Background: The predictive value of serum soluble ST2 (sST2) biomarker for diagnostics of cardiovascular pathologies is still poorly understood as well as the role of psychological stress on the risk of heart disease. Aim: This study aimed at determining the diagnostic significance of the sST2 level in athletes involved in speed-strength sports. In addition, stress as a risk factor for the development of cardiovascular pathology was assessed and analysed as well. Methods: A prospective study on Greco-Roman wrestlers was carried out at the Centre for Sports Medicine and Rehabilitation (Almaty, Republic of Kazakhstan). All participants (n = 30) were males aged 20 to 34 years. The control group consisted of volunteers (VO) (n = 30). Anthropometric and hemodynamic parameters of athletes were studied along with electrocardiography (ECG) and ECG tests. The sST2 level was determined before (BT) and immediately after (AT) training. The stress level was determined using The Perceived Stress Scale-10 (PSS-10). Results: The average age of the athletes was 26.57 ± 3.6 years. The total training experience was 14.57 ± 4.02 years. According to the ECG data, minor deviations from the norm (13.3%) and an abnormal ECG (33.3%) were identified. Echo-CG data showed "moderate" and "pronounced changes" in 23.3% and 53.3% of cases, respectively. The sST2 level of VO (337.1 ± 61.8 pg / mL) was lower than that of BT (548.1 ± 32.6 pg / mL) (p ≤ 0.001). The sST2 level of AT, it was significantly higher (830.01 ± 71.6 pg / mL) than BT (p ≤ 0.001). The average and high level of stress among athletes was in 43.3% and 56.7% of cases, respectively. Stress increased the likelihood of developing distinctly abnormal ECG (OR = 1.06, 95% CI 1.01–1.08; p = 0.02). The stress level showed a positive correlation with the sST2 level (r = 0.752, p = 0.01). The sST2 concentration and categorical echocardiography data demonstrated a dependent positive correlation (r = 0.6, p = 0.01). Conclusions: Athletes' sST2 levels exceeded thresholds both before and after training. Moreover, the relationship between an increase in sST2 levels and abnormal ECG abnormalities and a high level of stress in athletes was determined. sST2 concentration was associated with cardio-pulmonary stress triggered by the cumulative exercise dose as well as with lifelong psychological stress. Our findings indicate that the elevated sST2 concentrations in athletes could be used as the predictive value. However, clinical relevance and results validity require further intensive studies.  Resumen. Antecedentes: El valor predictivo del biomarcador ST2 soluble en suero (sST2) en la enfermedad cardiovascular aún no se conoce bien, así como el papel del estrés psicológico en el riesgo de enfermedad cardiovascular. Objetivos: Este estudio tuvo como objetivo determinar la importancia diagnóstica del nivel de sST2 en atletas involucrados en deportes de velocidad-fuerza. Además, también se evaluó y analizó el estrés como factor de riesgo para el desarrollo de patología cardiovascular. Métodos: Se llevó a cabo un estudio prospectivo sobre luchadores grecorromanos en el Centro de Medicina y Rehabilitación del Deporte (Almaty, República de Kazajstán). Todos los participantes (n = 30) eran hombres de entre 20 y 34 años. El grupo de control estaba formado por voluntarios (VO) (n = 30). Se estudiaron los parámetros antropométricos y hemodinámicos de los atletas junto con las pruebas de electrocardiografía (ECG). El nivel de sST2 se determinó antes (BT) e inmediatamente después (AT) del entrenamiento. El nivel de estrés se determinó utilizando la Escala de Estrés Percibido-10 (PSS-10). Resultados: La edad promedio de los deportistas fue de 26,57 ± 3,6 años. La experiencia de formación total fue de 14,57 ± 4,02 años. Según los datos del ECG, se identificaron