Abstract Objective To illustrate the importance of treatment duration with intramuscular testosterone undecanoate (Nebido®) for the final spermatogenesis recovery after treatment cessation. Also, to show a subsequent poor efficacy of the selective estrogen receptor modulator (SERM) clomiphene citrate (CC) in treating steroid-induced azoospermia following Nebido® cessation and describe that initial oligozoospermia, existing before starting Nebido®, largely contributes to that treatment outcome. Methodology Setting: Department of Human Reproduction and Department of Endocrinology, Clinical Hospital Center Rijeka, Rijeka, and Department of Endocrinology, Clinical Hospital Center Sestre milosrdnice, Zagreb, Croatia. Patient: A male patient having been diagnosed with primary hypogonadotropic hypogonadism, oligozoospermia and low testosterone (T) level, was treated with intramuscular testosterone undecanoate (TU) depot 1 g (Nebido®) to prevent further progression of testosterone deficiency symptoms (low mood, energy and concentration, fatigue, muscle weakness). Interventions: Stopping Nebido® and treatment with CC 50 mg per day 5 days per week for 3–6 month to recover spermatogenesis. Main outcome measures: T levels and semen analyses. Results Semen analyses did not return to values before taking Nebido® 1 year after cessation nor after 3 months of treatment with CC. Values of T, follicle stimulating hormone (FSH) and luteinizing hormone (LH) dropped even more than before starting Nebido®, after 1 year of cessation. Conclusions Here we describe a case of initially idiopathic gonadal failure with subsequent secondary gonadal failure and infertility resulting from testosterone replacement therapy (TRT) treatment, and poor spermatogenesis recovery outcome of CC used post Nebido® cessation.

Uvod/cilj: Cilj je ovog istraživanja ispitati stupanj znanja i stavove o oralnom zdravlju medicinskih sestara, prvostupnica/prvostupnika i magistri/magistara sestrinstva, zaposlenih u KBC-u Rijeka, te vidjeti postoje li razlike u znanju i stavovima u odnosu na stupanj stručne spreme. Ispitanici i metode: Istraživanje je provedeno na uzorku od 100 ispitanika muškog i ženskog spola, u dobi od 22 do 62 godine. Ispitanici su medicinske sestre sa završenom srednjom školom (49 %) i prvostupnici ili magistri sestrinstva (51 %). Istraživanje je provedeno putem upitnika Hiroshima University-Dental Behavioral Inventory (HU-DBI). Rezultati: Medicinske sestre / medicinski tehničari koji su prvostupnici ili magistri sestrinstva imaju statistički značajno viši ukupan HU-DBI (p = 0,033). Statistički značajne razlike nađene su u pitanjima o pažljivom četkanju svakog zuba (p = 0,026), o odgađanju odlaska stomatologu sve dok ne zaboli zub (p = 0,008) i o tome da im se dogodilo da im stomatolog kaže da dobro četkaju svoje zube (p = 0,016). Zaključak: Dobiveni rezultati pokazuju da medicinske sestre sa završenim studijem sestrinstva imaju više znanja o oralnom zdravlju i da ta znanja primjenjuju u svom svakodnevnom održavanju oralne higijene

Introduction: In December 2019, the existence of a new type of disease, caused by SARS coronavirus 2 (SARS-CoV-2), was discovered in the city of Wuhan, the Republic of China. The disease itself is characterized by a large number of patients with moderate and severe clinical presentation, who require hospital treatment. The organization of the healthcare system of Serbia, during the aforementioned pandemic, has required the engagement of all doctors, regardless of their specialty, in the treatment of patients with COVID-19. Surgeons of all branches, from the Surgery Clinic of the Clinical Hospital Center Zemun, were directly engaged in the treatment of both primary manifestations of the virus and the numerous surgical complications arising in the wake of this disease, but also in the treatment of primary acute surgical diseases in COVID-19-positive patients. Aim: The aim of this paper is to present the functioning of the Surgery Clinic of CHC Zemun, in the conditions of the COVID-19 pandemic, as well as to present the types and results of surgical procedures performed in patients with the COVID-19 infection. Methods: This study was conceived as a retrospective study and was conducted in the period between February 2020 and April 2021, in patients who had undergone emergency surgery and in whom the COVID-19 infection had previously been verified. A total of 232 patients surgically treated at the level of the entire Surgery Clinic of the CHC Zemun participated in the study. Results: Chest drainage, due to the development of pneumothorax, accounted for a quarter of all surgical procedures performed, while, in 53.85% of cases, surgical treatment was undertaken due to acute abdomen of various etiology, and in 21.15% of cases, due to vascular diseases. Conclusion: The COVID-19 pandemic is, in itself, a major challenge for the entire healthcare system. The role of the surgeon is significant, both in organization and in direct treatment, which is additionally complicated by the uniqueness of the entire situation and the severity of the disease itself. In addition to their involvement in the treatment of the COVID-19 infection itself, surgeons were, in a large number of cases, engaged in their primary activity in health care, i.e., in the treatment of surgical diseases and complications of COVID-19, performing demanding surgical procedures in very difficult and unique conditions.
