The Effects of Instruction Exercises on Performance and Kinetic Factors Associated With Lower-Extremity Injury in Landing After Volleyball Blocks

Context: Female volleyball players are more predisposed to anterior cruciate ligament injury in comparison with their male counterparts. Recent research on anterior cruciate ligament injury prevention strategies has shown the positive results of adopting the external focus (EF) of attention in sports. Objective: To determine the effect of 6-week EF instruction exercises on performance and kinetic factors associated with lower-extremity injury in landing after the volleyball blocks of female athletes. Design: Pretest and posttest control study. Setting: University research laboratory. Participants: Thirty-two female volleyball players (18–24 y old) from the same team randomly divided into experimental (n = 16) and control (n = 16) groups. Intervention: The experimental group performed a 6-week exercise program with EF instructions. The control group continued its regular volleyball team schedule. Main Outcome Measures: To assess function, single-leg triple hop test for distance was used. A force plate was used to evaluate kinetic variables including vertical ground reaction forces, the rate of loading, and dynamic postural stability index. All data were assessed at baseline and after the intervention. Results: There was a significant increase in single-leg triple hop test (P < .05) and in the first and second peak ground reaction force, rate of loadings, dynamic postural stability index (P < .05). Conclusion: According to the results of this study, anterior cruciate ligament injury prevention programs should incorporate EF instruction exercises to enhance the kinetics and to increase athletes’ functional performance.

Individuals returning to sport after anterior cruciate ligament reconstruction (ACLR) are at an increased risk of sustaining a subsequent ACL injury. It is suspected that increased reliance on visual information to maintain stability may factor into this increased risk. The connection between visual reliance and ACLR is not well understood during dynamic tasks. Examination of the proposed visual reliance may help improve returning to sport standards and reduce subsequent ACL injury risk. A total of 12 ACLR individuals and 12 age- and sex-matched controls completed several trials of a normalized dynamic hop task on both limbs under 3 different visual conditions (eyes open, low visual disruption, and high visual disruption). Stroboscopic eyewear were worn by each participant to disrupt vision during testing. Ground reaction force data were collected during landing. Dynamic postural stability was assessed using 2 separate calculations: dynamic postural stability index and time to stability. No significant statistical interactions or group differences were observed. The stroboscopic eyewear did increase the medial–lateral stability index from the eyes open to the low visual disruption condition (P < .05). These findings suggest that ACLR individuals do not rely on visual information more than controls during a dynamic hop task.

Escasos trabajos han valorado el riesgo de lesión del LCA en fútbol femenino mediante un enfoque multifactorial. El objetivo del presente estudio fue describir los déficits funcionales y las asimetrías manifestadas por jugadoras de fútbol en diferentes pruebas de valoración, con el fin de realizar un análisis integrador del riesgo de lesión desde diferentes perspectivas funcionales y mecánicas. En este estudio de caso participaron 17 jugadoras de fútbol femenino de categoría Primera Autonómica y Preferente Autonómica Femenina española, con rangos de edad de 14 a 23 años. Las pruebas realizadas fueron: OverHead Squat (OHS), Drop Vertical Jump Test (DVJ), Tuck Jump (TJ) y Triple Hop Test (TH). Se halló que todas las jugadoras presentaron mejores resultados para la pierna dominante, mostrando una media mayor de flexión lateral del tronco (TLFA) al aterrizar con la pierna izquierda (8,88±5,15 º) que al aterrizar con la derecha (7,05±5,40 º) en el DVJ unipodal. Además, se encontró que gran parte de las jugadoras (41,20%) presentaron riesgo medio de lesión en el OHS, así como un desplazamiento medio de las rodillas en sentido medial (valgo) en el aterrizaje bipodal del DVJ (-1,88 cm), mientras que la mayoría de ellas (65%) no mostraron asimetrías neuromusculares de miembro inferior. En conclusión, las jugadoras muestran principalmente déficits funcionales en la estabilidad central (CORE) y en la estabilidad de rodilla para resistir y transmitir fuerzas en acciones de alto riesgo para el LCA como los saltos-aterrizajes. Por tanto, estos déficits deberían ser abordados de forma integral con las futbolistas.

Background Little is known about the evolution of anterior cruciate ligament injury rates among elite alpine skiers. Purpose To evaluate epidemiologic aspects of anterior cruciate ligament injuries among competitive alpine skiers during the last 25 years. Study Design Descriptive epidemiology study. Methods Data were collected from elite French national teams (379 athletes: 188 women and 191 men) from 1980 to 2005. Results Fifty-three of the female skiers (28.2%) and 52 of the male skiers (27.2%) sustained at least 1 anterior cruciate ligament injury. The overall anterior cruciate ligament injury incidence was 8.5 per 100 skier-seasons. The primary anterior cruciate ligament injury rate was 5.7 per 100 skier-seasons. The prevalence of reinjury (same knee) was 19%. The prevalence of a bilateral injury (injury of the other knee) was 30.5%. At least 1 additional anterior cruciate ligament surgery (mean, 2.4 procedures) was required for 39% of the injured athletes. Men and women were similar with regard to primary anterior cruciate ligament injury rate (P = .21), career remaining after the injury (P = .44), and skiing specialty (P = .5). There were more anterior cruciate ligament injuries (primary, bilateral, reinjuries) among athletes ranking in the world Top 30 (P < .001). Anterior cruciate ligament-injured athletes had a career length of 7.5 years, whereas athletes with no anterior cruciate ligament injury had a career of 4.5 years (P < .001). Finally, injury rates remained constant over time. Conclusion Anterior cruciate ligament injury rates (primary injury, bilateral injury, reinjury) among national competitive alpine skiers are high and have not declined in the last 25 years. Finding a way to prevent anterior cruciate ligament injury in this population is a very important goal.
