A clinical study to compare the dental plaque removal of an electric versus a manual toothbrush over a 4-week period with children age 3-6 years and 7-10 years

Background: The best and the most reliable methods to manage the dental plaque are still mechanical procedures. It has been traditionally recommended that a firm fruit such as an apple be eaten to minimize caries and control plaque at the end of a meal. However, several studies have reported contradictory findings about the microbial plaque function of the apples. Some studies, for instance, have found that apples contain sugar and, therefore, can cause plaque growth; some other researches, on the other hand, have shown that they have the potential to decrease plaque due to their mechanical plaque removal function. This study, therefore, aimed to compare the effects of apple-chewing method and that of tooth-brushing one on plaque removal. Methods: The study group included 48 healthy dental students with good oral health status, who were randomly selected to participate in this comparative, crossover clinical study. First, they were asked to brush their teeth or eat an apple. After 2 weeks, the experiment was repeated with the order reversed. Plaque indexes (PIs) were determined as before brushing/apple eating (baseline, B), immediately afterward (A), and 24 hours afterward (24). Results: Over time, there was a significant shift in the plaque index pattern between the groups (P value<0.001) but this discrepancy, in general, was not significant between the group using apple and the one using toothbrush (P value =0.495), as well as between the group using yellow apples, and the ones using red apples or the toothbrushes (P value =0.768). Conclusions: Comparing the two plaque control methods, it was found they were extremely similar; however, chewing yellow apples was discovered to be more effective method in reducing dental plaque than chewing red apples or using toothbrushes.

Abstract Background Removal of oral biofilm from the oral mucosa is essential for preventing risk of respiratory and gastrointestinal infection in elderly people. Currently, no device is available which can remove oral biofilm from oral mucosa effectively and safely. Therefore, the effectiveness and safety of the Micro Scale Mist UNIT (MSM-UNIT), a newly developed dental plaque removal device utilizing high speed sprays of fine water droplets, were evaluated for biofilm removal, including the rate and surface roughness for simulated tooth surface and mucous membrane. Methods Simulated tooth and oral mucosa coated with an artificial biofilm of Streptococcus mutans were used for evaluation of effectiveness, with uncoated substrates as the controls. The MSM-UNIT and a conventional air ablation device were operated under recommended instructions. The effectiveness was evaluated from the rate of removal of the biofilm, and the safety was evaluated from the damage observed by scanning electron microscope and surface roughness. Results The biofilm removal rate of the MSM-UNIT was significantly higher than that of AIRFLOW. Little damage was observed in the area treated by the MSM-UNIT. The surface roughness of the MSM-UNIT treated area on simulated tooth surface and oral mucosa showed no significant difference to the control area. In contrast, cracks and powder were observed in the area treated by AIRFLOW. In particular, the surface roughness of the AIRFLOW treated area for Toughsilon was significantly larger than that of the control. Conclusions The MSM-UNIT could be used safely and effectively for removing biofilm not only on simulated tooth surfaces but also simulated mucous membrane. The MSM-UNIT has no harmful effect on teeth or oral mucosa, and may be used for comprehensive oral care for patients during nursing care and the perioperative period.

Previous research has shown the inhibitory effects of hop bract polyphenols (HBP) on cariogenic streptococci in vitro, but their effects in humans have not been investigated. This double-blind, crossover clinical study tested the hypothesis that HBP delivered in a mouthrinse suppresses plaque regrowth in humans. Twenty-nine healthy male volunteers had all plaque removed, and refrained from all oral hygiene for 3 days, except for rinsing with a mouthrinse containing 0.1% HBP or a placebo. The results showed that the mean amount of plaque assessed by the Patient Hygiene Performance score after the volunteers used the HBP mouthrinse was significantly less than that after they used the placebo (p < 0.001). The number of mutans streptococci in the plaque samples after volunteers used the HBP mouthrinse was significantly lower than that after they used the placebo (p < 0.05). These findings suggested that HBP, delivered in a mouthrinse, successfully reduced dental plaque regrowth in humans.
