Uterine leiomyosarcomas are aggressive tumors associated with a poor prognosis. These neoplasms have high metastatic potential, more frequently affecting the lungs, liver, and peritoneum. There are very few cases of metastasis to the thyroid described in the literature. We present the case of a 47-year-old female diagnosed with uterine leiomyosarcoma metastatic to the thyroid gland. In this case report, we want to emphasize the utility of ancillary studies to help differentiate a leiomyosarcoma from anaplastic thyroid carcinoma since cytologic evaluation alone can be challenging.

The thyroid gland, usually located below and anterior to the larynx, consists of two bulky lateral lobes connected by a relatively thin isthmus. The thyroid is divided by thin brous septae into lobules composed of about 20 to 40 evenly dispersed follicles, lined by a cuboidal to low columnar [1] epithelium, and lled with PAS-positive thyroglobulin. Female, 66 years old with a chief complain of shortness of breath and lump over her neck since 6 month. Neck USG showed enlarged right lobe of thyroid gland with hyperechoic nodules. FNAC confirmed anaplastic carcinoma of thyroid. patient underwent chemotherapy using doxorubicin as agent of choice. She had difficulty in breathing and she kept on high flow of oxygen, even after she had stridor and planned for tracheostomy. MRI revealed heterogeneously enhancing mass (9.6*6.5*10.5 cm) replacing right lobe of thyroid, and extending superiorly till the level carotid bifurcation and inferiorly superior mediastinum. Lesion is partially encasing trachea and laryngeal airway, posteriorly extending into prevertebral space and anteriorly invading strap muscles and sternocleidomastoid muscle. Now it become very difficult stage. She transfers to ICU. intubation tried but failed. As mass was just anterior to trachea and difficult to do tracheostomy without intubation. all hope failed and she put continue to high flow of oxygen.

Coronavirus disease 2019 (COVID-19) has overwhelmed healthcare globally with millions of cases and over 2 million deaths worldwide. The hypercoagulable state associated with COVID-19 is a well-recognized complication that carries a poor prognosis. Marantic endocarditis, or non-bacterial thrombotic endocarditis (NBTE), is caused by a sterile vegetation on the cardiac valves resulting from the deposition of fibrin and platelet aggregates. It is highly associated with the hypercoagulable and acquired inflammatory states. Herein, we report a unique and rare case of COVID-19 presenting with NBTE.

Anaplastic thyroid cancer (ATC) is one of the most aggressive tumors in human medicine. Despite the multimodal therapy, the average survival of patients is just several months. Fortunately, it is an extremely rare tumor. Most frequently, ATC occurs in the elderly, with the average age of the patients being almost 70 years. It is extremely rare that ATC occurs in people under the age of 40. We report a case of a 33 year old woman with ATC, with the diameter of the tumor being 8x7cm, confirmed by histopathology and imunohistochemistry. In January 2001, a total thyroidectomy was performed, and the tumor was completely resected. Postoperatively, the patient received radiation therapy. Ten years after the surgery, on regular check-ups, the patient is still living and there is no evidence of a recurrent tumor or metastases. This case is instructive for two reasons, as it shows that ATC can occur in younger people and that there is a possibility of long term survival.

The ectopic tissue of the thyroid gland along the descent of thyroglossal duct is a rare congenital aberration with or without the presence of normal thyroid gland. Single or dual ectopic thyroid has been reported. Three ectopic focuses at different locations are extremely rare. We present a rare case report of twenty years old female who presented with swelling over the anterior aspect of the neck mimicking thyroglossal cyst. Up on imaging evaluation ectopic thyroid at three different levels were diagnosed by computed tomography (CT) neck. Hence surgery was avoided and patient was managed with thyroxine supplement and follow up.

Fetal midgut volvulus is an extremely rare life threatening condition with poor prognosis. It often remains undiagnosed on antenatal ultrasound and manifest as intestinal obstruction in both antenatal and post natal period. Following is the case report of intrauterine midgut volvulus causing proximal obstruction of stomach and duodenum. The infant survived postnatally after caesarean section delivery with prompt and appropriate surgical intervention. Twisting of bowel loops around the mesenteric vessels suggestive of whirpool sign was the most signicant clue leading to the diagnosis of volvulus.