desviaciones menores de la norma (13,3%) y un ECG anormal (33,3%). Los datos de Echo-CG mostraron cambios "moderados" y "pronunciados" en el 23,3% y el 53,3% de los casos, respectivamente. El nivel de sST2 del grupo VO (337,1 ± 61,8 pg / mL) fue menor que el de BT (548,1 ± 32,6 pg / mL) (p ≤ 0,001),). El nivel de sST2 en AT fue significativamente mayor (830.01 ± 71.6 pg / mL) que en BT (p ≤ 0.001). El nivel medio y alto de estrés entre los deportistas fue del 43,3% y el 56,7% de los casos, respectivamente. El estrés aumentó la probabilidad de desarrollar un ECG claramente anormal (OR = 1,06; IC del 95%: 1,01-1,08; p = 0,02). El nivel de estrés mostró una correlación positiva con el nivel de sST2 (r = 0,752, p = 0,01). La concentración de sST2 y los datos de la ecocardiografía categórica demostraron una correlación positiva dependiente (r = 0,6, p = 0,01). Conclusión: Los niveles de sST2 de los atletas excedieron los umbrales tanto antes como después del entrenamiento. Además, se determinó la relación entre un aumento en los niveles de sST2 y anomalías anormales del ECG y un alto nivel de estrés en los atletas. La concentración de sST2 se asoció con el estrés cardiopulmonar desencadenado por la dosis acumulativa de ejercicio, así como con el estrés psicológico de por vida. Nuestros hallazgos indican que las concentraciones elevadas de sST2 en los atletas pueden usarse como valor predictivo. Sin embargo, se requieren más estudios.

Background: An anteroposterior (AP) radiograph of the elbow in 45° of flexion has been suggested to increase the diagnostic accuracy of capitellum osteochondritis dissecans (OCD). Purpose: To assess the diagnostic performance, inter- and intraobserver reliability, and confidence level for identifying capitellum OCD using plain radiographs (AP, lateral, and 45° flexion AP). Study Design: Cohort study (Diagnosis); Level of evidence, 3. Methods: This was a retrospective study of pediatric and adolescent patients with capitellum OCD and a control group. Six independent clinicians who were blinded to the official radiologists’ reports reviewed images on 2 separate occasions, 1 week apart. A 5-point Likert scale was used to assess the clinicians’ level of confidence. Sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV), and accuracy were calculated for individual and combinations of radiographic views. Inter- and intraobserver reliability was determined using Cohen kappa (κ) coefficients. Results: A total of 28 elbows (mean age, 12.5 ± 2 years) were included. There were no differences in age ( P = .18), sex ( P = .62), or laterality ( P > .999) between groups. There were marked variations in the diagnostic accuracy between views: sensitivity (AP, 85.1; lateral, 73.2; 45° flexion AP, 91.7), specificity (AP, 89.3; lateral, 91.7; 45° flexion AP, 91.1), PPV (AP, 88.8; lateral, 89.8; 45° flexion AP, 91.1), NPV (AP, 85.7; lateral, 77.4; 45° flexion AP, 91.6), and accuracy (AP, 87.2; lateral, 82.4; 45° flexion AP, 91.4). Standard radiographs (AP and lateral views) failed to diagnose capitellum OCD in 4.8% of cases. The sensitivity of the 3 combined views was 100%. Confidence intervals in the clinicians’ diagnostic assessments were similar for each view (AP, 4.0; lateral, 4.0; and 45° flexion AP, 4.1). Interobserver reliability was substantial for AP and lateral views (κ = 0.65 and κ = 0.60, respectively) but highest for the 45° flexion AP radiographs (κ = 0.72). Intraobserver reliability for the 45° flexion AP view was moderate to almost perfect (κ = 0.45 to 0.93). Conclusion: The 45° flexion AP view can detect capitellum OCD with excellent accuracy, a high level of confidence, and substantial interobserver agreement. When added to standard AP and lateral radiographs, the 45° flexion AP view aids in the identification of capitellum OCD.

