One of the main medical and social problems is disability, which causes economic losses, both for citizens and for the state. This article presents an analysis of the indicators of disability and disability due to diseases of the ear and mastoid process in Kyrgyzstan for the period 2014–2019. In the republic in general and in the Osh region in particular, there is a slight increase in persons with disabilities among the adult category of the population in all classes of diseases under medical supervision in health organizations from 2014 until 2019 and a slight decrease in 2019. With the regional distribution of the frequency of the first recognized by persons with disabilities by diseases of the ear and mastoid process among the adult population, the Osh region ranks third, among children under 18 years old - seventh in rank. In the republic, against the background of the trend of a stable decrease in the number of adults with disabilities for diseases of the ear and mastoid process registered, an increase was observed among children under 18 years old only in 2018. Against the background of a decrease in the frequency of primarily recognized persons with disabilities for diseases of the ear and mastoid process among the category of adult population of the republic, children under 18 years old showed an increase in 2017 and 2018. In the Osh region, among adults, an increase was revealed in 2015 and 2016, and among children under 18 years old, against the background of a stable indicator for four years, a sharp increase in the indicator was in 2018. There is a need for a unified and integrated approach to disability-specific rehabilitation, as well as for individual rehabilitation and habilitation programs for persons with disabilities, with close inter-agency cooperation.

Communication between soldier at border line is crucial. Existing system used for communication between soldiers at border line in military consumes a lot of power. The greatest difficulties in Indian armed forces operation is the Soldiers are not able to do transmission of messages with headquarters base station controller in case of emergency or when needed any help. Also, the current status and location of the soldiers cannot be detected with this system. The proposed methodology gives us Long Range (LoRa) based medical supervision and emplacement trailing and tracking system for soldiers. This type of advanced design can be mounted on the soldier’s shoe to ensure their safety. In case of death of the soldier, the controller intimates to the camp office control along with soldier’s location. The proposed system includes sensors, GPS, and transmission modules, as well as miniaturized wearable physiological equipment. Hence, it is possible to implement a low-cost mechanism to provide needed help in the battlefield.

A case of severe systemic osteoporosis complicated by multiple fractures of the ribs and vertebrae in a young patient with late diagnosis of Still's disease as a result of independent long-term uncontrolled use of corticosteroids is presented. Lack of medical supervision, taking a high dose (35 mg / day) of prednisolone without recommendation and control of treatment led to untimely recognition and treatment of the underlying disease and the development of irreversible disabling transformations of the skeleton.

Regular medical supervision represents a fundamental component of the clinical management of obesity. In fact, when frequently supplied it reduces the risk of failure associated with any body weight reduction program (BWRP), resulting in body weight gain. The aim of the present study was to establish the potential beneficial effects of increasing medical supervision on weight loss and other auxometric and cardiometabolic parameters in a population of children and adolescents with obesity (n = 158; F/M = 94/64; age range 9.7–17.3 years; body mass index, BMI = 37.8 ± 6.9 kg/m2), followed up for one year in a real-world setting, after and before a 3-week in-hospital BWRP. Weight loss was significantly associated with medical supervision and changes in several auxometric and cardiometabolic parameters such as fat mass, fat-free mass, waist and hip circumferences, total and LDL cholesterols, triglycerides, glucose, insulin, HOMA-IR, systolic blood pressure and IDF criteria for the diagnosis of metabolic syndrome. As expected, weight loss and, congruently, medical supervision, were significantly higher in responsive and stable subjects than in those belonging to the non-responsive group and in responsive subjects than those belonging to the stable group. While weight loss was significantly higher in subjects having class 2 and 3 obesity than those belonging to class 1 obesity group, medical supervision was significantly higher in subjects having class 3 than those having class 1 obesity. Weight loss was significantly higher in subjects suffering from metabolic syndrome than those without; nevertheless, no significant difference was found in medical supervision between these groups. Finally, sex was associated with no differences in weight loss and medical supervision. In conclusion, based on the results of a real-world experience, frequent medical supervision increases the weight loss associated with a longitudinal multidisciplinary BWRP, with a parallel improvement of a set of auxometric and cardiometabolic parameters. Prospectively, incentivising regular medical supervision should reduce the risk of BWRP failure and body weight gain, thus contributing to counteract the detrimental transition from simple obesity to metabolic syndrome in pediatric patients.

