A term male newborn infant was apneic at birth, and endotracheal intubation was unsuccessful. He was stabilized for transport with a laryngeal mask airway. Laryngoscopy revealed tracheal atresia with intrathoracic distal tracheoesophageal fistula. A laryngeal mask airway may provide ventilation in tracheal atresia patients with a fistula.

Positive pressure ventilation with Ambu bag and face masks or T-piece resuscitation in preterm babies have proved their worth in acute neonatal airway management. However, failure rates are high and endotracheal intubation is the final life-saving procedure. However, intubation is an invasive procedure and requires good expertise, especially for handling acute emergency situations. Expertise of trainees as well as consultants is coming down rapidly due to various reasons. In these situations, supraglottic airways (laryngeal mask airway or LMA) seem to be ideal. Here, we are reviewing the application and utility of LMA in acute neonatal airway management.

LMA and Intubating LMA (LMA-Fastrach) have been widely used by anesthesiologists in operation theaters and have achieved great success. Its use in the emergency department and pre-hospital setting by EMS has recently been proven to be very successful. It is also a very useful tool in providing a quick airway in case of failed intubation and failed ventilation situations. In this article, some of its use in emergency departments and pre-hospital setting are discussed.
