Moyamoya syndrome (MMS) is a rare, chronic and progressive vasculopathy with a characteristic angiographic pattern and well-recognized predisposing conditions, such as cranial therapeutic radiation. We report the case of a 36-year-old Caucasian female with a history of craniopharyngioma treated with whole-brain radiotherapy 20 years previously. She was admitted to the emergency department with disorientation and imperceptible speech lasting for 1 hour. Upon examination, she had slight motor aphasia, without sensory or motor deficits. However, the neurological deficits worsened on standing position. The computed tomography (CT) angiogram and transcranial Doppler ultrasonography revealed occlusion of the distal portion of the left internal carotid artery (ICA). Mechanical thrombectomy was attempted without success. Head CT was repeated, revealing left periventricular acute ischaemic stroke. The cerebral angiography showed total occlusion of the left ICA with an exuberant network of transdural collateral vessels, confirming MMS. The patient completed a functional rehabilitation program with progressive improvement of deficits and maintained a multidisciplinary follow-up. MMS is a serious late complication from cranial radiation therapy and a well-described risk factor for ischaemic stroke in younger patients. Therefore, early detection and prompt treatment are mandatory, as the consequences can be disastrous, such as cognitive and neurologic decline due to repeated strokes.

ABSTRACT This clinical report describes the implant rehabilitation for the treatment of pathological roots resorption due to inadequate orthodontic movement in prosthetic space reduced by means of load and immediate implants, computer-guided surgery and use of the own tooth crown in a 21-year-old-patient. The atraumatic exodontics of the right and left upper lateral incisors was performed, and then immediate placement of osseointegrated implants using the computer guided surgery technique. The crown teeth itself was used in the immediate aesthetic and functional rehabilitation. Completion of the treatment resulted in a functional and aesthetic successful outcome and a 27 months follow-up presented uneventful. The procedures included in this complex rehabilitation treatment in the esthetic zone were appropriate and essential for the maintenance of the soft and hard tissues contour and thickness ensuring the excellence in rehabilitation.

ABSTRACT Esthetic and functional rehabilitation of severely worn dentition presents a significant challenge to the restorative dentist. The prerequisites in restoring the worn dentition are to understand the determinants of the occlusal vertical dimension and the effects of its alteration on the temporomandibular joint, muscle comfort, bite force, speech and long-term occlusal stability. The aim of this clinical report is to present the replacement of unsatisfactory metal-ceramic crowns of elements 12, 11, 21 and 22, by lithium disilicate glass-reinforced ceramic crowns in a patient with skeletal anterior open bite. A patient sought care at the Dental Hospital at the School of Dentistry of UFU, complaining of odor between the metal-ceramic crowns of the antero-superior teeth. After clinical and radiographic examination, invasion of the biological space was noticed, surgery was indicated in order to restore the biological space. After healing, the pre-existing cast metal posts were masked using an opaque composite resin and the teeth were reprepared for full all-ceramic crowns. Impressions were taken in two steps, using PVS associated to retraction cords. The ceramic copings were obtained in lithium disilicate ceramic, and the veneering was performed by stratification. After testing the ceramics crowns in relation to, fit, function and aesthetic results, cementation was completed using modified absolute isolation, followed by prophylaxis of preparations with pumice and saline, surface treatment of the ceramic restorations and luting using self-adhesive resin cement. Finally, an occlusal splint was produced to control the effects of bruxism and orofacial pain symptoms. The anterior open skeletal bite was a challenging factor for the rehabilitation of this patient; however, one must consider the entire process that the patient would undergo if choosing for orthognathic surgery, and the patient should be aware of the case limitations. In addition, the use of lithium disilicate glass-reinforced ceramics proved the versatility of this material for anterior aesthetic restorations.

Dental fluorosis is a dental condition caused by excessive intake of fluoride during enamel formation, which can lead to color abnormalities or defects on the tooth surface. The resultant abnormal appearance ranges in severity from mildly white and opaque to dark brown, which substantially affects patients’ esthetic characteristics and self-confidence. Treatment methods include tooth whitening or restoration. This clinical report describes the use of a minimally invasive esthetic technique in a 22-year-old woman with moderate dental fluorosis. The treatment plan included enamel microabrasion, at-home bleaching for 2 weeks, and subsequent resin infiltration for each tooth under a rubber dam. After 2 years of follow-up, evaluation of the patient’s esthetic appearance revealed that teeth affected by dental fluorosis could be successfully treated with a minimally invasive technique involving microabrasion, at-home bleaching, and resin infiltration.

ABSTRACT The esthetic and functional rehabilitation of severely worn dentition and partial edentulism is probably one of the most intellectually and technically demanding tasks faced by the restorative dentist. The severe wear of teeth and missing teeth has multiple implications in relation to loss of anterior guidance, reduction of vertical dimension and disfigurement of the occlusal plane. Hence, restoring vertical dimension without compromising occlusal, functional and esthetic requirements is utmost essential for long-term success. This clinical report describes the rehabilitation of patient with severely worn dentition and missing posterior teeth using the integration of fixed and removable prostheses.