The study aimed to identify the psychological stress level among orphans in Aqaba governorate and its relationship with both psychological hardness and life quality. The sample of the study was consisted of ( 102 ) male and female Orphans students whom were selected using the simple random method of sample selecting. Three scales were developed: The Psychological Stress Scale, The Psychological Hardiness Scale and The Life Quality Scale, these scales were checked for reliability and consistency, results indicated that the psychological stress level among orphan students in Aqaba governorate was medium as the emotional dimension came first with a high level while the physiological dimension came second with a medium level. Results also indicated a significant statistical negative relationship at α≥ 0.05 between psychological stress and its dimensions from the one hand, and each of psychological hardiness with its dimensions and life quality with its dimensions in the other. In addition, results indicated no differences between the two genders in the psychological stress level as whole and it was cognitive and emotional dimensions, but indicated differences between male and female participants in the physiological dimension for psychological stress.  Finally, the study included a number of recommendations including: further studies dealing with the relationship of psychological stress and other variables, the orphans of self-esteem and psychological adjustment.

CLINICAL PHENOMENOLOGY OF ADAPTATION DISORDERS IN PEOPLE WITH COMPUTER DEPENDENCYStarodubtseva Yu.The purpose of the study was comprehensive study of clinical-psychopathological and pathopsychological peculiarities of adaptation disorders in persons with computer dependence. 147 patients with adaptive disorders (F43.21, F43.22). The main group consisted of 85 patients with signs of computer dependence according to the results of AUDIT-like tests, the control group included 62 patients without signs of addictive behavior. The average age of the examined people was 27,0 ± 3,0 years. Clinical-psychopathological, clinical-anamnestic pathopsychological, psychodiagnostic and statistical methods were used to attain the aim.The structure of computer dependence in the surveyed people of the main group consisted of obsessive surfing (46.2%); computer games (22.3%); virtual dating (6.4%); passion for online gambling (13.7%); cybersex (1.4%). In clinical adaptation disorders, people with computer dependence are dominated by reduced mood; internal stress with inability to relax; increased susceptibility to previously neutral stimuli; irritability; asthenic symptoms; loss of interest in work or study, family and friends; violation of the sleep-wake cycle. According to the psychodiagnostic survey data, the examined people of the main group are characterized by anxiety and depression manifestations by the Hospital scale; severe depressive and anxiety episodes by the Hamilton scale; high levels of situational and personal anxiety by the method of Ch. D. Spielberger, high level of expressiveness of the psychological stress by the scale of T. A. Nemchin. Д. Спілбергера, високій рівень виразності нервово-психічної напруги по шкалі Т. А. Д. Спилбергера, высокий уровень выраженности нервно-психического напряжения по шкале Т. А. SummaryAim: In children, screening for haemorrhagic disorders is further complicated by the fact that infants and young children with mild disease in many cases most likely will not have a significant history of easy bruising or bleeding making the efficacy of a questionnaire even more questionable. Patients, methods: We compared the questionnaires of a group of 88 children in whom a haemorrhagic disorder was ruled out by rigorous laboratory investigation to a group of 38 children with mild von Willebrand disease (VWD). Questionnaires about child, mother and father were obtained prior to the laboratory diagnosis on the occasion of routine preoperative screening. Results: 23/38 children with mild VWD showed at least one positive question in the questionnaire, while 21/88 without laboratory signs showed at least one positive question. There was a trend to more specific symptoms in older children. Three or more positive questions were found only in VWD patients, but only in a few of the control group. The question about menstrual bleeding in mothers did not differ significantly. Sensitivity of the questionnaire for a hemostatic disorder was 0.60, while specifity was 0.76. The negative predictive value was 0.82, but the positive predictive value was only 0.52. Conclusions: Our small study shows, that a questionnaire yields good results to exclude a haemostatic disorder, but is not a sensitive tool to identify such a disorder.

