, health, and lifestyle factors. CRP levels ranging between 11 and 20 mg/L were significantly related to lower olfactory ability (β = −0.811, 95% confidence interval [CI] [−1.503 to −0.118]; p = .022). Likewise, CRP values above 20 mg/L were related to lower olfactory scores, an association that approached statistical significance (β = −0.996, 95% CI [−2.045 to 0.054]; p = .063). We found no associations between CRP and olfactory change ( ps > .368). Sensitivity analyses showed that associations between CRP and olfaction were confined to younger participants (age ≤72 years) and men ( ps < .034). Conclusions Our findings suggest a negative association between serum CRP levels and olfactory identification ability in aging that may be dependent on age and sex.

Abstract Introduction Several studies have suggested that chronic kidney disease (CKD) may be associated with olfactory impairment. However, to date, the impact of renal replacement therapies has only been partly defined. Methods We tested the olfactory function of 235 participants [50 kidney transplant recipients (KT), 49 hemodialyzed patients (HD), 30 peritoneal dialysis patients (PD), 51 patients with CKD not on dialysis (ND-CKD) and 55 healthy subjects (HS)] by the Sniffin’ Sticks test (Burghardt®, Wedel, Germany), including the sub-tests for the determination of odor threshold (T), odor discrimination (D), odor identification (I). Each subtest result was then summed up to a composite score, known as the TDI score. The Sino-Nasal Outcome Test-22 (SNOT22), Montreal Cognitive Assessment (MoCA) test and olfactory function Visual Analogue Scale (ofVAS) were also performed. Results The mean TDI score was significantly lower (and consistent with hyposmia), in HD, PD and ND-CKD compared to HS and KT (ANOVA p < 0.001). Similar results were observed in the I and D tests, and with the T score, though with regard to the latter, only in PD and ND-CKD patients. Multiple comparisons among groups demonstrated no significant differences between KT and HS. After adjustments for confounding factors, a significant linear association was found between both urea (β − 0.03, p < 0.003) and eGFR (β 0.08, p < 0.001) with TDI score. No significant association was observed between the TDI score and the ofVAS score (p = 0.293). Conclusions Olfactory impairment affects a large number of CKD patients in the pre-dialysis phase as well as those on dialysis. Kidney transplantation may reverse this condition with a possible positive impact on the quality of life and social behaviors/relationships. Graphic abstract

For years, the dog, man’s best friend, was the most widely employed scent-detector tool for civilian and military purposes. Recently, many studies highlighted the role of canine olfactory ability in the medical field, specifically in detecting different infectious, metabolic and neoplastic conditions. The objective of this literature review is to clarify the rationale behind dog’s ability to detect diseases, to assess the possible application for COVID-19 detection and to discuss the evidence available on the matter. Available evidence shows that properly trained disease-detector dogs are an efficient tool for identification of specific disease-associated volatile organic compounds marker profiles for a particular disease. And since COVID-19 positive persons have a specific volatilome different from non-infected persons, they can be recognised by the dogs, by sniffing different body fluids consequently aiding in the diagnosis of COVID-19. Possible applications of dogs as COVID-19 detectors will be an easy real-time mobile diagnostic aid with low cost and good performance. More evidence is needed to be able to describe standardised measures concerning the best fluid to test, testing procedure, time of possible detection according to disease evolution, risks associated with the dog exposure and to translate the good results in study setting into the real-life operational one.