Abstract Background and Aims Serum potassium (K+) exhibits a u-shaped association with mortality but uncertainty exists regarding optimal thresholds for survival and influencing factors. We examined the impact of serum K* on mortality in the Irish Health System with particular focus on kidney function and location of medical supervision. Method We utilised data from the Irish Kidney Disease Surveillance System (NKSS) to explore the association of serum K+ and mortality in a longitudinal cohort study. We identified all adult individuals (age > 18 years) who accessed health care from 2007 and 2012 in a regional health system with complete data on serum K+, associated laboratory indicators and vital status up to 31st December 2013 (n = 32,643). We randomly selected a single K+ measurement per patient with date of measurement as index date. Chronic kidney disease was defined as eGFR <60ml/min/1.73m² vs greater recorded at index date. Location of medical supervision was recorded as emergency room, inpatient location; outpatient clinic, and general practice location. The association of serum K+ was explored in categories and as a continuous variable in restricted cubic splines with mortality. Multivariable Cox regression determined hazard ratios and 95% confidence intervals with adjustment for baseline health indicators. Results Mean age was 57.1 years, 5,056 died (15.4%) with a median follow-up of 5.1 years. With adjustment, for age, sex, baseline health status, and location of medical supervision, the pattern of mortality was non-linear and u-shaped with greatest risks for patients with extreme values. Modelled as a continuous variable, the serum K +thresholds for optimal survival were from 4.1 to 5.2 mmol/L. Compared to patients without baseline CKD, the risks were attenuated for patients with CKD (p-value interaction 0.012). The associated risk thresholds were wider for CKD patients with significant increased risk above 5.8 mmol/L whereas, for those without CKD, serum K +thresholds for optimal survival were between 4.2-5.4 mmol/L. Similarly, mortality patterns were greatly attenuated for patients who were managed in the outpatient and general practice locations (p-value interaction <0.001) than the emergency room or inpatient settings (Figure 1). Conclusion Risk thresholds for optimal survival for serum K+ vary according to CKD and location of medical supervision in real-world clinical cohorts. Better understanding of these thresholds and effect modifiers are essential for inform decision making and therapeutic interventions. The variety of clinical manifestations and the absence of strictly specific features make tuberculosis and toxocariasis difficult to diagnose and treat. Doctors of various specialties are engaged in the diagnosis of toxocariasis, depending on which organ system is affected in a person and with whatsymptoms he turned to a medical institution.Purpose: to describe the case of detecting toxocariasis in a child who was treated in a tuberculosis hospital from a bacillary family contact with pronounced indicators of immunological tests and revealed changes during CT scan of the chest organs.A complex differential diagnostic path and constant medical supervision made it possible to establish a rather rare disease - toxocariasis in a child from family contact with a patient with tuberculosis.Conclusion. In the differential diagnostic series in patients withtuberculosis, the presence of parasitic diseases with a similar clinical and radiological picture cannot be excluded.

Drug abuse is an insurmountable problem. To solve the problem of drug abuse can not only be solved by one party. Currently, many parties, both from the government, students, and NGOs, have voiced and cared about this drug problem, but this has not been able to solve the problem. . (2) How is KAPA Narkoba's efforts in preventing drug abuse among students in Pemalang Regency? Drug abuse is use outside of medical purposes, without medical supervision and is an act against the law. Drug abuse is a process that progressively increases from the stage of trial and error to the level of use for entertainment, site use, regular use to addiction. Penyalahgunaan narkoba menjadi masalah yang belum bisa teratasi. Untuk menyelesaikan masalah penyalahgunaan narkoba tidak hanya bisa diselesaikan oleh salah satu pihak saja. Saat ini banyak pihak baik dari pemerintah, mahasiswa, maupun LSM yang telah menyuarakan dan peduli dengan masalah narkoba ini namun ini belum bisa mengatasi permasalahan tersebut  Berdasarkan uraian dalam latar belakang maka permasalahan yang akan diteliti adalah: (1) KAPA Narkoba Sebagai wadah organisasi Pembinaan Pelajar, (2) Bagaimana upaya KAPA Narkoba dalam mencegah penyalahgunaan narkoba di kalangan pelajar Kabupaten Pemalang? Penyalahgunaan narkoba adalah penggunaan di luar keperluan medis, tanpa pengawasan dokter dan merupakan perbuatan melanggar hukum. Penyalahgunaan narkoba merupakan suatu proses yang makin meningkat dari taraf coba-coba ke taraf penggunaan untuk hiburan, penggunaan situsional, penggunaan teratur sampai kepada ketergantungan.
