Prior research has suggested that a set of unique characteristics may be associated with adult cigarette smokers who are able to quit smoking using e-cigarettes (vaping). In this cross-sectional study, we aimed to identify and rank the importance of these characteristics using machine learning. During July and August 2019, an online survey was administered to a convenience sample of 889 adult smokers (age ≥ 20) in Ontario, Canada who tried vaping to quit smoking in the past 12 months. Fifty-one person-level characteristics, including a Vaping Experiences Score, were assessed in a gradient boosting machine model to classify the status of perceived success in vaping-assisted smoking cessation. This model was trained using cross-validation and tested using the receiver operating characteristic (ROC) curve. The top five most important predictors were identified using a score between 0% and 100% that represented the relative importance of each variable in model training. About 20% of participants (N = 174, 19.6%) reported success in vaping-assisted smoking cessation. The model achieved relatively high performance with an area under the ROC curve of 0.865 and classification accuracy of 0.831 (95% CI [confidence interval] 0.780 to 0.874). The top five most important predictors of perceived success in vaping-assisted smoking cessation were more positive experiences measured by the Vaping Experiences Score (100%), less previously failed quit attempts by vaping (39.0%), younger age (21.9%), having vaped 100 times (16.8%), and vaping shortly after waking up (15.8%). Our findings provide strong statistical evidence that shows better vaping experiences are associated with greater perceived success in smoking cessation by vaping. Furthermore, our study confirmed the strength of machine learning techniques in vaping-related outcomes research based on observational data.

This study investigated UK adults’ changes in cigarette smoking and vaping during the COVID-19 pandemic and factors associated with any changes. Data were from an online longitudinal study. A self-selected sample (n = 332) of 228 smokers and 155 vapers (51 participants were both smokers and vapers) completed 5 surveys between April 2020 and June 2021. Participants self-reported data on sociodemographics, COVID-19-related, and smoking/vaping characteristics. During the 12 months of observations, among smokers, 45% self-reported a quit attempt (27.5% due to COVID-19-related reasons) since the onset of COVID-19 pandemic and the quit rate was 17.5%. At 12 months, 35.1% of continuing smokers (n = 174) reported smoking less and 37.9% the same, while 27.0% reported an increase in the number of cigarettes smoked/day. Among vapers, 25.0% self-reported a quit attempt (16.1% due to COVID-19-related reasons) and the quit rate was 18.1%. At 12 months, 47.7% of continuing vapers (n = 109) reported no change in the frequency of vaping/hour, while a similar proportion reported vaping less (27.5%) and more (24.8%). Motivation to quit smoking and being younger were associated with making a smoking quit attempt and smoking cessation. Being a cigarette smoker was associated with vaping cessation. Among a self-selected sample, COVID-19 stimulated more interest in reducing or quitting cigarette smoking than vaping.

Abstract This randomized controlled trial aimed to evaluate the efficacy and ascertain the feasibility of a general health promotion approach that uses instant messaging to deliver brief motivational interviewing to help smokers with non-communicable diseases quit smoking. A total of 60 participants who had medical follow-up in a special out-patient clinic were randomized into two groups, 30 in the intervention group received brief motivational interviewing to assist them with their chosen behavioral changes, and 30 in the control group received only a smoking cessation booklet. The primary outcome, by intention-to-treat, was biochemically validated abstinence at 12 months. The intervention group had a higher biochemically validated abstinence rate than the control group at 12 months (16.7% vs 6.7% P=.23) although the difference was not statistically significant (Adjusted OR 2.4, 95% CI 0.43–13.75; P=.32). In addition, the intervention group had a significantly higher self-reported reduction of at least 50% in daily cigarette consumption than the control group at 6 months (Adjusted OR 7.2, 95% CI 1.22–42.44; P=.03).This study demonstrated the potential efficacy and feasibility of a general health promotion approach that uses instant messaging to deliver brief motivational interviewing to help smokers with non-communicable diseases quit smoking.