Background: Serum ferritin is known as an index for body iron stores also as an inflammatory marker and it is influenced by several disease. We were looking for a correlation between HbA1c and S. Ferritin in type 2 DM. Methodology: The present study a total of 150 participants were enrolled of which 100 were confirmed cases of Type 2 Diabetes Mellitus and rest 50 age and sex matched healthy subjects constituted the control group. All were screened for HbA1c, Fasting blood sugar, Post prandial blood sugar and S.Ferritin. Results: A highly significant variation and positive correlation was observed with respect to S.Ferritin and HbA1c levels. Mean S.Ferritin was high in the subgroup with poor glycemic control. Conclusion: The fasting, post prandial sugar levels, HbA1c and S.Ferritin were significantly higher in the diabetic subjects. This study shows a positive correlation between HbA1c and S. Ferritin levels. So we can conclude that in diabetic patients S. Ferritin may serve as an independent marker of poor glycemic and metabolic control. In addition to the classical pathogens, which are well understood and well identified, new pathogens with the potential to spread epidemiologically are being identified. Some of these little-known organisms are the simplest Blastocystis spp. blastocystostosis. The clinical significance of Blastocystis spp. and its pathogenicity are still under discussion. This parasite belongs to a group of single-celled eukaryotic organisms living in the colon of the human intestine. Blastocystis spp. is known to be found both in people with reduced immune status and in individuals without any clinical manifestation. It has been established that a sufficiently high degree of invasiveness is observed in persons with gastrointestinal tract diseases, dermatosis, allergic reactions, in patients with carriers of the human immunodeficiency virus, etc. Possessing persistence factors, protozoa blastocysts contribute to the inactivation of host defensive mechanisms, providing a stable anthogonistic effect. In recent years, many works have been devoted to the characteristics of the persistent properties of Blastocystis spr., however, individual properties of blastocysts, in particular, anticytokine activity (ACA), have not yet been studied. In this regard, the work studied the anticytokine activity of microorganisms isolated from healthy subjects and patients with gastrointestinal tract diseases. A high prevalence of the studied characteristic in the subjects was shown. The expression of anticytokine activity in the obtained isolates of blastocysts was the highest in the group of persons with gastric ulcer disease, which decreased in the order of duodenal ulcer, chronic cholecystitis, chronic gastritis, etc. The data obtained in this work on the high level of ACA expression in blastocyst isolates obtained from individuals with gastrointestinal diseases as compared with the control group enables to conclude that their exometabolites may influence the local cytokine balance [1], which supports the inflammatory process.

Background : Sleep Apnea Syndrome (SAS) is a cause of hypertension, increasingcardiovascular risk and cardiovascular disease such as stroke, coronary artery disease, myocardial infarction, arrhythmias, heart failure, increasing the risk of death in patients with heart disease, independent of other causative factors. So far, in Vietnam there are very few studies on Obstructive Sleep Apnea Syndrome (OSAS) and cardiovascular risk factors . Self-making SASD07 is trustly for detecting OSAS with statistical significiant in comparision with StarDustII (gold criteria). Subjects and Methods: Cross sectional study, comparision with control group: 136 peoples (68 in disease group and 68 in control group). Patients were parallelly measured with StarDustII and SASD07 to detect OSAS and find the corellation with cardiovascular risk factors. Results: There is a positive correlation between SBP with the severity of OSAS (r = 0.459, p < 0.001), positive correlation between DBP with the severity of OSAS (r = 0.352, p < 0.003). No statistically significant differences between severe OSAS and fasting blood glucose, cholesterol, HDL Cholesterol, Non - HDL Cholesterol, LDL Cholesterol and TG median (p > 0.05). There is a positive correlation between AHI and neck circumference (r = 0.511, p < 0.001), waist circumference (r = 0.585, p < 0.001), BMI (r = 0.380, p < 0.01). SASD07 diagnostic value of detecting OSAS compared with StarDustII have Kappa = 0.72, (standard error 0.06, p <0.001). Conclusion: The risk factors related to OSAS in our study is neck circumference, waist circumference, systolic blood pressure, diastolic blood pressure. SASD07 have a good value in diagnosing of OSAS compared with polysomnography StarDustII. Key words: Sleep Apnea Syndrome, cardiovascular risk factor, SASD07.

