It registers in the publication, that one of major is the problem of providing of safety of navigation, the successful decision of which conduces to the decline of harm of human life and environment. Paid attention, that sailing of maritime vessels in the straitened districts is considerably complicated by the navigation obstacles and intensive navigation, which create pre-conditions for the origin of emergency situations, thus sailing in the straitened districts is characterized by the fleeting change of navigation situation, that requires development of operative methods of estimation of danger of rapprochement and choice of safe maneuver of divergence. The high level of the use on the ship of information technologies determines expedience of computer realization of the developed methods of prevention of collision of vessels. It is specified, that in the conditions of intensive navigation there are the situations of dangerous rapprochement of ship with two targets at presence of navigation danger in the district of maneuvering. The analysis of the last achievements and publications is resulted on the theme of prevention of collisions of vessels and it is shown that at the locally-independent management for this purpose offered method of forming of flexible strategies of divergence, methods of nonlinear integral invariance and theories of optimum discrete processes. Formalization is also produced cooperation of vessels in the situations of dangerous rapprochement and choice of strategy of divergence. Strategy of urgent divergence is considered at rapprochement of vessels on small distances and description of process of divergence of vessels and choice of maneuver of divergence is produced in terms of differential antagonistic game. It is shown that for the decision the problem set in the article it is necessary to choose the course of deviation the nearest to programmatic, and which does not belong to none of three impermissible great number of courses of deviation: with the first target, with the second target and with a navigation danger. The condition of the safe passing by the ship of point danger, which foresees superiority of value of distance of the shortest rapprochement with her above the value of maximum possible distance of rapprochement, is resulted, and his analytical expression which allows to expect scope safe courses of deviation of ship. For estimation of danger of rapprochement of ship with a target equalizations of scopes of region of impermissible parameters of motion of ship are resulted. The situation of rapprochement of ship is considered with two targets and the regions of impermissible values of parameters of motion of ship in relation to each of targets are formed. The condition of dangerous rapprochement is formulated. The example of situation of rapprochement of ship with two targets is represented, when there is a point navigation danger in the district of sailing. The graphic reflection of regions of impermissible values of parameters of motion of ship in relation to each of targets is shown, on which the maximum courses of deviation on navigation limitations are also inflicted by the dotted Судноводіння | Shipping & Navigation ISSN 2306-5761 | 2618-0073 30-2020 100 Національний університет «Одеська морська академія» lines. ABSTRACTIn the scheme for geological disposal of high level radioactive nuclear wastes, the burial pit is to be isolated from the sphere of human life by a multiple-barrier system, which consists of an artificial barrier, composed of a canister, an overpack and a bentonite cushioning layer, and a natural barrier, which is essentially the bedrock. As the greatest as well as essentially the sole detriment to its integrity would be corrosion by groundwater. The groundwater comes to it seeping through the bentonite zone, thereby attaining conceivably the pH of transition from general corrosion to passivity, pHd, the behaviors of mild steel in such a groundwater environment have been examined. It has been shown that the pHd is lowered (enlargement of the passivity domain) with rising temperature and carbonate-bicarbonate concentration, while it is raised (enlargement of the general corrosion region) with increasing concentrations of chloride and sulfate ions.

The article addresses the issues of ensuring safe operation of mining equipment in surface mining. It describes the main factors and situations that pose a high risk to human life and health. The most dangerous incidents are shown to be related to limited visibility and blind spots for operators of mining equipment, which can result in collisions and personnel run over. The main technologies and specific solutions used to design collision avoidance systems are described and their general comparison is provided. A particular focus is placed on monitoring the health of employees at their workplace by means of portable personal devices that promptly inform the dispatcher of emergency situations. General technical requirements are formulated for designing of the system to prevent equipment collisions and personnel run over in surface mining operations. The paper emphasizes the importance of introducing a multifunctional safety system in surface mines in order to minimise the possibility of incidents and accidents throughout the entire production cycle.

