As minimally invasive techniques in radiology suites become more common, the need for anesthesia support will increase. While recognizing and addressing a patient’s comorbidities and other concerns are similar to what is already done in the surgical setting, the additional requirements and constraints of the imaging environment and the procedure are unique and call for specific solutions. Just as in the operating room, there is frequently no single best anesthetic technique for a given procedure. The anesthetic should be aligned with the demands of the procedure and the skill sets of the providers. Patient safety always takes precedence, and the location should never be permitted to compromise care.

Medical confidentiality is of individual and of general interest. Medical confidentiality is not absolute. European countries differ in their legislative approaches of consent for data-sharing and lawful breaches of medical confidentiality. An increase of interference by the legislator with medical confidentiality is noticeable. In the Netherlands for instance this takes the form of new mandatory duties to report resp. of legislation providing for a release of medical confidentiality in specific situations, often under the condition that reporting takes place on the basis of a professional code that includes elements imposed by the legislator (e.g. (suspicion of ) child abuse, domestic violence). Legislative interference must not result in the patient loosing trust in healthcare. To avoid erosion of medical confidentiality, (comparative) effectiveness studies and privacy impact assessments are necessary (European and national level). Medical confidentiality should be a subject of permanent education of health personnel.

ABSTRACT Background: Congenital malformations of lung, such as congenital lobar emphysema, cystic adenomatoid malformation, bronchial cyst, pulmonary sequestration are of rare occurrence. However, our hospital being in a rural area attracts a large number of patients with complex malformations. Method: This retrospective analysis is done to evaluate diagnostic accuracy, safety and efficacy and outcomes of open lung resections at a rural hospital. Results: 16 children with congenital lung malformation underwent open resection in a rural hospital, with no mortality and minimal morbidity, no complications and more than 24 months follow-up. Conclusion: Open lung resections are found to be safe, effective in a rural setting also.
