BACKGROUND: Some studies suggested that saline infusion sonohysterography (SIS) has been used to detect uterine cavity abnormalities before in vitro fertilization (IVF) cycles to improve treatment success rates and decrease the number of cycle cancellations and embryo implantation failures. Some of the factors contributing to the dissemination and acceptance of the technique include the fact that it is a simple, less painful, less expensive, less invasive, and well-tolerated procedure when compared to hysteroscopy. AIM: The aim of the study is to evaluate uterine cavity abnormality with SIS performed before IVF. METHODS: A descriptive retrospective study involving 551 female partners who had SIS before IVF/ICSI treatment at private Halim Fertility Center from January 2014 until December 2017. Five hundred and fifty-one infertile woman was included in this study before IVF/ICSI cycles. Patients agreed to have an ultrasound assessment of the uterine cavity with the use of saline as the contrast medium. SIS procedure was scheduled postmenstrual period in the early-mid follicular phase and 1-3 months before starting IVF/ICSI treatment. RESULTS: From 551 patients, we found 527 (94.4%) cases with the normal uterine cavity. The uterine cavity abnormalities were detected in 5.56% of cases included in this study (28 (5.02%) cases with endometrial polyps, two (0.36%) cases with intrauterine adhesions, and one (0.18%) case with Müllerian duct anomalies). CONCLUSION: SIS before IVF treatment could be a good option for evaluating uterus cavity before IVF to improve success rates of pregnancy.

This prospective study investigated 79 pre- and 25 post-menopausal women with abnormal uterine bleeding who underwent conventional transvaginal ultrasonography (TVS) and saline infusion sonohysterography (SIS) and compared the results with histopathological findings obtained by dilatation and curettage, hysteroscopy or hysterectomy. Histological examination revealed normal endometrial histology in 28 patients, intracavitary polyps in 46 patients, submucosal fibroids in 18 patients, intramural fibroids in six patients and endometrial hyperplasia in six patients. The sensitivity and specificity of TVS in detecting endometrial polyps were 65.2% and 87.9%, respectively, compared with 91.3% and 93.1% for SIS. The sensitivity and specificity of TVS in detecting uterine fibroids were 95.8% and 95.0%, respectively, versus 91.6% and 98.7% for SIS. These results show that SIS is a satisfactory method of identifying lesions and that it is easy and cost-effective, and improves on the diagnostic utility of TVS. SIS is also a less invasive alternative to hysteroscopy, so should result in less morbidity in the evaluation of abnormal uterine bleeding in women.

<bold>Introduction:</bold> The present study was conducted to evaluate the validity of Transvaginal Sonography (TVS) in perimenopausal women with abnormal uterine bleeding (AUB), validity of saline infusion sonohysterography in perimenopausal women with abnormal uterine bleeding and to compare the diagnostic accuracy of the above mentioned modalities. <bold>Material and Methods:</bold> It was a prospective comparative study of TVS and Saline Infusion Sonohysterography (SIS) in 100 perimenopausal women presenting with AUB. The findings of TVS and SIS was compared with histopathology. <bold>Result:</bold> The sensitivity of TVS for submucosal fibroid, polyp, intramural fibroid and endometrial hyperplasia was found to be 50%,44.44%, 40%, 33.33% respectively while specificity being 88.29%, 95.60%, 88.88%, 95.60% respectively. The sensitivity of SIS for endometrial hyperplasia and submucosal fibroid was found to be same (66.66%) whereas for polyp and intramural fibroid was 55.55% and 50% respectively. Specificity of SIS for polyp, endometrial hyperplasia, intramural fibroid and submucosal fibroid was found to be 98.90%,95.60%,92.2% and 90.42% respectively. Overall sensitivity and specificity of TVS was 79.4% and 75.8 % respectively whereas for SIS is 82.4% and 80.3% respectively. <bold>Conclusion:</bold> SIS is simple, highly sensitive and specific technique to detect intrauterine pathology.

Study carried out in Radiology Department of Jinnah Hospital Lahore on four patients with age ranges from 3546 years revealed that the Saline Infusion Sonohysterography plays an important role in diagnosing endometrial abnormalities especially Endometrial Polyps in women`s with complaint of intermenstural bleeding. Saline Infusion Sonohysterography requires minimal patient`s preparation, has few complications & is well tolerated by patients. Given its advantages over other techniques for uterine evaluation, Saline Infusion Sonohysterography will play an even larger role in pelvic imaging in future.
