In the summer of 2020, an elderly woman in her eighties sought medical attention due to headache and visual disturbances. The diagnosis was unclear, she became blind on both eyes and developed extended scalp necrosis. Later it was clear that these symptoms were due to accelerating symptoms of giant cell arteritis (GCA). GCA is one of the most common disease form of vasculitis and can have various symptoms. In this report, we describe a case of an advanced disease course, discuss the main symptoms, diagnosis and treatment.

Abstract Giant cell arteritis (GCA) can be a rheumatologic complication of checkpoint inhibitor immunotherapy for the treatment of cancer. A rare and serious manifestation of GCA is scalp necrosis. We report a case of a patient who developed GCA and subsequent scalp necrosis after the initiation of immunotherapy with the checkpoint inhibitor pembrolizumab for the treatment of metastatic melanoma. Such advanced cases of GCA may be increasingly recognized in the immunotherapy era of oncology.

Giant-cell arteritis (GCA), also referred to as temporal arteritis, is the most common primary vasculitis of the elderly involving the extracranial branches of the carotid arteries, in particular, the temporal artery. Patients usually present with temporal headaches, visual impairment, fever, and scalp tenderness. Scalp necrosis associated with GCA is a rare occurrence with approximately 100 cases reported in the literature to date. It is a therapeutic emergency requiring urgent management as it may lead to irreversible loss of vision. To increase awareness of this severe complication, we report a patient with a scalp necrosis revealing a GCA.
