Objective: To evaluate the appropriateness of the use of hip joint replacements (HJRs) using explicit criteria developed by an expert panel. Methods: Observational study. Nine hundred ninety-seven patients from five hospitals with a diagnosis of osteoarthritis, avascular necrosis, hip fracture, or revision who were undergoing HJR were consecutively included in the study during a 1-year period. The appropriateness of the indication was judged by explicit criteria. Complications were recorded at the time of the intervention and 3 months postoperatively. Results: Of the 1,030 interventions, 604 were for osteoarthritis, 31 avascular necrosis, 191 fractures, and 204 revisions. No differences were found among the hospitals for the main clinical and patient variables. Indications for surgery were considered appropriate in 59% of cases, uncertain in 32%, and inappropriate in 8%, mainly in the osteoarthritis group. Differences were found in the rates of appropriateness among some centers. The complication rate did not differ among the groups based on the level of appropriateness of the procedure. Conclusions: The appropriate use of HJR, as measured by the criteria established by the panel, identified a moderate percentage of inappropriate indications. Those equivocal and inappropriate cases demand further studies to identify patients with an adequate risk-to-benefit ratio from this procedure.

Avascular necrosis is a disease in which cellular death of bone component occurs due to interruption of the blood supply. Bone fractures, joint dislocations, alcoholism and long-term use of steroids are the commonly found risk factors of the disease. The disease generally happens in 35 to 60 years old population and commonly affects hip joint. About 10,000 to 20,000 people usually develop osteonecrosis of head of femur yearly in United States. This case deals with a diagnosed case of avascular necrosis of the femoral head in a 68 years old female. Patient had been suffering from pain in the left hip joint since 5 years. She had been under allopathic conservative treatment for her complaints, but symptoms aggravated rapidly since 4 months. So, for further management, she came to Out Patient Department of Panchakarma, Rishikul Campus where three sittings of local application of Mustadi Upanaha on left hip region along with Taila Dhara with Dhanwantaram Taila was performed. After three sittings, she got significant relief in joint pain and her quality of life. The assessment was done based on both subjective as well as objective parameters after each sitting. This study reveals that Panchakarma procedure like Mustadi Upnaha, Taila Dhara provided a significant relief in this case.

Avascular necrosis of the femoral head is a complex medical and social problem. The rapid development of the disease, the complexity of diagnosis and the prevalence among young patients leads to impaired hip joint function and further disability. To improve the quality of life of patients, the Orthopedic Department of the Traumatology Center of Khabarovsk developed a method of minimally invasive two-stage decompression. Surgical treatment was performed in 30 cases in patients with ANFH of stages 0, 1, 2 (by ARCO). Evaluation of treatment results was carried out before operative treatment, after 6 and 12 months. 12 months after surgical treatment, positive dynamics was noted, namely, pain reduction, a walking distance increase, abandonment of crutches, opportunity to use public transport, ability to sit for a long time in one and the same position, to put on shoes, and the abandonment of constant intake of non-steroidal anti-inflammatory drugs. The assessment of hip joint function in 20% of cases showed an excellent result, in 26,6% of cases — a good result, in 40% — satisfactory, and in 13,3% (4 people) — unsatisfactory result, which proves the effectiveness of the proposed treatment method and the feasibility of its use.