Lung cancer is the leading cause of mortality from malignant tumors all over the world. Since most patients at the time of diagnosis already have stage III-IV of the disease, the search for new effective treatment strategies for advanced NSCLC is the most important problem of modern oncology. The results of the study of the anti-PD1 monoclonal antibody pembrolizumab were a real breakthrough in the treatment of NSCLC. In the KEYN0TE-001 study, the expression of PD-L1 on tumor cells was validated as a predictive biomarker of the drug's efficiency. Pembrolizumab demonstrated the possibility of achieving long-term objective responses, and a 4-year 0S with all histological types in the subgroup of pre-treated patients with PD-L1 expression> 50% was 24.8% and 15.6% in the PD-L1> 1% group. In a phase 2/3 randomized study KEYN0TE-10 in the 2nd line treatment of NSCLC with PD-L1 expression > 1% pembrolizumab significantly increased life expectancy compared to docetaxel and confirmed the possibility of longterm duration of objective responses, even after cessation of treatment. Then the focus of research shifted to the 1st line of treatment. About 30% of patients with NSCLC have a high level of PD-L1 expression on tumor cells and demonstrate the most impressive response to pembrolizumab therapy. A randomized phase 3 study KEYN0TE-024 compared the effectiveness of pembrolizumab monotherapy with a standard platinum combination in patients with advanced NSCLC with a high level of PD-L1 expression without EGFR mutations or ALK translocation. Compared with the platinum doublet the administration of pembrolizumab significantly increased all estimated parameters, including the median of progression-free survival (mPFS was 10.3 months versus 6 months; HR = 0.50; 95% CI 0.37-0.68, p < 0.001), the objective response rate (ORR 44.8% versus 27.8%), duration of response (in the pembrolizumab arm the median was not reached, in the chemotherapy (CT) group - 6.3 months). Despite the approved crossover, the use of pembrolizumab in the 1st line of treatment more than doubled the life expectancy of NSCLC patients with high PD-L1 expression as compared to CT: the median overall survival (OS) was 30.0 months versus 14.2 months (HR = 0.63, p = 0.002), 1-year OS 70.3% versus 54.8%; 2-year OS - 51.5% versus 34.5%. The remaining population to study were untreated patients with any level of PD-L1 expression. A randomized phase 3 study KEYNOTE-189 evaluated the effectiveness of adding pembrolizumab to the platinum combination in the 1st line treatment of non-squamous NSCLC without EGFR and ALK mutations with any PD-L1 expression. The addition of pembrolizumab to the standard 1st line CT significantly increased all estimated efficacy indicators including OS, PFS and ORR. After a median follow-up of 10.5 months the median OS in the pembrolizumab combination group was not reached and in CT group was 11.3 months. The estimated 12-months survival was 69.2% and 49.4% respectively (HR = 0.49; 95% CI 0.38-0,64; p <0.001). The median PFS was 8.8 months versus 4.9 months, alive 1 year without progression 34.1% and 17.3% of patients respectively (HR = 0.52; p <0.001). The ORR in the group with pembrolizumab reached 47.6% versus 18.9% in CT group, moreover the tumor regressions were much longer. Finally a randomized 3-phase study KEYN0TE-407 evaluated the effectiveness of adding pembrolizumab to 1st-line CT of NSCLC with squamous histology with any PD-L1 expression. As the first analysis showed, the addition of permboli-zumab significantly increased OS of patients with squamous NSCLC, median OS 15.9 months versus 11.3 months in the groups of pembrolizumab + CT and placebo + CT respectively (HR = 0.64; 95% CI 0,49-0.95; p = 0.0006), median PFS 6.4 months and 4.8 months respectively (HR = 0.56; 95% CI 0.450.70; p <0, 0001) and OrR 57.9% versus 38.4%, the median response duration 7.7 months versus 4.8 months. Thus, the convincing advantages of using pembrolizumab in 1st line therapy were demonstrated in 3 randomized phase 3 studies: in monotherapy of NSCLC of any histological subtype with high PD-L1 expression, and in combination with CT in squamous and non-squamous hystologies regardless of the level of PD-L1 expression.

