A 3-year-old female presented with double-outlet right ventricle associated with supravalvular mitral ring, parachute mitral valve, left persistent superior vena cava, and atrial septal defect. The supra-annular mitral valve ring was resected. Only one papillary muscle of the mitral valve could be detected and this was minimally divided to achieve adequate valve opening. An intraventricular tunnel repair technique using a polytetrafluoroethylene patch was carried out to repair the double-outlet right ventricle, which had a subaortic ventricular septal defect. The follow-up results at 2 years are excellent.

AbstractCatheter ablation of ventricular tachycardia is often difficult in patients after surgery for CHD. In patients with a ventricular septal defect patch, it is necessary to decide which ventricular side is appropriate for catheter ablation. In this article, we report a case of successful point catheter ablation of re-entrant ventricular tachycardia. Identification of the ventricular septal defect patch using intra-cardiac echocardiography was useful.

The authors achieved biventricular repair in a 1-year-old boy having double outlet right ventricle and unusual straddling of the mitral valve. There was a substantial abnormal muscular bar present within the right ventricle, and some of the tendinous cords of the mitral valve were attached to this structure. Preoperative echocardiography was particularly informative in enabling the surgeon to arrange the optional operative procedures. The postoperative course was smooth and uneventful.

AbstractIsolation of the left innominate artery from the right aortic arch is a rare anomaly. Herein, we present an even rarer case of incomplete isolation of the proximal left innominate artery with the right aortic arch in a 3-month-old female infant with a double-outlet right ventricle and pulmonary stenosis. Surgical repair at 6 months of age was successful, leading to the restoration of adequate flow in the left arm.
