Zielsetzung: Die Einführung von Fallpauschalen impliziert eine Verkürzung der Verweildauern und kann daher sowohl Auswirkungen auf die Leistungsqualität der stationären Behandlung haben als auch zu einer Verlagerung stationärer Leistungen in den ambulanten Bereich führen. Diese Untersuchung beschäftigt sich daher mit den Auswirkungen des Entgeltsystems «Fallpauschalen» auf die akutstationäre Versorgung von Patienten anhand der ausgewählten Fallgruppen «Varizenexhairese» und «Hernienoperation». Methodik: Es wurde eine retrospektive Vergleichsuntersuchung mit zwei Erhebungsteilen durchgeführt. Neben einer Auswertung von Patientendaten sechs nordrhein-westfälischer Krankenhäuser (N = 875) erfolgte eine Patientenbefragung (N = 510). Ergebnisse: Für die untersuchten Fallgruppen wurden die Liegezeiten von 1995 bis 1997 um zwei Tage (Varizenexhairese) bzw. einen Tag (Hernienoperation) reduziert, liegen damit aber noch über der von der Expertenkommission des BMG vorgegebenen «Katalogverweildauer». Die Verkürzung der Liegezeiten führt zu einem Anstieg des Anteils ambulant behandelter Patienten. Die Patienten nehmen ihren Gesundheitszustand zum Zeitpunkt der Entlassung aus der Akutklinik subjektiv als schlechter wahr. Dies wirkt sich jedoch in der ambulanten Phase nicht aus. Schlußfolgerung: Die durch die Verkürzung der Verweildauer bedingten Leistungseinschränkungen stationärer Versorgung können für die untersuchten Fallgruppen bisher durch die ambulante Nachsorge kompensiert werden. Bei einer weiteren Verkürzung der Verweildauern sind jedoch negative Auswirkungen auf den Gesundheitszustand der Betroffenen zu erwarten.

After a thourough development phase, a new system of health financing was introduced in Hungary in 1993. One of the cornerstones of the system was the financing of acute hospital care through Diagnosis-Related Groups (DRGs). This method was part of a comprehensive healthcare model, elaborated and published around 1990 by experts of Gyógyinfok, a public institute. The health financing system that was finally introduced reflcted in large part this theoretical model, while the current Hungarian system differs from it in some important respects. This study aims to develop a performance evaluation system that can facilitate performance evaluation at region, hospital, and department levels to enable better cost management for sustainable development. A multi-level system of performance evaluation informs a hierarchical assessment of cost management from regions to hospitals to departments using diagnosis-related group (DRGs). Various metrics are developed employing the variances between targets and actuals where targets are determined from two perspectives: benchmarking using external regional prices and change management using internal data. Targets for the latter are statistically based and specifically incorporate variability. The model is applied to two hospitals, twenty departments, nine DRGs and 1071 inpatients. The analyses indicate that the approach can provide a practical evaluation tool that allows for particular characteristics at multiple levels. The system provides macro-micro and external-internal perspectives in performance, enabling high-level variances to be decomposed thereby identifying sources of performance variability and financial impact.
