Introduction: In the literature ovarian tumors are knowed to be one of the most deadliest gynecological malignancies [1-4]. In US this type of malignancy represents 2.3 % of all cancer-related death and about 4 % of all new cancer cases among women. There are several studies that have reported the role of lipid profiles and it’s role in ovarian tumorigenesis. Fatty acids are essential for cancer cells progression[4-8]. In our study we investigated the true differance in circulating lipid profiles (total cholesterol TC, triglyceride TG, high-density lipoprotein cholesterol HDL, low density lipoprotein cholesterol LDL) among patients with and without ovarian tumors (OT) using a meta-analytical approach. Mehods: The meta-analysis was conducted using the MOOSE guidelines. PubMed, EMBASE and Cochrane Library were extensively searched ( with a period of publication restriction between 2007 and 2019) to indetify published studies using the following keywords: “ total cholesterol ”, “high-density lipoprotein”, “ triglycerides ”, “ low-density lipoprotein ”, “ ovarian cancer”, “ ovarian tumor ”, “ lipid profile ”. The search methodology is shown in Fig. 1 and all references of retrieved articles were searched manually. Results: Seven studies, involving 1542 OT cases and 2195 non-cases of OT were included in this meta-analysis and I² statistics ranged between 97 and 99%. Mean circulating TC and HDL were significantly lower among OT cases compared to non-OT cases (P<0.04 and P<0.005). Conclusion: There is a modest significant association between circulating HDL and risk of ovarian tumor but it is crucial to elucidate the implications of HDL in tumor manifestations and growth.

Demons syndrome is defined by hydrothorax and ascites associated with a benign genital tumor that resolves after resection of the tumor. However, Demons syndrome with pericardial effusion has never been reported. Intensive care unit–acquired weakness is a neurological sequela to sepsis/systemic inflammatory response syndrome, or multi-organ failure. A 47-year-old, nulligravid, Japanese woman, was transferred to our hospital for refractory heart failure and a ruptured ovarian tumor. She had an 11-cm left ovarian tumor with ascites, hydrothorax, and pericardial effusion; she was intubated for pulmonary hypertension and admitted to the intensive care unit for septic shock. Four days later, a left salpingo-oophorectomy was performed for Demons syndrome with pericardial effusion. The histological diagnosis indicated a serous cystadenoma with fibrotic changes. Following surgery, ventilator weaning was delayed due to intensive care unit–acquired weakness. The association between Demons syndrome and pericardial effusion should be recognized to ensure early treatment and for preventing sequalae from the disease.

The aim of the investigation was to study the role of diagnostic laparoscopy in treatment of patients with ovarian cancer. The analysis of 480 laparoscopic operations, which was carried out on 42 patients with benign tumors, 29 cases of suspected tumor malignization, 67 patients with non gynecological pathology with suspected ovarian neoplasia and 174 women with established ovarian cancer, made it possible to define indications to laparoscopic operations. Using this method the authors managed to solve differential-diagnostic difficulties in 81,7% cases. The results of the investigation revealed the advantages of the method as to patients with benign tumors. This method is recommended for ovarian tumor and early recurrence diagnostics and also for morphological verification of tumor development. But the role of laparoscopy in treatment of malignant ovarian cancer, especially its early forms, still has to be further investigated.

Adipose-derived stem cells (ADSC) are multipotent mesenchymal stem cells derived from adipose tissues and are capable of differentiating into multiple cell types in the tumor microenvironment (TME). The roles of ADSC in ovarian cancer (OC) metastasis are still not well defined. To understand whether ADSC contributes to ovarian tumor metastasis, we examined epithelial to mesenchymal transition (EMT) markers in OC cells following the treatment of the ADSC-conditioned medium (ADSC-CM). ADSC-CM promotes EMT in OC cells. Functionally, ADSC-CM promotes OC cell proliferation, survival, migration, and invasion. We further demonstrated that ADSC-CM induced EMT via TGF-β growth factor secretion from ADSC and the ensuing activation of the TGF-β pathway. ADSC-CM-induced EMT in OC cells was reversible by the TGF-β inhibitor SB431542 treatment. Using an orthotopic OC mouse model, we also provide the experimental evidence that ADSC contributes to ovarian tumor growth and metastasis by promoting EMT through activating the TGF-β pathway. Taken together, our data indicate that targeting ADSC using the TGF-β inhibitor has the therapeutic potential in blocking the EMT and OC metastasis.

A holistic and multidisciplinary approach is required for ovarian cancer diagnosis and management. Ovarian tumor diagnosis is based on clinical symptomatology followed by radiological confirmation. Tumor markers are considered just adjunct. Management of ovarian mass is surgical and intra -op frozen section aids in understanding the nature of the mass but histopathology is definitive. Ovarian torsion affects 2.5 to 7.4% of all ovarian tumors. The present case will highlight the destructive changes brought about by torsion which could ultimately result in missed tissue diagnosis.
