Quality control becomes an important key for companies in suppressing the number of defective produced products. Six Sigma is a quality control method that aims to minimize defective products to the lowest point or achieve operational performance with a sigma value of 6 with only yielding 3.4 defective products of 1 million product. Stages of Six Sigma method starts from the DMAIC (Define, Measure, Analyze, Improve and Control) stages that help the company in improving quality and continuous improvement. Based on the results of research on baby clothes products, data in March 2018 the percentage of defective products produced reached 1.4% exceeding 1% tolerance limit, with a Sigma value of 4.14 meaning a possible defect product of 4033.39 opportunities per million products. In the pareto diagram there were 5 types of CTQ (Critical to Quality) such as oblique obras, blobor screen printing, there is a fabric / head cloth code on the final product, hollow fabric / thin fabric fiber, and dirty cloth. The factors caused quality problems such as Manpower, Materials, Environtment, and Machine. Suggestion for consideration of company improvement was continuous improvement on every existing quality problem like in Manpower factor namely improving comprehension, awareness of employees in producing quality product and improve employee's accuracy, Strength Quality Control and give break time. Materials by making the method of cutting the fabric head, the Machine by scheduling machine maintenance and the provision of needle containers at each employees desk sewing and better environtment by installing exhaust fan and renovating the production room.

The objective of this paper is to provide information from multiple perspectives on the current state of the practice with regard to using recycled materials and recycling agents (RAs) in asphalt concrete mixtures. This information was collected through a survey of U.S. transportation agencies and RA suppliers combined with a search of RA-related specifications and pilot projects previously constructed. Moreover, a case study describing the Virginia Department of Transportation’s experience with RAs provides a tangible example of how at least one agency is approaching the potential implementation of these technologies. This practice review was achieved by documenting the experience, lessons learned, and best practices of multiple asphalt experienced contractors and asphalt binder suppliers in the Virginia area. This paper follows a similar survey conducted in 2014 as part of NCHRP 09-58 and provides a second look at the use of RAs across North America. Not all state departments of transportation have experience with using RAs. Factors preventing the use of RAs included specification limitations, lack of expertise in processing recycled materials, supporting data, and negative prior experiences. Developing a performance-based testing framework is mandatory for the successful use of RAs. In general, good and frequent communication with the RA supplier is critical and necessary during the planning stages, the production of mixtures, and the continuous quality control by the supplier to resolve issues when they arise. Finally, a strong quality control and quality assurance-testing program should be implemented to ensure that materials meet the properties needed to produce a good-performing mixture.

SummaryQuality assurance is the corrective action applied to any observed discrepancy between optimal and actual level of medical care requiring continuous quality control. Obviously, quality assurance is of utmost importance also in psychiatric practice. Quality care as defined by the degree of adherence to standards and guidelines can be measured in terms of structural, process and outcome indicators applied to inpatient, outpatient and complementary services. To promote the implementation of practice guidelines into psychiatric care conceptual, methodological, and organisational requirements must be taken into account.
