Abstract A 4-year-old girl underwent an open-neck surgical procedure for a recurrent pyriform sinus fistula (PSF). A catheter could not be inserted through the endoscope into the opening of the fistula. An open-neck surgical procedure with coring out of the fistula stained with a dye revealed that the fistula was missed near the upper lobe of the left thyroid. A guide wire was successfully inserted via the endoscope into the fistula, and the wire was gripped with forceps under fluoroscopic guidance and removed. As a modification of the surgical approach and treatment for the orifice of the PSF, a catheter was exchanged through the guide wire, and a nylon thread was passed into the catheter. The tip of the nylon thread from the oral side was fastened and fixed to a gauze ball. After removing the nylon thread, the orifice of the sinus fistula was recognized and sutured. She was discharged uneventfully and has done well without a postoperative recurrence for 12 months.

Objectives. Pyriform sinus fistulae arise from disturbances in the development of the fetal third and fourth branchial pouches and are predominantly found on the left side. We report the rare case of a right-sided pyriform sinus fistula presenting as a lateral neck abscess.Study Design. Case report.Methods. A 24-year-old woman presented with a two-week history of right-sided neck abscess. A fluoroscopic sinogram revealed a fistulous tract extending from the abscess to the apex of the right pyriform sinus. It was determined that the fistula was likely a third or fourth branchial remnant, a rare right-sided finding. Chemocauterization of the fistulous tract with 40% trichloroacetic acid was used to successfully treat the patient.Results. Approximately 93–97% of branchial pouch anomalies are left sided. Treatment options include surgical excision and cauterization.Conclusions. Branchial cleft cyst and pyriform sinus fistula must be considered in the diagnosis of cervical abscess in either side of the neck.

Osteochondroma (OC) is one of the most common benign tumors of bone but is rarely found in the mandibular condyle. The aim of this case report is to demonstrate condylectomy through endaural approach (EA) as a conservative alternative for the treatment of the OC. A 60-year-old woman having an OC of 23 millimeters length in the mandibular condyle. The patient underwent a surgical procedure for tumor resection. To generate less morbidity, a conservative approach was performed.
