BACKGROUND: There are many risk factors that result in musculoskeletal disorders because of work. This also occurs in the instant food industry, where apart from manual load handling and repetitive work, the production process can also results in risks. AIM: The aims of this study are to predict the musculoskeletal disorders in the next 50 years and the effectiveness of scenarios for controlling musculoskeletal disorders. METHODS: This study employed Research and Development method through a dynamics system approach. This research was conducted in one of the industries that produce instant food in South Sulawesi, Indonesia. The data obtained was based on interviews, which were further analyzed using Interpretative Structural Modeling. RESULTS: Based on the simulation results for 30 years, it was found that there was an increase in the average musculoskeletal disorder incidence by 20.63% per year. At the end of the simulation in 2050, the number of musculoskeletal disorder incidents became 48481.69. In this case, the simulation for 30 years (2020–2050) was conducted on a model of controlling occupational diseases at an instant noodle company in Makassar by providing treatment in the form of reducing risk factors that cause musculoskeletal disorders. Based on the simulation results for 30 years (2020–2050), musculoskeletal disorders have the most significant contribution to the increase of occupational diseases incidents as a whole. The increase in musculoskeletal disorders is an accumulation of several risk factors that exist in the instant noodle production process. CONCLUSIONS: Prediction of the musculoskeletal disorder incidence using a dynamic system approach for 30 years (2020–2050) has increased by an average of 20.63% per year. The behavior of the model after receiving treatment on the occurrence of musculoskeletal disorders has an average decrease in the average incidence of 51.11% per year. In this case, to control the musculoskeletal disorder incidence, the elements or variables controlled simultaneously are work posture, lifting load, and length of work.

BACKGROUND: Interleukin-6 (IL-6) and inducible Nitric oxide Synthase (iNOS) have an effect on neuropathic pain in the inflammatory process in peripheral nerve injuries. AIM: This study aims to examine the effect of anti-IL-6 receptor antibody on IL-6 and iNOS levels as a consideration for the treatment of neuropathic pain in a rat model of peripheral nerve injury. METHODS: Twenty-eight young adult male Wistar rats were treated for peripheral nerve injury and then divided into two groups. Fourteen treatment groups (Group P) were given anti-IL-6 receptor antibody by injection at a dose of 100 g/day by injection into the saphenous vein in the rat’s leg for 3 days. In both groups, the serum IL-6 and iNOS levels were assessed on the 3rd day after administration of anti-IL-6 receptor antibody in group P, using the sandwich ELISA method. RESULTS: The results showed that the administration of anti-IL-6 receptor antibody did not have a significant effect on reducing IL-6 and iNOS levels in group P (p > 0.05). Administration of anti-IL-6 receptor antibody had more effect on IL-6 levels on iNOS levels, where a decrease in IL-6 levels caused a decrease in iNOS levels in group P (p = 0.004 and r = 0.693). CONCLUSIONS: We conclude that the present administration of anti-IL-6 receptor antibody cannot be considered as a treatment for neuropathic pain in peripheral nerve injuries, but can be used to influence IL-6 levels on iNOS levels.

BACKGROUND: Occupational burnout in the most general terms is seen as a long-lasting stressful situation resulting from continuing occupational stress of moderate intensity. AIM: The objective of this article is to provide analysis of occupational burnout in psychiatrists and nurses of psychiatric hospitals, and the development of occupational burnout in students and residents. MATERIALS AND METHODS: The level of occupational burnout was determined using the “Attitude to work and occupational burnout” test, Seashore Group Cohesion Index, Stolin’s Self-Concept Questionnaire, and the Freiburg Multidimensional Personality Questionnaire (Freiburg Personality Inventory). The study proved the existence of patterns of occupational burnout in psychiatrists and nurses of psychiatric hospitals, as well as in students and residents. RESULTS: It has been revealed that the development of the burnout syndrome is influenced by personal qualities of psychiatrists and nurses (residents and students), and the managerial features of their activities, and that occupational burnout develops in psychiatrists and nurses over the course of their professional careers, but starts to emerge during their education. The score of the Integrative Burnout Index ranges from 48.99 in students of the [BLINDED] University to 23.52 in psychiatrists of the Alekseev Psychiatric Hospital N1. Occupational burnout syndrome is spread in medical students worldwide, with its level higher than in the overall population, in students of other specialties. CONCLUSIONS: Burnout at the stage of getting the education can hinder the professional development of students, expose patients to risk, and promote the development of various personality problems in future psychiatrists and nurses. The need of preventing professional burnout at the stage of obtaining a specialty has been substantiated.

