We report a case of lithium-induced downbeat nystagmus and horizontal gaze palsy in a 62-year-old woman who was treated for a bipolar affective disorder with lithium carbonate for one month. At presentation serum lithium was within therapeutic range. No alternative causes of the ocular motility disturbances were found, and the patient improved significantly as lithium carbonate was discontinued.

This paper summarizes the major dermatologic and immunologic complications associated with lithium carbonate ingestion. This drug has demonstrated proven efficacy in the treatment of bipolar affective disorder. This fact, the authors believe, outweighs its dermatologic complications, and only exacerbation of psoriasis warrants termination of therapy.

The authors present the case of a bipolar patient treated with cognitive therapy after she refused treatment with lithium carbonate. Treatment strategies are described, particularly those relating to mania, and possible reasons for their apparent success are discussed.
