Background. Müllerian duct malformations delineate a miscellaneous group of congenital anomalies that result from arrested development, abnormal formation, or incomplete fusion of the mesonephric ducts. Case. This paper describes the diagnosis and management of a noncommunicating rudimentary horn complicated by severe pelvic pain and associated endometriosis. Conclusion. This condition was diagnosed by laparoscopy and hysteroscopy examination. Operative videolaparoscopy proved to be a successful approach for the treatment of this congenital Müllerian anomaly.

Rudimentary horn pregnancy is a rare obstetric entity and the diagnosis and management may pose some problems especially in a low-resource center. We report a case of G1P0 with pregnancy of 28 weeks gestation with diagnosed intrauterine fetal death, which was suspected as extra uterine pregnancy bases on failed attempts of termination of pregnancy and on laparotomy, a dead fetus of 16 weeks was found in left unruptured noncommunicating horn of unicornuate uterus. Diagnosis of rudimentary horn pregnancy is difficult and can be missed in ultrasound. It requires a high index of suspicion.

A 45-year-old woman admitted to our hospital complaining of perimenopausal uterine bleeding not responding to medical treatment. Ultrasound evaluation revealed unicornuate uterus with adenomyosis and it was so difficult to see the distant small left rudimentary horn on ultrasound. The patient underwent laparotomy with total hysterectomy for both horns and was sent to pathologist that indicated adenomyosis and non-communicating non-cavitated left rudimentary horn.

Unicornuate uterus with rudimentary horn is a rare type of anomaly. The incidence is approximately 1/100,000. Mullerian anomalies are associated with many obstetrical and gynecological complications. Non-communicating and functional rudimentary horn can cause severe pain in abdomen due to accumulation of the blood causing its distension. Authors report a case of 25 years old patient with unicornuate uterus with non-communicating rudimentary horn containing functional endometrium presenting with chief complaints of severe pain in lower abdomen associated with multiple episodes of vomiting.
