Genetic epidemiological study of the population of the North Ossetia-Alania Republic, Russia, is caring out by the Laboratory of Genetiс Epidemiology of Research Centre for Medical Genetics (Moscow, Russia).Within the framework of the standard protocol we study the maximum of possible population-genetic characteristics. Based on the 27583 marriage records, the index of endogamy, the intensity of metisation and ethnic marriage assortativness in 8 regions of North Ossetia and Vladikavkaz have been analyzed. The differences in the marriage and migration characteristics are revealed for two subethnoses: the Irons and the Digors. The Digors have a higher value of endogamy index and a lower metisation level than the Irons have.

Objective: To evaluate the appropriateness of the use of hip joint replacements (HJRs) using explicit criteria developed by an expert panel. Methods: Observational study. Nine hundred ninety-seven patients from five hospitals with a diagnosis of osteoarthritis, avascular necrosis, hip fracture, or revision who were undergoing HJR were consecutively included in the study during a 1-year period. The appropriateness of the indication was judged by explicit criteria. Complications were recorded at the time of the intervention and 3 months postoperatively. Results: Of the 1,030 interventions, 604 were for osteoarthritis, 31 avascular necrosis, 191 fractures, and 204 revisions. No differences were found among the hospitals for the main clinical and patient variables. Indications for surgery were considered appropriate in 59% of cases, uncertain in 32%, and inappropriate in 8%, mainly in the osteoarthritis group. Differences were found in the rates of appropriateness among some centers. The complication rate did not differ among the groups based on the level of appropriateness of the procedure. Conclusions: The appropriate use of HJR, as measured by the criteria established by the panel, identified a moderate percentage of inappropriate indications. Those equivocal and inappropriate cases demand further studies to identify patients with an adequate risk-to-benefit ratio from this procedure.

Avascular necrosis is a disease in which cellular death of bone component occurs due to interruption of the blood supply. Bone fractures, joint dislocations, alcoholism and long-term use of steroids are the commonly found risk factors of the disease. The disease generally happens in 35 to 60 years old population and commonly affects hip joint. About 10,000 to 20,000 people usually develop osteonecrosis of head of femur yearly in United States. This case deals with a diagnosed case of avascular necrosis of the femoral head in a 68 years old female. Patient had been suffering from pain in the left hip joint since 5 years. She had been under allopathic conservative treatment for her complaints, but symptoms aggravated rapidly since 4 months. So, for further management, she came to Out Patient Department of Panchakarma, Rishikul Campus where three sittings of local application of Mustadi Upanaha on left hip region along with Taila Dhara with Dhanwantaram Taila was performed. After three sittings, she got significant relief in joint pain and her quality of life. The assessment was done based on both subjective as well as objective parameters after each sitting. This study reveals that Panchakarma procedure like Mustadi Upnaha, Taila Dhara provided a significant relief in this case.
