Objective: Many techniques have been developed to enhance the gingival thickness, gingival level and emergence profile around the implant in the esthetic zone. Introduction: In this study, a buccal rotational flap was used to improve the implant site in the esthetic zone and increase gingival tissue thickness. Methods: Two cases involved the use of a rotational flap during second-stage implant surgery, one case involved the use of a temporary crown with a healing abutment, and another case involved the use of a healing abutment. Result: The cases were followed up until the final crown was placed. The implant site was improved in 2 cases; the gingival thickness increased, the gingival level was enhanced and the emergence profile was developed. Conclusion: Many factors affect the results of a rotational flap; some factors are surgical, while others are prosthetic, biological and anatomical.

Pendahuluan : celah alveolar (gnatoschisis) merupakan salah satumolformasi kongenital yang terjadi pada struktur mulut dan wajah.Banyak permasalahan dapat terjadi bila penyakit ini tidak dilakukanperawatan seperti ketidakstabilan struktur tulang maksila, dukunganskletal yang kurang pada dasar hidung, fistula oronasal yang berulang,dukungan periodontal yang kurang sehingga terjadi retensi makanandan diastema antara gigi dan region celah atau insisivus yang tidakerupsi. Tujuan: Melaporkan sebuah kasus penatalaksanaan fistulaoronasal. Kasus dan penatalaksanaan : Seorang wanita berusia 43tahun mengeluhkan celah pada gusinya yang berlokasi antara insisivussentral dan insisivus lateral maksila kiri. Celah tersebut membuatmakanan selalu masuk pada rongga hidung. Pasien pernah dilakukanoperasi sebelumnya untuk penutupan celah bibir dan langit-langit.Pemeriksaan intra oral menunjukkan terdapat sebuah fistula oronasalpada bagian vestibulum pada regio 21 dan 22. Penatalaksanaan darikasus ini adalah penutupan oronasal fistula pada vestibulummenggunakan teknik pedicle rotational flap dan autograft dari tulangsimphisis mandibula. bedahan. Simpulan : Hasil perawatanmenunjukkan penyembuhan dan tidak terdapatnya keluhan setelahperawatan. Pemeriksaan radigrafi menunjukkan terjadinya penyatuanantara tulang dan bonegraft pada darah alveolar regio 21 dan 22.

We treated 16 patients with 17 hypoplastic thumbs (eight Type IIIB and nine Type IV) using a non-vascularized fourth metatarsal transfer with a rotational flap and multi-staged reconstruction. The average age at the first operation was 24 months. The average follow-up time was 46 months. All patients achieved reasonable hand function and were able to use the reconstructed thumb to grip small and large objects. The operated thumb achieved an average Kapandji score of 6.7 and average pinch strength of 0.9 kg. There were two cases of graft nonunion. All parents are satisfied with the function and appearance of the reconstructed thumb and donor foot. We conclude that non-vascularized fourth metatarsal transfer is a feasible reconstruction method for patients with Types IIIB and IV hypoplastic thumbs. The reconstruction allows for the preservation of a 5-digit hand with reasonable function and appearance and minimal donor site morbidity, although long-term growth of the metatarsals still need to be monitored.