Evidence-based cessation assistance increases cessation rates. Activating preferences during decision making could improve effectiveness further. Decision aids (DAs) facilitate deciding by taking preferences into account. To develop effective DAs, potential end users' (i.e., individuals motivated to quit) needs and experts' viewpoints should be considered. Therefore, the aim of this needs assessment was: (1) To explore end users' needs and (2) to obtain consensus among smoking cessation counsellors and scientific experts to develop a self-administered DA to support end users in choosing cessation assistance. Data was gathered via two approaches: (1) twenty semi-structured interviews with potential end users and (2) two three-round Delphi studies with 61 counsellors and 44 scientific experts. Interview data and the first Delphi rounds were analysed qualitatively, the other Delphi rounds were analysed quantitatively. Potential end users acquired information in different ways, e.g., via own experiences. Important characteristics to decide between tools varied, however effectiveness and costs were commonly reported. Experts reached consensus on 38 and 40 statements, e.g., tools should be appropriate for users' addiction level. Although some trends emerged, due to the variation among stakeholders, a 'one size fits all'-approach is undesirable. This heterogeneity should be considered, e.g., by enabling users to customise the DA.

Összefoglaló. Bevezetés: Nagyszámú kutatás igazolta, hogy a dohányzás növeli a legjelentősebb krónikus betegségek kockázatát. Habár 2009 óta csökkenő tendenciát mutat Magyarországon a hagyományos dohányzók aránya, az e-cigarettát kipróbálóké az utóbbi években folyamatosan növekszik. Célkitűzés: A 2018-ban a felnőtt lakosság körében végzett Népegészségügyi Felmérés dohányzásra és e-cigaretta-használatra vonatkozó eredményeinek bemutatása az előző vizsgálatok tükrében. Módszer: A kérdőíves felmérésben 1586 fő került személyesen lekérdezésre. Az iteratív súlyozás a többlépcsős mintavételi designhatást és a 2016-os mikrocenzus adatait vette figyelembe. Eredmények: 2018-ban a dohányzók aránya a magyar felnőtt lakosság körében 28,7% (95% MT: 26,3–31,1%), az e-cigarettát használók aránya pedig 1,7% (95% MT: 1,1–2,5%) volt. Az iskolai végzettség a 65 év alattiak esetében a dohányzást befolyásoló tényező volt (EH: 3,32; 95% MT: 2,53–4,34), de a 65 éves és annál idősebb korcsoportban már nem (EH: 1,11; 95% MT: 0,59–2,09). Az e-cigarettát kipróbálók és használók között a leginkább említett (54,3% 95% MT: 44,0–64,5%) motivációs tényezőcsoport a dohányzásról való leszokással, az ártalomcsökkentéssel és a visszaesés megelőzésével volt kapcsolatos. A 65 éves és idősebb korcsoportban a dohányzók aránya 2015-höz képest emelkedett. 2018-ban az alapfokú iskolai végzettségűek körében volt a legmagasabb a dohányzók aránya, míg 2014-ben az érettségivel nem rendelkező középfokú végzettségűek körében. Következtetés: Bár összességében csökkent, az alacsony iskolai végzettségűek és az idősek körében emelkedett a dohányzók aránya Magyarországon. Az e-cigarettát kipróbálók és használók száma növekvő tendenciát mutat hazánkban. Orv Hetil. 2022; 163(1): 31–38. Summary. Introduction: The body of evidence suggests that smoking increases the risk of the most prevalent chronic diseases. Although the proportion of traditional smokers in Hungary has been on a declining trend since 2009, the proportion of those who tried e-cigarette has been steadily increasing in recent years. Objective: To present – in the light of previous studies – the results of the Public Health Survey among adults in 2018 on smoking and e-cigarette use. Method: 1586 persons were personally interviewed in a survey. The iterative weighting algorithm considered both the design effect of multistaged sampling and the 2016 Hungarian microcensus. Results: In 2018, the proportion of smokers in the Hungarian adult population was 28.7% (95% CI 26.3–31.1%), and the proportion of e-cigarette users was 1.7% (95% CI 1.1–2.5%). Educational level was a predictor of smoking among respondents younger than 65 years old (OR 3.32; 95% CI 2.53–4.34), but not for those aged 65 years or older (OR 1.11; 95% CI 0.59–2.09). Among e-cigarette ever or current users, the most commonly mentioned (54.3% 95% CI 44.0–64.5%) motivational factor-group to try or use e-cigarettes included motivations to quit smoking, to reduce harm, and to avoid relapsing. In the population aged 65 years old or older, the proportion of smokers increased compared to 2015. The proportion of smokers was the highest among those with primary education in 2018, while in 2014, it was the highest among those with secondary education without a graduation certificate. Conclusion: In Hungary, although overall smoking rates are declining, the smoking rate in the low educational group and among the elderly increased. The number of people trying or using e-cigarettes is showing an increasing trend in our country. Our results call for a complex public health intervention program including prevention and smoking cessation supporting subprograms with high focus on those with primary education. ObjectivesTo examine the effect of Korea’s 2015 tax policy, discuss its effectiveness and limitations and present future directions for tax policy in the context of the tobacco endgame.DesignA retrospectively reconstructed cohort study.SettingKorea, August 2014–October 2015.ParticipantsThe study examined 41,605 male smokers aged 19 years and older who participated in the 2015 Korea Community Health Survey.Measures and analysisBinary and multinomial logistic regression was used to assess the impact of the tax policy on smoking-related behaviour. We adjusted for demographic and health-related variables.ResultsAmong 41,605 men who were smokers in 2014, 15,499 (35.85%, weighted) reported being affected by the price increase. Of all smokers, 1,772 (3.96%, weighted) reported quitting smoking because of the tobacco price increase. Others reduced their smoking amount (n=9,714, 22.48%, weighted) or made other changes such as switching brands (n=4,013, 9.41%, weighted). An additional 2,401 smokers (5.72%, weighted) quit smoking for reasons other than the tobacco price increase. Compared with those in the highest quintile of household income, the odds that those in the lowest quintile quit smoking due to the price increase were almost twice as high (OR=1.98, 95% CI 1.54 to 2.54).ConclusionsKorea’s 2015 tobacco price increase affected a significant number of smokers within a year, especially in the lowest income group, inducing some to quit or reduce their smoking amount. However, more smokers quit for reasons independent of the price change. Tax policy can effectively reduce smoking, but needs to be combined with other policies for optimal results.

