Interdisciplinary approach in management of women’s chronic pelvic pain

This review represents a look at the problem of chronic pelvic pain syndrome of obstetrician-gynecologist and neurologist. The necessity of an interdisciplinary approach at different stages of medical care for these women is shown in the review (from prevention and diagnosis to treatment and the establishment of a long-term medical supervision of these patients). The existing contradictions in this issue are shown and the necessity of creating a single classification and elaborating individual approaches to treatment is underlined. Key words: chronic pelvic pain, chronic pelvic pain syndrome, interdisciplinary approach.

Chronic pelvic pain syndrome (CPPS) is equally common in both men and women, causes worsening quality of life, social isolation and disability. The treatment of CPPS requires long-term pharmacotherapy associated with the development of class-specific side effects of nonsteroidal anti-inflammatory drugs (NSAIDs). Genetic study of the carriage of polymorphic alleles of the CYP2C9 gene involved in the metabolism of non-steroidal anti-inflammatory drugs (NSAIDs) prescribed to patients with CPPS is an urgent and in-demand task of modern healthcare. This study allows us not only to determine the genotypes of patients with CPPS but also to identify ways of personalized approach to therapy.

Chronic pelvic pain syndrome in women is the multidisciplinary problem with the complex multicomponent pathogenesis. Pelvic pain is the result of the convergence of three pathological processes: reducing the threshold of pain sensitivity, the emergence of persistent generators of pathological polymodal afferent impulses, violations of efferent impulses. Among the most significant changes that initiate and maintain chronic pelvic pain, there are such as: various venous disorders, violations of vegetative innervation, chronic inflammatory processes, myofascial syndromes, somatic depression and conversion disorders. Treatment of chronic pelvic pain is difficult, long-term and insufficiently effective process. Acupuncture can provide significant assistance in the treatment of this syndrome.
