Abstract Community care is increasingly the mainstay of mental healthcare provision in many countries and patient satisfaction is an important barometer of quality of patient care. This paper explores the key factors associated with patient satisfaction with community mental health services in England and then compares providers’ performance on patient satisfaction. Our analysis is based on patient-level responses from the community mental health survey, which is run annually by the Care Quality Commission (CQC) for the years 2010 to 2013. We perform a repeated cross-section analysis, identifying factors associated with patient satisfaction via a multi-level ordered probit model, including both patient- and provider-level variables. We identify hospital-specific effects via empirical Bayes estimation. Our analysis identifies a number of novel results. First, patient characteristics such as older age, being employed, and being able to work, are associated with higher satisfaction, while being female is associated with lower satisfaction. Service contact length, time since last visit, condition severity and admission to a mental health institution, are all associated with lower satisfaction. Second, treatment type affects satisfaction, with patients receiving talking therapies or being prescribed medications being more satisfied. Third, care continuity and involvement, as proxied by having a care plan, is associated with higher satisfaction. Fourth, seeing a health professional closer to the community improves satisfaction, with patients seeing a community-psychiatric nurse, a social worker or a mental-health support worker being more satisfied. Finally, our study identifies the need for service integration, with patients experiencing financial, accommodation, or physical health needs being less satisfied. At a provider level, we find a negative association between the percentage of occupied beds and satisfaction. We further identify significant provider-specific effects after accounting for observable differences in patient and provider characteristics which suggests significant differences in provider quality of care.

Objectives: The aim of this study was to investigate whether people born in non-English-speaking countries differed from clients born in Australia on quality of medication management, measured by mean neuroleptic dose, method of administration, use of atypical antipsychotics and perceived compliance with medication, and to investigate the influence of matching the client with a case manager from the same ethnic background on these measures. Method: Information about medication and perceived compliance was provided for 168 clients of five community mental health services in Melbourne. Chlorpromazine equivalent doses (CPZe) were calculated, and average dose, route of administration, percentage receiving atypical antipsychotics and perceived compliance with medication were analysed by country of birth and preferred language. Each analysis was repeated for clients matched to a case manager from the same ethno-linguistic background compared to those with ‘unmatched’ case managers. Results: While non-English-speaking background (NESB) clients generally did not receive different dose sizes, those born in Vietnam had a lower mean dose. People born in a non-English-speaking country tended to be more likely than the Australian born to receive a depot injection, although this was not quite statistically significant. Twenty-seven percent of clients received an atypical neuroleptic; age was a significant factor, with older clients less likely to receive an atypical. There was no difference in receipt of atypicals or perceived compliance by country of birth, language or gender. Matching for a case manager of the same background had no effect except for route of administration, with matched clients less likely to receive depot medication than unmatched. Conclusions: Generally, the ethnic background of clients had little influence on the quality of medication management they received from community mental health services.

Introduction The need to improve the quality of community mental health services for people with Complex Emotional Needs (CEN) is recognised internationally and has become a renewed policy priority in England. Such improvement requires positive engagement from clinicians across the service system, and their perspectives on achieving good practice need to be understood. Aim To synthesise qualitative evidence on clinician perspectives on what constitutes good practice, and what helps or prevents it being achieved, in community mental health services for people with CEN. Methods Six bibliographic databases were searched for studies published since 2003 and supplementary citation tracking was conducted. Studies that used any recognised qualitative method and reported clinician experiences and perspectives on community-based mental health services for adults with CEN were eligible for this review, including generic and specialist settings. Meta-synthesis was used to generate and synthesise over-arching themes across included studies. Results Twenty-nine papers were eligible for inclusion. Six over-arching themes were identified: 1. The use and misuse of diagnosis; 2. The patient journey into services: nowhere to go; 3. Therapeutic relationships: connection and distance; 4. The nature of treatment: not doing too much or too little; 5. Managing safety issues and crises: being measured and proactive; 6. Clinician and wider service needs: whose needs are they anyway? The overall quality of the evidence was moderate. Discussion Through summarising the literature on clinician perspectives on good practice for people with CEN, over-arching priorities were identified on which there appears to be substantial consensus. In their focus on needs such as for a long-term perspective on treatment journeys, high quality and consistent therapeutic relationships, and a balanced approach to safety, clinician priorities are mainly congruent with those found in studies on service user views. They also identify clinician needs that should be met for good care to be provided, including for supervision, joint working and organisational support.

RIASSUNTOScopo — L'obiettivo di questo studio è quello di descrivere i patterns di utilizzazione dei pazienti in contatto con 5 Unità Operative di Psichiatria lombarde secondo quattro classi: alti utilizzatori lungoassistiti, alti utilizzatori non lungoassistiti, lungoassistiti non alti utilizzatori, non alti utilizzatori non lungoassistiti. Disegno — Studio descrittivo a partire dai dati ricavati dal Sistema Informativo Psichiatrico regionale; è stata analizzata una coorte di 5.670 pazienti nell'ambito della prevalenza annua relativa all'anno 1994. Setting — 5 Unità Operative di Psichiatria della Regione Lombardia (Merate, Treviglio, Crema, Desio, Castano Primo), con una popolazione complessiva di 610.184 residenti di eta superiore ai 14 anni. Principali misure utilizzate — Sono state prese in considerazione alcune variabili sociodemografiche e cliniche relative ai pazienti; oltre un'analisi descrittiva dei quattro patterns, è stata effettuata una analisi logistica multinomiale. Risultari — Gli alti utilizzatori lungoassistiti (AU-LA), pur rappresentando solo il 5.3% del campione (4.9 casi per 10.000 residenti di età superiore ai 14 anni), consumano il 60% delle risorse espresse in SCS; solo la condizione di separato, divorziato, vedovo è predittiva per tale pattern. Gli alti utilizzatori non lungoassistiti (AU-non LA) costituiscono 1.2% del campione (1.1 casi per 10.000) ed utilizzano il 7.8% del SCS. Variabili predittrici di tale pattern sono l'eta compresa tra i 15-44 anni, l'assenza di un'attivita lavorativa e di un partner, la diagnosi di un disturbo mentale grave e la presenza di contatti con i servizi psichiatrici negli anni 1985-1989. I lungoassistiti non alti utilizzatori (LA-non AU) rappresentano il 23.4% della coorte (21.6 casi per 10.000) e vengono al secondo posto per consumo dirisorse (18.1% del SCS). Sono variabili predittive: l'età compresa tra i 15-44 anni, il vivere da solo, l'assenza di un'attività lavorativa e di un partner, la diagnosi di un disturbo mentale grave e la presenza di contatti con i servizi psichiatrici antecedenti al 1990. I pazienti non lungoassistiti non alti utilizzatori (non LA-non AU), pur rappresentando il 70.1% della coorte (64.8 casi per 10.000), consumano solo il 13.8% del SCS. Conclusioni — I dati mostrano che complessivamente l'attivita delle UOP è orientata nei confronti dei pazienti piu gravi, anche se sono rilevabili marcate differenze tra le UOP lombarde rispetto all'utilizzazione dei servizi. È confermata l'utilita di un Sistema Informativo a diffusione regionale che permetta di monitorare l'evoluzione nel tempo e nel territorio regionale dei patterns di utilizzazione.
