Purpose:To analyze the visual and refractive outcome of the bioptics procedure combining multifocal intraocular lens implantation and excimer laser surgery in young patients with high hyperopic eyes not suitable for a single surgical procedure.Methods:This retrospective case series included 10 eyes of five patients (age range 18–30 years) with high hyperopia (spherical equivalent +8.51 ± 0.85 diopters (D)). They had been treated with serial multifocal intraocular lens implantation followed 6 weeks later by laser in situ keratomileusis for residual hyperopia. Uncorrected distance visual acuity, uncorrected near visual acuity, corrected distance visual acuity, corrected near visual acuity, and manifest refraction were evaluated before surgeries, after multifocal intraocular lens implantation, and 3 months post laser in situ keratomileusis.Results:No patients were lost to follow-up (6 months). The mean spherical equivalent decreased to +2.05 ± 1.33 D after multifocal intraocular lens implantation and to −0.10 ± 0.58 D after the laser in situ keratomileusis procedure. Success of the procedures was determined by uncorrected visual acuity. LogMAR uncorrected distance visual acuity improved by a total of more than six lines from 1.05 ± 0.18 LogMAR to 0.46 ± 0.12 LogMAR post multifocal intraocular lens implantation and to 0.15 ± 0.06 LogMAR after both surgeries. The LogMAR uncorrected near visual acuity increased by 0.81 ± 0.82 LogMAR after lens implantation due to loss of accommodation, and all eyes reached a LogMAR of 0 at 1 month postoperatively following laser in situ keratomileusis.Conclusions:A bioptics approach involving multifocal intraocular lens followed 6 weeks later by a laser in situ keratomileusis procedure for the correction of very high hyperopia enabled the resolution of the residual refractive error in young very high hyperopic patients.

Purpose To evaluate the effect of astigmatism axis on uncorrected distance visual acuity (UDVA) in emmetropic eyes that underwent laser refractive surgery. Methods This retrospective study included patients who underwent laser in situ keratomileusis or photorefractive keratectomy between January 2000 and December 2015 at the Care-Vision Laser Centers, Tel Aviv, Israel. Eyes with a 3-month postoperative spherical equivalent between -0.5 D and 0.5 D were included in this study. Eyes with ocular comorbidities and planned ametropia were excluded. Study eyes were divided into 3 groups according to the steep astigmatic axis: with the rule (WTR) (60-120), oblique (31-59 or 121-149), and against the rule (ATR) (0-30 or 150-180). The UDVA of these 3 groups was compared. The oblique group was divided into oblique ATR and oblique WTR, which were compared with each other. Results A total of 17,416 consecutive eyes of 8,708 patients were studied. The WTR eyes (n = 10,651) had significantly better UDVA (logMAR 0.01 ± 0.08) than the oblique (n = 3,141, logMAR 0.02 ± 0.09) and ATR eyes (n = 3,624, logMAR 0.02 ± 0.10) (p<0.001). The oblique WTR group had significantly better UDVA than the oblique ATR group (p<0.001). The UDVA of the oblique and ATR groups was similar. Stepwise multiple regression analysis showed that the group accounted for 15% of the UDVA variance (p = 0.04). Conclusions The astigmatic axis has a small but significant effect on UDVA in emmetropic eyes; WTR was better than oblique and ATR astigmatism. Therefore, when correcting astigmatism, it may be preferable to err towards WTR astigmatism.

Background: Many studies have used functional optical zone (FOZ) as a measure to compare different refractive laser treatment modalities. However, to our knowledge, no study has compared wavefrontoptimized (WFO) and wavefront-guided (WFG) laser in situ keratomileusis (LASIK) using FOZ. We compared the FOZ after WFO versus WFG LASIK in patients with myopia and myopic astigmatism.Methods: In this prospective comparative study, we included 100 myopic eyes of 50 patients with or without astigmatism. They were divided into two groups according to the platform used: WFO or WFG femtosecond LASIK. Using Holladay’s equivalent keratometry reading (EKR) report of Pentacam HR, FOZ was defined as a zone centered on the pupil center with a standard deviation (SD) of 0.5 D, around the mean EKR. The differences in FOZ between the two platforms were analyzed at 3 months postoperatively. Visual acuity, refractive error, corneal asphericity (Q-value), and root mean square of higher-order aberrations (RMS for HOAs) were evaluated and compared.Results: The mean ± SD of patient age was 26.64 ± 5.67 years. The preoperative characteristics of the two groups were comparable (all P > 0.05). The intended optical zone (IOZ) was 6 mm in both groups. The mean laser ablation depth was significantly greater in the WFG group (18 ?m per D) than in the WFO group (16 ?m per D) (P = 0.035). At 3 months postoperatively, the mean ± SD of FOZ diameter was 4.32 ± 0.94 mm (71.99 ± 15.68% of intended optical zone) in the WFO group and 4.16 ± 1.13 mm (69.33 ± 18.78% of intended optical zone) in the WFG group, with no significant difference between the two groups (P = 0.622). The change in corneal asphericity was greater in the WFG group than in the WFO group (P = 0.034). Postoperative mean corrected and uncorrected distance visual acuity, manifest refraction, and RMS for HOAs showed no significant difference between the two groups (all P > 0.05).Conclusions: We found that WFG LASIK resulted in greater ablation depth and change in corneal asphericity than WFO LASIK at 3 months postoperatively. However, there was no significant difference in FOZ diameter, refractive error, and RMS for HOAs between the two groups. Further research is needed to confirm these findings.

Purpose. To evaluate the long-term efficacy, accuracy, stability, and safety of hyperopic laser in situ keratomileusis (LASIK) using a 213 nm wavelength solid-state laser. Methods. This prospective noncomparative case series consisted of 34 eyes of 17 patients which underwent hyperopic LASIK using a 213 nm solid-state laser (Pulzar Z1, CustomVis) at an outpatient refractive surgery center in Manila, Philippines. The preoperative and postoperative examinations included uncorrected distance visual acuity (UDVA), subjective manifest refraction, corrected distance visual acuity (CDVA), cycloplegic refraction, slitlamp biomicroscopy, and keratometry (). Main Outcome Measures. Accuracy, efficacy, stability, and safety of the refractive procedure. Results. Mean follow-up was months. At the end of follow-up, 26.47% had a UDVA of 20/20 and 94.12% had a UDVA of ≥20/40. Manifest refractive spherical equivalent (MRSE) was within ±0.50 D of the target refraction in 55.88% and within ±1.0 D in 85.30% of the study eyes. Refractive stability was noted in the 1st postoperative month while hyperopic regression was noted after the 3rd postoperative year. No eye lost more than 2 lines of CDVA. Conclusion. Our results show that the 213 nm solid state laser system is safe, effective, accurate, and predictable for the treatment of hyperopia.

Seven years after uneventful laser in situ keratomileusis (LASIK), a 48-year-old woman presented one week after being hit with an iron cord with blurry vision, pain, and irritation. The injury resulted in traumatic flap dislocation, epithelial ingrowth, and macrostriae. Following epithelial removal, the flap was refloated and repositioned. Nine interrupted sutures were used to secure the flap. Three-weeks after surgery with no sutures remaining, the epithelial ingrowth and macrostriae had resolved with a visual acuity of 20/20.
