Context: Hockey is a skillful contact sport with an elevated injury risk at higher levels of play. An understanding of injury incidence, type, mechanism, and severity at various levels of competition aids the clinician treating these athletes. The purpose of this clinical review is to discuss the epidemiology of hockey injuries at various levels of participation, including youth, high school, junior, college, and professional. Evidence Acquisition: A literature search was performed by a review of PubMed, Embase, and Cochrane databases and included articles published from 1988 to 2017. Studies were included in this review if determined to be of high quality and containing injury data relevant to the levels of competition. Pertinent data regarding ice hockey injury epidemiology and prevention at various levels of competition were analyzed. Study Design: Clinical review. Level of Evidence: Level 4. Results: Youth, high school, junior, college, and professional hockey players have unique injury patterns. Injuries occur much more often in a game compared with a practice, and injury risk increases with each level of competition. Preventative measures, such as mandatory facial protection and delayed body checking in games until age 13 years, are proven strategies to reduce the risk of facial injury and concussion. Conclusion: An understanding of common injury types and mechanisms according to age and level of play aids the clinician in diagnosis and management. This information can also guide preventative strategies in the areas of education, coaching, rule enforcement, rule modifications, equipment improvement, and sportsmanship.

Background: Body checking is the most common injury mechanism in ice hockey. Rule changes have sought to mitigate body checking exposure among youth players. In 2011, USA Hockey changed the legal body checking age from Pee Wee (11/12-year-olds) to Bantam (13/14-year-olds). Interestingly, Bantam players with checking experience during Pee Wee had a lower concussion risk relative to Bantam players without checking experience in a sample of Canadian youth hockey players. Understanding the head impact biomechanics underlying these findings could further elucidate the consequences of this rule change. Purpose: To determine the association between Pee Wee checking exposure and head impact biomechanics in a cohort of Bantam players. Methods: We prospectively collected data on Bantam ice hockey players during the 2006/07-2009/10 seasons and the 2012-2013 season. The 2006/07-2009/10 cohort (n= 61, age=13.9±0.5 years, height=168.2±8.7 cm, mass=59.9±10.4 kg) was allowed to body check (BC) as a Pee Wee player. The 2012-2013 cohort (n=15, age=13.3±0.4 years, height=167.5±7.4 cm, mass=57.5±8.6 kg) was not permitted to body check (NBC) as a Pee Wee player. Over the course of each season, head impacts were measured using in-helmet accelerometers. Only head impacts with linear acceleration ≥10 g were included in our analysis. Main outcome measures were mean linear acceleration (g) and rotational acceleration (rad/s2). Levene’s tests assessed equality of variance between groups. We employed mixed effects models to assess group differences in mean linear and rotational acceleration between BC and NBC groups. Results: The BC and NBC groups did not differ in height (t74=0.28, p=0.78) or mass (t74=0.84, p=0.40). When assessing group differences in head impact biomechanics, the NBC experienced significantly greater linear acceleration (F1,74=4.36, p=0.04) and greater rotational acceleration (F1,74=21.2, p<0.001) relative to the BC group. On average, the NBC group experienced 23.1 ± 0.87 g linear acceleration and 1993.5 ± 68.4 rad/s2 rotational acceleration compared to the BC group, which experienced 21.2 ± 0.30 g linear acceleration and 1615.9 ± 45.2 rad/s2 rotational acceleration. Conclusions: Bantam ice hockey players without body checking experience during their Pee Wee years experienced greater average linear and rotational acceleration relative to players with Pee Wee body checking experience. While removing body checking from Pee Wee ice hockey may reduce short-term injury risk, these athletes may demonstrate more high-risk head impact biomechanics when legally allowed to body check. Future research should continue to examine the influence of policy changes on head impact biomechanics and injury risk in youth ice hockey. [Figure: see text]

Context Historically, methods of monitoring training loads in runners have used simple and convenient metrics, including the duration or distance run. Changes in these values are assessed on a week-to-week basis to induce training adaptations and manage injury risk. To date, whether different measures of external loads, including biomechanical measures, provide better information regarding week-to-week changes in external loads experienced by a runner is unclear. In addition, the importance of combining internal-load measures, such as session rating of perceived exertion (sRPE), with different external-load measures to monitor week-to-week changes in training load in runners is unknown. Objective To compare week-to-week changes in the training loads of recreational runners using different quantification methods. Design Case series. Setting Community based. Patients or Other Participants Recreational runners in Vancouver, British Columbia. Main Outcome Measure(s) Week-to-week changes in running time, steps, and cumulative shock, in addition to the product of each of these variables and the corresponding sRPE scores for each run. Results Sixty-eight participants were included in the final analysis. Differences were present in week-to-week changes for running time compared with timeRPE (d = 0.24), stepsRPE (d = 0.24), and shockRPE (d = 0.31). The differences between week-to-week changes in running time and cumulative shock were also significant at the overall group level (d = 0.10). Conclusions We found that the use of an internal training-load measure (sRPE) in combination with external load (training duration) provided a more individualized estimate of week-to-week changes in overall training stress. A better estimation of training stress has significant implications for monitoring training adaptations, resulting performance, and possibly injury risk reduction. We therefore recommend the regular use of sRPE and training duration to monitor training load in runners. The use of cumulative shock as a measure of external load in some runners may also be more valid than duration alone.

Objective Concussion is a common injury in ice hockey, and previous research suggests some misconceptions and unsafe attitudes amongst players. The purpose of this study was to assess sport concussion knowledge, attitudes and the effect of sport concussion history in UK-based male ice hockey players across three levels of competition: professional, semi-professional and amateur. Methods Sixty-one participants across a number of UK ice hockey teams completed the Rosenbaum Concussion Knowledge and Attitudes Survey and reviewed a series of statements to assess knowledge (concussion knowledge index), attitudes (concussion attitude index) and misconceptions of concussion. Results Level of competition and concussion history had no significant effect on concussion knowledge index or concussion attitude index. A positive significant relationship exists between playing experience and concussion knowledge index and concussion attitude index. Statements identified common misconceptions and areas of accurate knowledge regarding concussion symptoms suggesting that male ice hockey players have a higher level knowledge compared to a sample of the UK general public. Playing experience was associated with increased knowledge and increasingly safe attitudes towards concussion. Conclusion Despite knowledge relating to loss of consciousness and correct management of symptoms being generally accurate, there are worryingly unsafe attitudes regarding aspects of concussion. Such attitudes may well pose significant threats to players’ safety and long-term health.
