In patients with failed hormone regulation who are scheduled for indispensable total thyroidectomy, the risk of thyroid storm with severe end-organ complications has to be anticipated. This case report presents the successful surgical and anaesthesiological management of a patient with Graves’ disease, without any signs of perioperative thyroid storm. Possible recommendations for treatment are presented.

Thyroid carcinosarcoma is a rare and aggressive neoplasm of the thyroid gland, with a recurrent character and a bad prognosis, despite the introduction of a multimodal treatment that is not yet codied. We present a case of thyroid carcinosarcoma, number 32 described in the literature, in a 64-year-old man complicating a multinodular goiter that had been evolving for 10 years and which relapsed 6 months after total thyroidectomy. On the anatomopathological level, thyroid sarcoma was mentioned before the microscopic aspect of the tumor and conrmed by the immune histochemical study. The recurrence was massive requiring the use of tumor reduction surgery that allowed the avoidance of a tracheotomy and a good locoregional control of the tumor. And yet the patient died of lung metastases 4 months later.

Pretibial myxoedema and thyroid acropachy are rare manifestations of Graves’ disease. Clinically, pretibial myxoedema is characterised by hyper-pigmented,asymmetrical, indurated non-pitting lesions of the lower limbs, whilst thyroidacropachy presents as digital clubbing and swelling of digits and toes. We describe a59-year-old female, who presents with pretibial myxoedema and thyroid acropachy,a decade post-thyroidectomy for Graves’ disease.

Background: Goiter sometimes has retrosternal extension in the mediastinum. And may present with compressive symptoms such as dyspnea, dysphagia, dysphonia or sleep apnea. Treatment of choice is total thyroidectomy and removal of the intrathoracic portion of thyroid. However, these cervicomediastinal masses may be separately encapsulated, and a sternotomy may be required for complete and safe excision of the mediastinal mass to achieve decompression of the surrounding structures and preventing the hemorrhagic complications if attempted from cervical incision
