Chronic hepatitis B virus infection is a major cause of cirrhosis and hepatocellular carcinomaworldwide. Antiviral treatment can safely and effectively suppress viral replication and reduce riskof disease progression but need long term therapy. However, understanding the natural history ofhepatitis B virus infection is important when deciding whether to treat or not to treat the patients.On the basis of virus-host interaction, the natural course of HBV infection can be divided into four phases.The elevation of alanine aminotransferase (ALT) level is an important indicator of immune-mediatedinflammation to eliminate HBV infected hepatocyte.
