AbstractA case of basal cell adenocarcinoma of the parotid gland is reported. Histologically it is to be distinguished from basal cell adenoma and adenoid cystic carcinoma. Clinically the presentation may appear benign. Surgical excision is the treatment of choice. The prognosis is uncertain because of lack of follow-up data.

The term basal cell adenoma was used by Kleinsasser and Klien 1 in 1967 to describe benign salivary gland tumors comprised of a single type of epithelium. Basal cell adenocarcinoma is a relatively new entity, and there are only a few cases reported in the literature. It has a morphology like that of a basal cell adenoma but has growth characteristics of a malignant neoplasm.

Diagnosis of mid-cheek mass has always been a diagnostic dilemma for clinicians. The incidence of the accessory parotid gland (APG) has been well documented in the literature. However, a diverse spectrum of pathologies can arise from this accessory parotid tissue. Basal cell adenoma is a rare benign tumor that predominantly affects the parotid gland. Its occurrence in the accessory parotid gland has been documented in 9 cases till date. Surgical approaches for the management of such APG lesions have been in debate. This report documents a case of Basal Cell Adenoma arising in accessory parotid tissue in a 55-year-old woman who was treated by transoral excision. This paper also reviews previously documented cases of all APG lesions and attempts to elucidate the rationale for selecting an appropriate surgical approach.

ABSTRACT Basal cell adenoma (BCA) is a significantly rare benign salivary gland neoplasm that includes isomorphic basaloid cells. Presence of myoepithelial cell is a characteristic of this tumor. Basal cell adenoma accounts for only 1 to 2% of all salivary gland epithelial tumors.