Abstract Subjective hyperosmia refers to a self-reported olfactory ability that is superior to a normal, intact sense of smell (normosmia), and is associated with olfactory emotional experience. The current study used voxel-based morphometry to investigate the gray matter volume (GMV) in people with self-rated hyperosmia (subjective hyperosmia, SH, N = 18) in comparison to people with self-rated normal olfaction (subjective normosmia, SN, N = 14). Participants’ olfactory function were assessed by the extensive olfactory test battery, the “Sniffin’ Sticks” test. Within the predicted brain regions (regions-of-interest analyses), the SH participants showed larger GMV of the left hippocampus as compared to SN participants (FWE corrected p < 0.05). Further, the whole-brain search indicated that SH had larger GMV of the bilateral hippocampus, the right hypothalamus, the left precuneus, and the left superior frontal gyrus as compared to the SN group. ROI analyses showed positive correlations between the left hippocampal GMV and odor threshold or discrimination scores across all participants. In addition, the whole-brain analysis suggested that the self-rated olfactory ability was positively associated with GMV in the cerebellum, superior frontal gyrus and the precentral gyrus among SH participants. In conclusion, the current results suggest that SH was associated with increased GMV in several brain regions that were previously shown to be involved in the processing of cognitive aspects of odors.

Abstract The purpose of our cohort study was to quantify olfactory deficits in Coronavirus disease 2019 (COVID-19) patients using Sniffin’ Sticks and a pre-post design to evaluate olfactory recovery. Thirty adult patients with laboratory-confirmed mild to moderate forms of COVID-19 underwent a quantitative olfactory test performed with the Sniffin’ Sticks test (SST; Burghardt, Wedel, Germany), considering olfactory threshold (T), odor discrimination (D), and odor identification (I). Results were presented as a composite TDI score (range 1–48) that used to define functional anosmia (TDI ≤ 16.5), hyposmia (16.5 &lt; TDI &lt; 30.5), or functionally normal ability to smell (TDI ≥ 30.5). Patients also self-evaluated their olfactory function by rating their ability to smell on a visual analogue scale (Visual Analog Scale rating) and answering a validated Italian questionnaire (Hyposmia Rating Scale). Patients were tested during hospitalization and about 2 months after symptoms onset. During the hospitalization, the overall TDI score indicated that our cohort had impairments in their olfactory ability (10% was diagnosed with anosmia and more than 50% were hyposmic). Almost all patients showed a significant improvement at around 1 month following the first test and for all the parts of the SST except for odor identification. None of the subjects at 1 month was still diagnosed with anosmia. We also quantified the improvement in the TDI score based on initial diagnosis. Anosmic subjects showed a greater improvement than hyposmic and normosmic subjects. In conclusion, within a month time window and 2 months after symptoms’ onset, in our cohort of patients we observed a substantial improvement in the olfactory abilities.

Objective The symptoms and long-term sequelae of SARS-CoV-2 infection have yet to be determined, and evaluating possible early signs is critical to determine which patients should be tested and treated. The objective of this ongoing study is to evaluate initial and short-term rhinologic symptoms, olfactory ability, and general quality of life in patients undergoing SARS-CoV-2 testing. Study Design Prospective case-control. Setting Academic institute. Methods Adult patients tested for SARS-CoV-2 were prospectively enrolled and separated into positive and negative groups. Each participant completed 4 validated patient-reported outcome measures. The UPSIT (University of Pennsylvania Smell Identification Test) was distributed to patients who were SARS-CoV-2 positive. Results The positive group reported significantly decreased sense of smell and taste on the 22-item Sinonasal Outcome Test (SNOT-22) as compared with the negative group (mean ± SD: 3.4 ± 1.7 vs 1.2 ± 1.4, P < .001). The positive group had a much higher probability of reporting a decrease in smell/taste as “severe” or “as bad as it can be” (63.3% vs 5.8%) with an odds ratio of 27.6 (95% CI, 5.9-128.8). There were no differences between groups for overall SNOT-22 domain scores, PHQ-4 depression/anxiety (Patient Health Questionnaire−4), and 5-Level EQ-5D quality-of-life scores. Mean Self-MOQ (Self-reported Mini Olfactory Questionnaire) scores were 7.0 ± 5.6 for the positive group and 1.8 ± 4.0 for the negative group ( P < .001). The mean UPSIT score was 28.8 ± 7.2 in the positive group. Conclusion Symptomatic patients who are SARS-CoV-2 positive report severe olfactory and gustatory dysfunction via the Self-MOQ and SNOT-22 as compared with symptomatic patients testing negative.
