A 51-year-old woman presented with a massive right-sided hydrothorax after 84 days on CAPD. The presence of dialysate in the right pleural cavity was demonstrated by the recovery from the fluid of intraperitoneal methylene blue injected into the peritoneal space. Talcage of the pleura was partly successful in preventing recurrence of the pleural fluid and CAPD was continued without further major problems. This paper has discussed the pathophysiology and treatment of this serious complication of CAPD.
