Reconstruction of the temporomandibular joint (TMJ) region is challenging. The conventional direct preauricular incision permits only limited access to the TMJ and surrounding structures, therefore risking injury to the facial nerve during retraction. The ideal approach allows sufficient exposure, preservation of underlying neurovascular structures, and achieves an optimal aesthetic outcome. We describe a preauricular posttragal incision with a superficial musculoaponeurotic system flap to allow wide exposure of the zygomatic arch, TMJ, condyle, and coronoid process. We postulate that this approach improves access, lessens the amount of retraction required, and creates a more inconspicuous scar.

ABSTRACTBackground : Gingival enlargement, a terminology for an increase in the size of the gingiva, is a general feature of gingival dis­eases. Gingival melanin pigmentation does not present a medical problem, but is likely to generate a concern in aesthetic function. Although melanin pigmentation is not a chief complain, this condition still need to be corrected. Clinicians are often confronted with a challenge in achieving gingival esthetics. Purpose : This case report aims to explain how to achieve esthetical and functional demands in gingival enlargement and gingival hyperpigmentation case with gingivectomy and gingival depigmentation. Case: This surgical procedure aim to retain esthetical and functional demands. Case Management: An 18-years-old male presenting with maxillary chronic inflam­matory gingival enlargement associated with prolonged orth­odontic therapy is reported. Orthodontic appliances are factors for irritation and plaque retention that interfere oral hygiene and regulate gingival inflammation. Surgical therapy was performed to provide a better aesthetic outcome and prevent plaque retention. Conclusion: The combination of gingivectomy and gingival depigmentation can resolve gingival enlargement and gingival hyperpigmentation perfectly with close proximity to the ideal gingival condition.Keyword : Gingival depigmentation, Gingivectomy, Orthodontic

All methods of breast cancer treatment may potentially lead to breast deformities, which are often associated with the reduced mental well-being of patients. Breast conserving therapy (BCT) is commonly used, and its core element is breast conserving surgery (BCS).The aim of this study was to determine the level of satisfaction with the aesthetic outcome of surgery and quality of life (QoL) of breast cancer patients undergoing BCT in a longitudinal study performed three months, six months and 12 months after surgery. This longitudinal observational study was carried out on a group of 91 women. The Breast-QTM BCT 1.0 questionnaire was used in this study. Before surgery, patients assessed their satisfaction with the appearance of their breasts (SwB) at an average level of M = 56.0. Satisfaction with the aesthetic outcome (SwO) of BCS was highest among all patients three months after surgery (M = 63.0). The mean score in the sub-scale of psychosocial well-being (Psycho-soc W-B) before surgery was M = 62.0, while in the subsequent periods of the study, it was higher. The mean score for physical well-being (Physical W-B) before surgery was M = 69.92; and in the subsequent study periods, it was lower. The level of patient satisfaction with the outcome of the surgery and the QoL related to health do not differ significantly in post-operative observation. QoL in terms of psychosocial functioning in patients undergoing BCT is significantly higher 12 months after surgery compared to the pre-operative period. Patient satisfaction with the BCS aesthetic outcome is positively related to the evaluation of QoL in terms of psychosocial functioning.

With the advent of job rotation and the sharing of VDTs by a multiplicity of users, it is becoming necessary to invest in a workstation that is multi-user orientated. Considering the numerous systems on the market, a dilemma arises as to system choice. Buying furniture that is adjustable does not reflect which user is accomodated by the adjustments. Four chairs and four desks were evaluated against the ergonomic ‘ideal’ standard. This provided an objective measure in the form of an ergonomic composite score. The lower the score, the closer it approaches the ‘ideal’. Subjective evaluation was gathered through a questionnaire. Interestingly the results of the ergonomic composite scores reflected users subjective preferences. Using both these methods of evaluation it is possible to make a choice and invest in the best system available The primary deciding factors are the ‘ease’ of adjustment and the ‘range’ of adjustments possible.