BACKGROUND: In intracytoplasmic sperm injection (ICSI) infertility treatment cycles, measuring serum Progesterone level at day 9 after oocyte retrieval could be used as a predictor of success. METHODS: Sixty-nine women were prospectively included in this study, treated with fresh embryo transfer ICSI cycles. Progesterone analyses were performed on the day of oocyte pick up (day 0) at serum and follicular fluid, then re-assessment at serum on day 9 after oocyte retrieval. The data were compared to evaluate the correlation among hormones measured on day 0 and day 9 with pregnancy rate. RESULTS: Pregnancy rate of Iraqi women was 22% (n = 15), mean serum progesterone on day 9 among pregnant ladies was (mean ± SD, 39.5 ± 13.0) which was significantly higher than that who failed to get pregnant (mean 23.2 ± 11, p = 0.001), then after adjustment of the baseline readings (day 0) estimated mean = 11.1. The differences were still significant, receiver operating characteristic curve area showed that serum Progesterone on day 9 after pick up can differentiate correctly between those who will conceive and those who will not, with a sensitivity = 0.933 and specificity = 0.519, at cut off point = 22.33 and above, (Area under the curve = 0.822, p = 0.001). CONCLUSION: Serum progesterone on day 9 could be one of the predictors of endometrial receptivity and pregnancy, which is actually of great value for both doctors and patients during that stressful period till the date of confirmation, which might negatively affect treatment outcome, as well as the psychosocial and pharmacological impact of medication and limited activity for a failed one.

Abstract BACKGROUND: Chronic renal failure (CRF) is a reduction in chronic renal function that leads to non-reversible and progressive kidney tissue damage. Blood pressure is a major risk factor that can increase the mortality rate by up to 20 times in patients with chronic renal failure who are undergoing hemodialysis therapy. Intradialytic exercise is an intervention that can be used to control systolic blood pressure. AIM: This study was to analyze the effect of intradialytic exercise on changes in blood pressure in chronic renal failure patients undergoing hemodialysis therapy at Indriati Solo Baru Hospital. METHODS: The research method is quasi-experimental design pre-post with control group design. The study population was 97 dialysis patients. The sampling technique used a purposive sampling of 30 people. Data analysis using Paired t-Test. RESULTS: The research results showed  that the sex characteristics of most respondents were male as many as 17 people (56.7%), the average age of the respondents was 51.03 years, the pre-intervention in the control group obtained an average systolic blood pressure of 168.93 mmHg and 106 diastole, 120 mmHg, while the blood pressure in the control group post was 173.13 mmHg systole and 107 mmHg diastoles. In the treatment group, the average blood pressure in the pre-intradialytic exercise showed 162.20 mmHg systole and 104.27 mmHg diastole, while the post-intradialytic exercise system blood pressure was 153.13 mmHg and diastole 94.33 mmHg. CONCLUSION: There is an effect of intradialytic exercise on changes in blood pressure in chronic renal failure patients undergoing hemodialysis therapy at Indriati Solo Baru Hospital with a p-value of 0.025. Intradialytic exercise can be recommended as structured physical exercise therapy for hemodialysis therapy patients.   KEYWORDS: intradialytic exercise, chronic renal failure, patients, changes in blood pressure, hemodialysis

BACKGROUND:  The article examines the history and statistics of the pandemic spread. AIM: The study aimed to  develop a mathematical model reflecting the time dependence of the parameters characterizing the spread of the pandemic. MATERIALS AND METHODS: Differential equations were used to study the spread of the pandemic. RESULTS:  The case, where the coefficients of morbidity and recovery are different is considered. The patterns of change in the number of people susceptible to the disease and the number of infectious patients are revealed as a function of time. Using the developed model, the peak of the pandemic is found, i.e., the time at which the number of infectious patients will be the maximum.

BACKGROUND: Domestic wastewater can cause health problems and pollute groundwater sources. Such pollution not only has a negative impact on health and the environment, but also on the cost in providing clean water. AIM: The outcome of domestic wastewater treatment through a proper technique is expected to meet the clean water quality standard for sanitation purposes. MATERIALS AND METHODS: The experiment was conducted to determine the effect of Hydraulic Retention Time (HRT) on the levels of Biochemical Oxygen Demand (BOD) and Total Suspended Solid (TSS) of domestic wastewater. The experiment was carried out with 6 variations of HRT, namely 1 hour, 2 hours, 4 hours, 6 hours and 8 hours with 4 repetitions. The media running process was carried out for 14 days until the reactor condition was in steady state. RESULTS: The results showed that the removal values ​​for COD, Oil and Fat, Ammonia and Total Coliform parameters were 68.03%, 46.51%, 69.64% and 68.99%, respectively. Based on the variation of HRT of 1 hour, 2 hours, 4 hours, 6 hours and 8 hours on the BOD parameter, the removal values ​​obtained were 11.7%, 21.3%, 34.7%, 49.0% and 64.1%, respectively. Furthermore, for the TSS parameter, the values obtained were 17.3%, 25.4%, 30.6%, 42.3% and 50.4%, respectively. CONCLUSION: HRT was proven to have a significant effect on the levels of BOD and TSS of domestic wastewater with a p-value of <0.05 at the 95% confidence level

