Abstract BackgroundAcute severe organophosphorus pesticide poisoning (ASOPP) is a common severe emergency in developing countries. At present, the incidence of complications and mortality are still very high, and there are some difficulties in treatment. More and more studies have found that hemoperfusion combined with hemodialysis can improve the prognosis of patients based on conventional emergency treatment, but there is a lack of evidence-based medical evidence. Our meta-analysis is mainly to clarify the efficacy of hemoperfusion combined with hemodialysis on ASOPP.MethodsWe searched several databases. After formulating relevant inclusion and exclusion criteria, qualified studies were included and data were extracted. The outcome indicators were cure rate, efficacy, incidence of complications, time for cholinesterase level to return to normal, coma time, time of hospitalization and atropine dosage. Results were analyzed using risk ratios (RR), odds ratios (OR), weighted mean difference (WMD), standard mean difference (SMD), and 95% confidential interval (95% CI). The cochrane collaboration tool was used for the assessment of the risk of bias of every included studies. In terms of statistical methods, we used RevMan software (version 5.3) and Stata (version 14 and 16) for data analysis.ResultsWe included 35 RCTs, including 2650 participants. Compared with the emergency conventional treatment group, the use of hemoperfusion combined with hemodialysis to treat ASOPP patients based on conventional emergency treatment significantly increased the cure rate [RR=1.20, 95% CI (1.16, 1.25), p<0.00001], improved the efficacy [OR=2.76, 95% CI (2.07, 3.67), p=0.000], and reduced the incidence of complications [RR=0.26, 95% CI (0.20, 0.34), p<0.00001] and also reduced the patient's time for cholinesterase level to return to normal [WMD=-4.05, 95% CI (-4.40, -3.70), p<0.00001], coma time [SMD=-2.62, 95% CI (-3.08, -2.15), p<0.00001], time of hospitalization [WMD=-5.80, 95% CI (-6.57, -5.02), p<0.00001], and atropine dosage [WMD=-194.81, 95% CI (-223.55, -166.08), p<0.00001].ConclusionHemoperfusion combined with hemodialysis can apparently improve the prognosis of patients and has a good effect in the treatment of acute severe organophosphorus pesticide poisoning. In the future, more scientifically designed large-scale, high-quality, multi-center randomized controlled trials are needed to further illustrate the significance of this study.

It was a retrospective analysis of the doctor practices of author on random patients for emergency treatment who needed standard electrocardiogram examinations.

Abstract Background Quality of care for Chinese patients with heart failure was substandard. It is of utmost value to ascertain the characteristics related to quality of care to narrow the gap. Methods Data from 2,064 heart failure patients between 1 January 2012 and 31 December 2015 at a hospital in Fujian Province were analyzed. Bayesian Network was used to assess the regulatory relationships between demographic, clinical characteristics and compliance with quality indicators. Results The compliance with quality indicators ranged from 42.5% to 90.2%. The compliance with recommended doses for medications all reached or was close to 100% except indapamide. In Bayesian network, residence place, hypertension, troponin, B-type natriuretic peptide, heart rate, lung disease, number of emergency treatment, ejection fraction directly regulated the compliance and gender, age, medical payment method, myocardiopathy, coronary heart disease, arrhythmia had indirectly effect. The lower compliance was found in patients under emergency treatment, patients with abnormal testing indicators, patients without specific comorbidities and patients with NRCMS or self-paying. Patients with lung disease and those who lived in urban area had longer length of stay. Conclusions The compliance with medication indicators for heart failure were suboptimal, but recommended doses were prescribed in patients who received medications. A series of strategies should be developed to improve the quality of care, such as expanding the scope and depth of knowledge of guidelines and clinical pathway, integrating the reminder and quality assessment model into hospital medical record information system, paying more attention to vulnerable population and improving the medical security system.

The rapid outbreak of coronavirus syndrome 2 (SARS-CoV-2) has engulfed the entire international community and triggered serious public health issues. Orthodontists may encounter patients with suspected or confirmed SARS-CoV-2 infection and may need to work vigilantly to avoid the spread of infection, consecutively provide care and emergency treatment. The objective of this review is to provide a brief overview of the effects of SARS-CoV-2 and COVID-19 on orthodontic treatment, and to address risk management and the facilitation of orthodontic emergency care and post-pandemic orthodontic practice, using data and literature currently available.

In order to improve the comprehensive nursing effect of the hospital emergency treatment, this paper analyzes the process of the hospital emergency treatment. In addition, this paper combines the possible risks to analyze the risk factors of the comprehensive nursing in the hospital emergency treatment and builds an intelligent analysis model based on the actual situation of the hospital emergency treatment. At the same time, this paper conducts a systematic survey of emergency services and gives the composition and structure of the system. In addition, this paper divides the business required by the system into modules, including registration module, doctor workstation, nurse workstation, query statistics module, decision-making module, and maintenance module. Finally, this paper suggests that in the process of the clinical triage, more ideas for improving the existing evaluation model should be proposed, and experience should be transformed into advantages, so as to improve emergency triage skills; establish an objective, quantitative, and scientific concept of emergency classification and triage; and fully realize scientific triage and precise triage.

Climate change affects public health, and improving eco-efficiency means reducing the various pollutants that are the result of economic activities. This study provided empirical evidence of the quantitative impact analysis of climate change on the health conditions of residents across China due to improvements that have been made to eco-efficiency. First, the indicators that were collected present adequate graphical trends and regional differences with a priori evidence about their relationships to each other; second, the present study applied Sensitivity Evaluation with Support Vector Machines (SE-SVM) to Chinese provincial panel data, taking the Visits to Hospitals, Outpatients with Emergency Treatment, and Number of Inpatients as proxy variables for the health conditions of the residents in each area and temperature, humidity, precipitation, and sunshine as the climate change variables, simultaneously incorporating the calculated eco-efficiency with six controlling indicators; third, we compared in-sample forecasting to acquire the optimal model in order to conduct elasticity analysis. The results showed that (1) temperature, humidity, precipitation, and sunshine performed well in forecasting the health conditions of the residents and that climate change was a good forecaster for resident health conditions; (2) from the national perspective, climate change had a positive relationship with Visits to Hospitals and Outpatients with Emergency Treatment but a negative relationship with the Number of Inpatients; (3) An increase in regional eco-efficiency of 1% increase the need for Visits to Hospitals and Outpatients with Emergency Treatment by 0.2242% and 0.2688%, respectively, but decreased the Number of Inpatients by 0.6272%; (4) increasing the regional eco-efficiency did not show any positive effects for any individual region because a variety of local activities, resource endowment, and the level of medical technology available in each region played different roles. The main findings of the present study are helpful for decision makers who are trying to optimize policy formulation and implementation measures in the cross-domains of economic, environmental, and public health.

Asthma is a common chronic condition, characterised by episodic acute exacerbations, which may require emergency treatment. According to Asthma UK, nationally 5 400 000 people are being treated for asthma; with life-threatening asthma attacks occurring every 10 seconds and three lives lost daily. In acute asthma, symptoms, including breathlessness, chest tightness, wheeze and cough, deteriorate progressively. Individuals with asthma symptoms often present to primary care and it is important that GPs can recognise and appropriately manage acute asthma. This article reviews the background, recognition and management of acute asthma in adults and children over the age of five, drawing on current guidelines.
