Myocardial Infarction with Non-Obstructive Coronary Arteries (MINOCA) accounts for 5–15% of all presentations of acute myocardial infarction. The absence of obstructive coronary disease may present a diagnostic dilemma and identifying the underlying etiology ensures appropriate management improving clinical outcomes. Cardiac magnetic resonance (CMR) imaging is a valuable, non-invasive diagnostic tool that can aide clinicians to build a differential diagnosis in patients with MINOCA, as well as identifying non-ischemic etiologies of myocardial injury (acute myocarditis, Takotsubo Syndrome, and other conditions). The role of CMR in suspected MINOCA is increasingly recognized as emphasized in both European and American clinical guidelines. In this paper we review the indications for CMR, the clinical value in the differential diagnosis of patients with suspected MINOCA, as well as its current limitations and future perspectives.

Verruciform xanthoma is a specific but not so common, papillary growth typically occurring on the oral mucosa of middle-aged persons. It is a unique entity and has found to be incident in 0.025-0.05% of all cases reported as per the literature study. One such case of 75 days duration was successfully managed with complete local surgical excision. The surgical site was monitored for one year postoperatively with no signs of recurrence or other postoperative complications. Clinical perspective, histopathological highlights and differential interpretation is discussed in this article with an in-depth review of the literature. It is important to consider this rare entity in the differential diagnosis of lesions involving oral mucosa as its clinical and histological features overlap with several other lesions.

AbstractPleomorphic carcinoma is a malignant and aggressive primary lung carcinoma that occurs at a rate of approximately 0.3%. This rarely encountered tumor may present a diagnostic challenge to neuroradiologists and pathologists, as it has propensity to present as hemorrhagic brain metastasis with unknown primary and result in delay in diagnosis that could impact clinical outcome. Herein, we report a unique case in its presentation in a 56-year-old female, having symptomatic brain metastasis prior to the discovery of the pulmonary lesion.

Abstract Background Evaluation of suspected coronavirus disease-2019 (COVID-19) patient is a diagnostic dilemma as it commonly presents like influenza in early stages. Studies and guidelines have emerged both for and against the use of imaging as a frontline tool to investigate such patients. Reverse transcriptase-polymerase chain reaction (RT-PCR) is suggested as the backbone of diagnosis. We designed and tested a diagnostic algorithm using artificial intelligence (AI) to determine the role of imaging in the evaluation of patients with acute flu-like presentation. Materials and Methods Overall, 3,235 consecutive patients with flu-like presentation were evaluated over a period of 240 days. All patients underwent plain radiographs of chest with computer-aided detection for COVID-19 (CAD4COVID) AI analysis. Based on the threshold scores, they were divided into two groups: group A (score < 50) and group B (score > 50). Group A patients were discharged and put on routine symptomatic treatment and follow-up with RT-PCR, while group B patients underwent high-resolution computed tomography (HRCT) followed by COVID-19 AI analysis and RT-PCR test. These were then triaged into COVID-19 and non-COVID-19 subgroups based on COVID-19 similarity scores by AI, and lung severity scores were also determined. Results Group A had 2,209 (68.3%) patients with CAD4COVID score of <50 while 1,026 (31.7%) patients comprised group B. Also, 825 (25.5%) patients were COVID-19 positive with COVID-19 similarity threshold of >0.85 on AI. RT-PCR was positive in 415 and false-negative in 115 patients while 12 patients died before the test could be done. The sensitivity and specificity of CAD4COVID AI analysis on plain radiographs for detection of any lung abnormality combined with HRCT AI analysis was 97.9% and 99% using the above algorithm. Conclusion Combined use of chest radiographs and plain HRCT with AI-based analysis is useful and an accurate frontline tool to triage patients with acute flu-like symptoms in non-COVID-19 health care facilities.

Based on available literature, pregnant women are at an increased risk of severe illness from severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection, compared to nonpregnant women. Consequences of coronavirus disease 2019 (COVID-19) in pregnancy have many implications in women’s lives other than unfavorable obstetric outcomes. In addition to managing acute respiratory illness and symptoms, caregivers should be equipped to detect and manage the short-term, intermediate, and long-term consequences of SARS-CoV-2 infection as well. Many pregnant women can remain asymptomatic and continue their pregnancy without being diagnosed. Pregnancy outcomes and consequences of SARS-CoV-2 infected yet asymptomatic mothers have not been very well explained. Reports of a new multisystem inflammatory syndrome in children (MIS-C) and multisystem inflammatory syndrome in adults (MIS-A) following COVID-19 have been described. However, MIS-A in pregnancy is an extremely rare presentation that can cause a huge diagnostic dilemma to caregivers. We describe the successful management of a pregnant woman with MIS-A following SARS-CoV-2 infection.

Stump appendicitis is a rare delayed complication of incomplete appendectomy. The clinical symptoms and signs are like acute appendicitis. The possibility of stump appendicitis is not clear to all clinicians. It represents a diagnostic dilemma with incidence of one in 50,000 cases. We report how we encountered diagnostic dilemma during the management of a 30 years old male patient of acute abdomen with history of appendectomy done for acute appendicitis even years back.
