Conservative surgical treatment of mixed tumours of the parotid gland

ntroduction: Salivary tumors represent a heterogeneous group of tumors of diverse location, histological structure and bio logical behavior. The purpose of this study was a retrospective evaluation of surgical treatment of a group of patients with newly dia gnosed benign tumors of the parotid salivary gland operated in the years 2014–2018 at the ENT department of Hospital České Budějovice, a.s. Material and methods: A type of surgery, defi nitive histology, postoperative complications and a number of recurrences were monitored. The data were evaluated by descriptive statistical methods. Results: A total of 190 surgeries with benign histological fi ndings in 182 patients were performed. The most common benign result was Warthin‘s tumor (90 cases, 47.4%), followed by pleomorphic adenoma (66 cases, 34.7%). Uncommon histological types of tumors were dia gnosed in 14 patients (7.4%). Non-tumorous fi ndings were found in 20 cases (10.5%). The most frequently performed procedure in 84 cases (44.2%) was extracapsular extirpation. Transient lesion in the area of innervation of the temporofacial and / or cervicofacial branch of the facial nerve was present in 34 (17.9%) patients. Permanent paresis of some of the branches of the facial nerve was reported in 2 (1.1%) patients. In 12 patients, the postoperative course was complicated by the development of salivary fi stula (6.3%). Pleomorphic adenoma relapsed after extracapsular extirpation in 5 cases (20.0%). After partial parotidectomy, one recurrence of pleomorphic adenoma (3.6%) was recorded. Warthin‘s tumor relapsed after simple extirpation in 4 cases (20.0%). Conclusion: Our evaluation results of the surgical treatment of benign parotid gland tumors lead us to make further eff orts improve the care of our patients. We see reserves mainly in the routine use of ultrasonographic examination with performance FNAC and thus planning a safe and suffi ciently radical operational solution. AbstractThe method of surgical treatment for benign tumours of the parotid gland has not yet been rationalized, but many authors recommend parotidectomy as the most appropriate procedure. A series of 289 operations for parotid swellings is reported; the majority of mixed and Warthin's tumours underwent enucleation without either complications or recurrence. The rationale of parotidectomy versus enucleation is discussed but in fast growing, deeply infiltrating and recurrent tumours, parotidectomy appears to be the best choice. In the other group of more commonly occurring tumours, enucleation represents a reliable time saving option.

Aim. To study the effectiveness of treatment of sialolithiasis by sialolithotripsy. Methods. Between 2015 and 2018, extracorporeal shockwave lithotripsy of salivary stones of 39 patients was performed in the Nasreddin Tusi Memorial Clinic. The stone was located in the submandibular gland and its duct for 33 patients and the parotid salivary glands and its duct for 6 patients. The average age of patients was 503.38 (between 22 and 77). Results. Stone fragmentation as a result of sialolithotripsy was achieved in 36 (92.7%) of 39 examined patients (p=0.031). In 3 patients the stone was completely fragmented, but due to the narrow lumen of the main duct, it did not move away. In 3 patients with calculus size more than 2 cm, the stone was not fragmented. According to our results, the necessary effect was achieved in 85% of cases and was unachieved in only 15% of cases. Conclusion. Given the complexity of the surgical treatment in patients with salivary stone and potential postoperative complications, especially with the stone localization within the parotid gland, we concluded that extracorporeal shockwave lithotripsy is a promising alternative treatment for patients with salivary stone.

Introduction:Sialectasis of parotid duct is an uncommon condition characterised by abnormal dilatation of a part or whole of the Stensen's duct. It may lead to recurrent parotid infections as well as a cause of concern for the patient. Superficial parotidectomy is the accepted surgical treatment for this condition. Case Report: A case of idiopathic dilatation of parotid duct is presented. It was treated by marsupialisation of the sialectatic segment.This procedure proved to be much simpler and safer than superficial parotidectomy whilst being equally effective. Conclusion: In cases of Stensen's duct sialectasis without any abnormality of the parotid gland, marsupialisation of the ectatic segment should be the treatment of choice.
