ABSTRACT Restoring the edentulous patient with an esthetic and functional restoration may present numerous challenges to the clinician. The patient's occlusal vertical dimension, centric relation, esthetics and phonetics need to be determined and maintained throughout the restorative process. This clinical report describes the fabrication of implant supported fixed prosthesis in the mandibular posterior region, a tooth supported fixed prosthesis in the mandibular anterior region and a tooth supported maxillary overdenture with a low profile attachment. Implant-supported prosthodontic rehabilitation of total edentulism remains the most complex restorative challenges. The main reason being the number of variables that affect both the aesthetic and functional aspects of the prosthesis. A hybrid denture or the ‘Toronto prosthesis’ is one that is fabricated over a metal framework and retained by screws threaded into implants. This article presents the fabrication of implant-retained maxillary Equator® attachment overdenture opposing mandibular implant-retained hybrid prosthesis. A total of four implants (Equinox® Myriad plus) were placed the maxillary arch and six implants (Equinox® Myriad plus) in the mandibular arch. The patient's occlusal vertical dimension, centric relation, aesthetics and phonetics were determined and maintained throughout the restorative process. This case report describes the management of a completely edentulous patient with a mandibular implant-supported fixed prosthesis and maxillary implant-retained overdenture with Equator® attachment.

ABSTRACT Esthetic and functional rehabilitation of severely worn dentition presents a significant challenge to the restorative dentist. The prerequisites in restoring the worn dentition are to understand the determinants of the occlusal vertical dimension and the effects of its alteration on the temporomandibular joint, muscle comfort, bite force, speech and long-term occlusal stability. This clinical report describes a sequential treatment procedure of full mouth rehabilitation of a patient with severe attrition and cervical abrasion of teeth with metal-ceramic restorations and implant supported fixed dental prosthesis after establishing a functional and esthetic vertical dimension and occlusal plane. Treatment options for patients with severe attrition resulting in reduced occlusal vertical dimension are often limited to fixed prosthesis to reestablish proper occlusal vertical dimension and functional occlusion. In some cases such as when there are limited finances, minimal esthetic concerns, and medical considerations fixed prosthesis may not be the ideal treatment option. Overlay removable partial dentures (ORPDs) can be used as a provisional or interim prosthesis as well as permanent prosthesis in these cases. While ORPDs can provide a reversible and relatively inexpensive treatment for patients with a significantly compromised dental status, there is not much scientific evidence in the literature on ORPDs. Most studies published on ORPDs to date are primarily reviews and clinical reports. In this article, literatures on ORPDs are summarized and a patient treated with interim and permanent ORPDs is presented. This article reviews previously published literatures on the use of ORPDs. Indications, advantages and disadvantages are discussed. Treatment protocol with an example of the prosthodontic treatment of a patient with severely worn dentition with an interim ORPD and later a permanent ORPD are discussed in details.