Introduction:The high level of morbidity and mortality from cardiovascular disease is largely due toinsufficient influence on the main risk factors that contribute to the development of myocardial infarction.Therefore, a detailed study and assessment of risk factors is among the most important problems of medical and social importance. The aim: To study and evaluate the impact of biological, social and hygienic, social and economic, psychological, natural and climatic risk factors on the development of myocardial infarction. Materials and methods: A sociological survey was conducted in 500 people aged 34 to 85. They were divided into two groups. The main group consisted of 310 patients with myocardial infarction. The control group consisted of 190 practically healthy people, identical by age, gender and other parameters, without diseases of the cardiovascular system. Results: It was defined that 30 factors have a significant impact on the development of myocardial infarction.Data analysis revealed that the leading risk factors for myocardial infarction were biological and socio-hygienic. The main biological factors were: hypertension and hypercholesterolemia. The man socio-hygienic factor was smoking. Conclusions: Identification of risk factors provides new opportunities for the development of more effective approaches for the prevention and treatment of myocardial infarction.

Aims: The etiopathogenesis of Rheumatoid Arthritis (RA) is not clearly understood. However, the role of the cytokines takes an important part in this mechanism. We aimed to bring a new approach to the concept of 'remission' in patients with RA. Background: RA is a chronic, autoimmune, inflammatory disease that involves small joints in the form of symmetrical polyarthritis and progresses with exacerbations and remissions. Pain, swelling, tenderness and morning stiffness are typical of the joints involved. Although it is approached as a primary joint disease, a wide variety of extra-articular involvements may also occur. It is an interesting pathophysiological process, the exact cause of which is still unknown, with many environmental, genetic and potentially undiscovered possible factors in a chaotic manner. Objective: In this cross-sectional study, sedimentation rate (ESR), C- Reactive protein (CRP), Tumor necrosis factor (TNF)-α, soluble-TNF-α receptor (TNF-R), Interleukin (IL)-1B and IL-10 were measured in three groups which were healthy volunteers, patients with RA in the active period, and patients with RA in remission. Disease activity score-28 (DAS-28) was calculated in active RA and RA in remission. Methods: This study included 20 healthy volunteers, 20 remission patients with RA and 20 active RA patients. Venous blood samples were collected from patients in both healthy and RA groups. Results: RA group consisted 43 (71.6%) female and 17 (28.4%) male. Control group consisted 11 (55%) female and 9 (45%) male. TNF-R was significantly high only in the active group according to the healthy group (p=0.002). IL-10 was significantly high in active RA according to RA in remission (p=0.03). DAS-28 was significantly high in active RA according to RA in remission (p=0.001). In the active RA group, ESR and TNF-R had a positive correlation (r:0.442; p=0.048). In the active RA group, there was also a positive correlation between TNF-R and CRP (r:0.621; p=0,003). Both healthy and active RA group had significant positive correlation between ESR and CRP (r: 0.481; p=0.032 and r: 0,697; p=0,001 respectively). Conclusion: TNF-R can be the main pathophysiological factor and a marker showing activation. TNF-R can be very important in revealing the effect of TNF on the disease and the value of this effect in the treatment and ensuring the follow-up of the disease with CRP instead of ESR in activation.

A study in human Onchocerciasis was undertaken in four endemic communities in Ondo State, Nigeria. In-depth interviews were conducted on peoples' knowledge, attitudes, and practices regarding Onchocerciasis aetiology, treatment, prevention, and symptoms. These were complemented by key informant interviews and focus group discussions. Based on this information, an educational program was set up which included the training of selected villagers (motivators) and community intervention organized by these motivators. Evaluation used a control group where intervention was focused on other health problems in the area. Onchocerciasis education took place only with the intervention group. At the start of the project, peoples' knowledge about Onchocerciasis, its cause, treatment, prevention, and symptoms were varied and only a small proportion could link the bite of the blackfly to Onchocerciasis. The educational intervention achieved a high level of participant satisfaction which was expressed in continuous attendance at workshops and keeping appointments with motivators. The intervention helped to bring a significant improvement in the knowledge, attitudes, and practices (KAP) of the respondents. The knowledge of Onchocerciasis aetiology increased to 79.8 percent, 71.5 percent, and 74 percent from 48.5 percent, 48.7 percent, 34 percent, and 45 percent pre-intervention in the four study areas used. The project demonstrated that a community-based health education can be effective in Onchocerciasis control.