We conducted this systematic review to document the published literature related to smoking behaviors and attitudes towards the smoke-free campus policy. Studies on universities that had implemented the smoke-free campus policy were included in this review. The search for published articles from January 2010 to December 2020 involved three main electronic databases: Ovid MEDLINE, ScienceDirect, and Scopus. We searched the databases with the following Boolean string: [(smoke-free OR tobacco-free) AND (campus OR university OR college) AND (knowledge* OR attitude* OR practice*)]. Seventeen studies were included in this review. The majority (n = 8) were from the United States, followed by Saudi Arabia (n = 2) and one each from Israel, Lebanon, Australia, Canada, the United Kingdom, Spain, and China. Eight studies reported a positive impact of the policy on smoking behavior (plan to quit smoking, attempt to quit smoking, reduce smoking). However, 11 studies reported that respondents were still exposed to second-hand smoke and that cigarette butts were still scattered around the university area. Nine studies reported negative attitudes towards smoking, and seven of 12 studies reported positive attitudes towards the policy. Overall, the smoke-free campus policy had mixed impacts. Nevertheless, we found that attitude towards a smoke-free campus and smoking behavior has a good impact.

OBJECTIVE: While the relationship between recreational cannabis and nicotine use is well established, little is known about nicotine use among users of cannabis for therapeutic purposes (CTP). METHOD: Patients attending a medical marijuana dispensary (N = 697; 75.3% White; 60.0% male) completed a survey examining nicotine use, routes of administration of nicotine and cannabis, and CTP qualifying conditions. RESULTS: More than one-third (39.3%) of participants reported current nicotine use. Compared to exclusive cigarette smokers, non-users of nicotine, and e-cigarette users were approximately four times more likely to vape, rather than to smoke, cannabis. Almost a third of those reporting e-cigarette use (30.6%) and more than half (56.7%) of those reporting smoking cigarettes reported stopping for one day or longer in the past year because they were trying to quit. Furthermore, 46.8% of cigarette smokers reported plans to quit smoking in the next six months (but not in the next month) and an additional 31.6% planned to quit in the next month. Having a psychiatric condition was associated with nicotine use and higher motivation to quit smoking. CONCLUSIONS: Users of CTP are more likely to use nicotine products than the general population. Importantly, the route of administration of nicotine products is related to the route of administration of CTP. If aerosolized (vaped) CTP is a less harmful route of administration than smoked CTP, dispensary staff should be aware of this relationship and take this into account when recommending a non-combustible route.
