The oral cavity is a complex environment constantly exposed to antigens from food and the oral microbiota. Innate immune cells play an essential role in maintaining health and homeostasis in the oral environment. However, these cells also play a significant role in disease progression. This review will focus on two innate phagocytes in the oral cavity: macrophages and neutrophils, and examine their roles during homeostasis and disease development, with a focus on periodontal disease and cancer. Macrophages have a well-known ability to polarize and be activated towards a variety of phenotypes. Several studies have found that macrophages’ polarization changes can play an essential role in maintaining health in the oral cavity and contribute to disease. Recent data also finds that neutrophils display phenotypic heterogeneity in the oral cavity. In both cases, we focus on what is known about how these cellular changes alter these immune cells’ interactions with the oral microbiota, including how such changes can lead to worsening, rather than improving, disease states.

Teeth are unique and complex anatomical organs that can provide relevant data about a person's health, and play an important role in forensic medicine. Teeth are exposed to food, drinks, and the microbiota of the oral cavity; therefore, they have developed a high resistance to localized demineralization. Nevertheless, the continuous demineralization–remineralization cycle present in the oral environment can be influenced by stress, medication, mineralization agents, and other factors such as individual habits, especially diet. In this study, based on attenuated total reflectance Fourier-transform infrared spectroscopy (ATR-FTIR) spectra from tooth samples of 36 patients, several parameters were estimated: the crystallinity index (CI), the phosphate/amide I ratio, and the carbonate/phosphate ratio. In addition, in eight representative samples (six of the root of the tooth and two of the enamel area of the crown), additional characterization by X-ray powder diffraction (XRPD), scanning electron microscopy (SEM) and energy-dispersive X-ray spectroscopy (EDS) was conducted. From the FTIR data, it was observed that the highest CI values were found in patients who smoked. Further, in both root and crown samples, the intensity of the absorption band corresponding to PO43- increased in patients undergoing treatment with psychotherapeutic drugs. On the other hand, the intensity of the absorption band of the amide I group decreased with medical treatment and with the patient's biological age. Moreover, it was found that the remineralization process was more active in enamel than in the root due to direct contact with saliva. Regarding the results obtained from the X-ray powder diffractograms, exposure to psychotherapeutic drugs affected the definition of the peaks corresponding to hydroxyapatite, both in the crown and root samples. Concerning SEM results, qualitative differences in the stratification process in demineralized surfaces were observed, and EDS analyses showed some differences in the Ca/P ratio between pathological samples and control ones, but without clear patterns. The above techniques, in particular ATR-FTIR, showed promise for the investigation of the effect of changes produced in the hydroxyapatite structure in teeth and, consequently, to determine possible strategies in the diagnostic protocol.

We aimed to clarify the physical factors associated with the incidence of aspiration pneumonia in a community-based integrated care unit. This retrospective cohort study included 412 patients aged 65 years or older admitted to a community-based integrated care unit. A new diagnosis of aspiration pneumonia made by the attending physician based on physical examination, imaging findings, and blood test data after 48 h of admission was considered as an incidence of aspiration pneumonia. Basic patient information, activities of daily living, swallowing function, nutritional status, cognitive function, oral health-related factors, and energy intake were retrospectively investigated. We classified the patients into a pneumonia group and a non-pneumonia group, and examined the factors associated with the development of aspiration pneumonia. The mean age was 86.9 ± 8.1 years, and the pneumonia group comprised 49 participants. Comparison between the groups showed significant differences in oral environment, denture use, cognitive functional independence measure, and discharge to home. In multivariate logistic regression analysis, oral environment (odds ratio (OR) = 0.229, 95% confidence interval (CI): 0.070–0.753, p = 0.015) and use of dentures (OR = 0.360, 95% CI: 0.172–0.754, p = 0.007) were independently associated with aspiration pneumonia. Oral care and the use of dentures may be effective in preventing aspiration pneumonia.

Alloys in oral cavity always interact with dynamic oral environment, such as pH, temperature, salivary conditions, and dietary habits. Coffee can further decrease pH in the oral cavity. Thus, coffee may increase the release of metal ions that may lead to various health diseases. This study aimed to quantitively investigate the effect of brewed Robusta coffee on the nickel ion release and their morphological structures; Methods: 20 alloy specimens were divided into 4 groups and placed in solutions for 48 and 168 h: (1) distilled water, (2) artificial saliva, (3) Robusta coffee, and (4) mixture of artificial saliva and Robusta coffee. AAS, XRF, and SEM were used for examinations; Results: The release of Ni2+ was found in all groups and robust release were found in the coffee only and mixture of coffee + artificial saliva solution after 168 h. Likewise, SEM showed that internal oxidation was high after 168 h of immersion; Conclusions: The concentration of nickel increased in saliva of low pH due to brewed coffee. Though the release of these ions is still within the tolerable amount in human body, it should be realized that it usually lasts for months or years. Dentists should be cautious when using instructions for patients with a history of allergies especially for dietary with low acidity.

AbstractTwo approaches can be taken when designing properties of the native oxide layers formed on Ti-based biomedical materials: (i) changing the chemical composition of the substrate by adding biocompatible, valve alloying elements, and (ii) changing the microstructure of the substrate—especially its level of defectiveness—through large plastic deformation. However, especially in the aggressive fluoridated oral environment, it is still unknown what factor is more effective in terms of enhancing oxide layer protectiveness against biocorrosion: (i) the presence of valve alloying elements, or (ii) a high number of structural defects. To gain knowledge about the separate influence of both of these factors, surface properties were examined for commercially pure Ti and Ti–Nb–Ta–Zr alloy in microcrystalline state as well as after multiple-pass cold rolling, a process that can be readily scaled up to the industrial level. This study showed that while valve-alloying elements and structural defects individually have a beneficial effect on Ti oxide layer properties in fluoridated medium, they not have to act in a synergistic manner. These findings have to be taken into account when designing future Ti-based dental materials together with analyzing their mechanical performance with respect to mechanical strength and elastic properties.

Lingonberry (Vaccinium vitis ideae L.) is a low-bush wild plant found in the northern hemisphere. The berries are used in traditional medicine in Finland to treat oral yeast infections. General and oral effects of lingonberries on the microbiome and inflammation are reviewed. A brief introduction to oral microbiome symbiosis and dysbiosis, innate and adaptive immunity and inflammation are included, and special features in microbe/host interactions in the oral environment are considered. In vitro anticancer, antimicrobial, antioxidant, anti-inflammatory, and in vivo mouse and human studies are included, focusing on the symbiotic effect of lingonberries on oral and general health.