UNSTRUCTURED Over half of Americans report looking up health-related questions on the internet, including questions regarding their own ailments. The internet, in its vastness of information, provides a platform for patients to understand how to seek help and understand their condition. In most cases, this search for knowledge serves as a starting point to gather evidence that leads to a doctor’s appointment. However, in some cases, the person looking for information ends up tangled in an information web that perpetuates anxiety and further searches, without leading to a doctor’s appointment. The Internet can provide helpful and useful information; however, it can also be a tool for self-misdiagnosis. Said person craves the instant gratification the Internet provides when ‘googling’ – something one does not receive when having to wait for a doctor’s appointment or test results. Nevertheless, the Internet gives that instant response we demand in those moments of desperation. Cyberchondria, a term that has entered the medical lexicon in the 21st century after the advent of the internet, refers to the unfounded escalation of people’s concerns about their symptomatology based on search results and literature online. ‘Cyberchondriacs’ experience mistrust of medical experts, compulsion, reassurance seeking, and excessiveness. Their excessive online research about health can also be associated with unnecessary medical expenses, which primarily arise from anxiety, increased psychological distress, and worry. This vicious cycle of searching information and trying to explain current ailments derives into a quest for associating symptoms to diseases and further experiencing the other symptoms of said disease. This psychiatric disorder, known as somatization, was first introduced to the DSM-III in the 1980s. Somatization is a psycho-biological disorder where physical symptoms occur without any palpable organic cause. It is a disorder that has been renamed, discounted, and misdiagnosed from the beginning of the DSMs. Somatization triggers span many mental, emotional, and cultural aspects of human life. Our environment and social experiences can lay the blueprint for disorders to develop over time; an idea that is widely accepted for underlying psychiatric disorders such as depression and anxiety. The research is going in the right direction by exploring brain regions but needs to be expanded on from a sociocultural perspective. In this work, we explore the relationship between somatization disorder and the condition known as cyberchondria. First, we provide a background on each of the disorders, including their history and psychological perspective. Second, we proceed to explain the relationship between the two disorders, followed by a discussion on how this relationship has been studied in the scientific literature. Thirdly, we explain the problem that the relationship between these two disorders creates in society. Lastly, we propose a set of intervention aids and helpful resource prototypes that aim at resolving the problem. The proposed solutions ranged from a site-specific clinic teaching about cyberchondria to a digital design-coded chrome extension available to the public.

OBJECTIVE: the present work proposed to extract Locust Bean Gum (LBG) from Algerian carob fruits, evaluate physicochemical and rheological properties (solubility). It aimed also to develop different formulations of strawberry jams with a mixture of LBG and pectin in order to obtain a product with a high sensory acceptance. METHODS: the physicochemical characteristics of LBG were assessed. The impact of temperature on solubility was also studied. The physical and the sensory profile and acceptance of five Jams were evaluated. RESULTS: composition results revealed that LBG presented a high level of carbohydrate but low concentrations of fat and ash. The LBG was partially cold-water-soluble (∼62% at 25°C) and needed heating to reach a higher solubility value (∼89% at 80 °C). Overall, the sensorial acceptances decreased in jams J3 which was formulated with 100% pectin and commercial one (J5). The external preference map explained that most consumers were located to the right side of the map providing evidence that most samples appreciated were J4 and J2 (rate of 80–100%). CONCLUSION: In this investigation, the LBG was used successfully in the strawberry jam’s formulation.

The COVID-19 pandemic has altered healthcare delivery platforms from traditional face-to-face formats to online care through digital tools. The healthcare industry saw a rapid adoption of digital collaborative tools to provide care to patients, regardless of where patients or clinicians were located, while mitigating the risk of exposure to the coronavirus. Information technologies now allow healthcare providers to continue a high level of care for their patients through virtual visits, and to collaborate with other providers in the networks. Population health can be improved by social determinants of health and precision medicine working together. However, these two health-enhancing constructs work independently, resulting in suboptimal health results. This paper argues that artificial intelligence can provide clinical–community linkage that enhances overall population health. An exploratory roadmap is proposed.