BACKGROUND: Mesenchymal stem cells (MSCs) have potent immunosuppressive properties to control systemic lupus erythematosus (SLE) disease by releasing several anti-inflammatory molecules, particularly indoleamine 2, 3-dioxygenase (IDO), and increasing regulatory T cells (Treg) to control innate and adaptive immune cells. However, how MSCs release IDO to modulate Treg in controlling B is poorly understood. Therefore, investigating IDO, Treg, and B cells following MSC administration in SLE is needed. AIM: This study aimed to investigate the ameliorating effects of MSCs in controlling B cells mediated by an increase of IDO-induced Treg in PBMC of SLE patients. METHODS: This study used a post-test control group design. MSCs were obtained from human umbilical cord blood and characterized according to their surface antigen expression and multilineage differentiation capacities. PBMCs isolated from SLE patients were divided into five groups: Sham (placebo group), control, and three treatment groups. The treatment groups were treated by coculturing MSCs to PBMCs with a ratio of 1:10, 1:25, and 1:40 for 72 h incubation. Treg and B-cell levels were analyzed by flow cytometry with cytometric bead array (CBA) while the IDO levels were determined by ELISA. RESULTS: This study showed that the percentages of B cells decreased significantly in groups treated by dose-dependent MSCs, particularly in T1 and T2 groups followed by increased Treg cell percentages. These findings were aligned with the significant increase of the IDO levels. CONCLUSIONS: MSCs regulated B cells through an increase of IDO-induced Treg in SLE patients’ PBMC.

BACKGROUND: The article presents the dynamics of biochemical indicators showing the tension of body functional systems in qualified Greco-Roman wrestlers at the pre-competition stage. Biochemical indicators can be regarded as the markers of training level, as a reflection of adaptive changes during performing training loads. AIM: The study aims to examine the adaptive reactions of body internal systems in wrestlers to the load performed at the stage of pre-competition training. METHODS: The methodological basis of the study is the examination of the reaction of body functional systems in wrestlers (n = 24) in response to the load performed at the stage of pre-competition training. The basis of the studied indicators of wrestlers’ organisms is the dynamics of the enzymatic activity (ALT and AST), the activity of creatine phosphokinase, and the balance of anabolic and catabolic processes in the course of a 2-week macrocycle of the pre-competition training. RESULTS: A high level of enzymatic activity (ALT and AST) was noted as the response to shock training load in the first and the second training macrocycles against the background of a negative trend during the entire sports event, which indicates a directed decrease in the heart’s tension muscle, being an indicator of adaptive changes occurring in wrestlers’ body energy. The high variability of AST indicators on the first day and creatine phosphokinase throughout the entire pre-competition training pointed out an individual level of adaptive reactions of the athletes’ bodies in response to the training load taken. CONCLUSIONS: The results of the study have shown notable dynamics in the indicators of the enzymatic activity of AST, creatine phosphokinase, and the hormone cortisol in a series of shock training loads, as the response to adaptive changes in body energy systems, the value of which should be considered during the pre-competition training.

BACKGROUND: Numerous attempts were made to reduce the adverse effects of the distal extension removable partial dentures (RPDs) and enhance their prognosis. High-performance polymers (HPP) were utilized in the construction of RPDs to maintain the health of the supporting structures. AIM: Thus, this study was prompted to compare the strains induced by Bio HPP and Cobalt- Chromium (Co Cr) Computer Assisted Design and Computer Assisted Manufacturing telescopic RPDs after 1 year of function. MATERIALS AND METHODS: A maxillary Kennedy class I was used in this study. Twelve telescopic RPDs were fabricated from two different materials. In Group A, six telescopic RPDs were milled from Co-Cr and in Group B, six telescopic retained RPDs were milled from Bio-HPP. Each partial denture was seated on the cast and introduced into the chewing simulator. The strain values were recorded using four strain gauges connected to a four-channel strain indicator. Statistical analysis of the resultant data was done using one-way ANOVA, followed by Tukey’s HSD for comparison within the same group. Student t-test was used for comparison between the different groups. The significance level was set at p ≤ 0.05. RESULTS: During unilateral loading, the results showed higher strains in Group A at the abutments (473.33 μm/m ± 10.8, 193.39 μm/m ± 10.8) and at the distal aspect of the ridge (470.83 μm/m ± 13.93, 185 μm/m ± 20.83) than Group B. Independent t-test showed statistically significant difference between strains at the abutments of both groups (t = 70.4, p ≤ 0.0001), (t = 36.84, p ≤ 0.0001). Furthermore, there was a statistically significant difference between strains at the saddles of both groups (t = 51.62, p ≤ 0.0001), (t = 34.72, p ≤ 0.0001) respectively (DOF = 10). CONCLUSIONS: In telescopic RPDs, Co Cr induces higher strain values on the abutments and the distal aspect of the ridge than Bio-HPP during bilateral and unilateral loading. During unilateral loading, Bio-HPP telescopic RPDs direct high strain values on the distal aspect of the ridge of the loaded side. CLINICAL IMPLICATIONS: The materials that induce less stresses on the supporting structures of telescopic partial dentures on the long-term can be used to maintain the health of periodontally affected abutments.
