Hemihyperplasia is a syndrome with overgrowth of limbs and associated with various genomic syndromes. Rarely patient presents with intestinal obstruction when it is associated with sub mucosal lipomas. Submucosal lipomas are common in colon. But whole of small and large bowel studded with them is very rare as in our case. It presents dilemma not only in diagnosis but in treatment also. Here we present a case of forty two years females with hemihyperplasia syndrome came to causality with recurrent intestinal obstruction. On CECT Abdomen diagnosis of intestinal obstruction due to intussusception was made. Whole of bowel is studded with submucosal lipomas. Patient was tried to manage conservatively but patient does not respond and Right Hemicolectomy was done of intussusception segment. Anastomosis leaked and on re-exploration end ileostomy was done. Obstruction due to Multiple Intestinal Lipomatosis (as a part of Genetic Syndrome) should be managed conservatively and if operative intervention is required avoid primary anastomosis.

The causes of intestinal obstruction are diverse with tissue adhesion, incarcerated hernia, and large bowel neoplasm being the most common causes. Cancer is not easy to diagnose in patients with a history of intestinal obstruction after the intraperitoneal operation following blunt trauma. Herein, we report the case of a patient who was diagnosed with colon cancer after undergoing adhesiolysis due to intestinal obstruction.

Neuroendocrine carcinoma (NEC) of the colon is a rare and very aggressive tumor, generally diagnosticated in an incidental way and during resolution of complications such as perforation or intestinal obstruction. The case of a 58-year-old male patient who was surgically intervened for presenting localized peritonitis pedhora scan to a perforated blind neuroendocrine tumor. Right hemicolectomy whit ganglion D2 resection and ileo – transverse primary anastomosis was performed. Histopathological examination described a well-differentiated neuroendocrine carcinoma of small cells, with no vascular or neuronal infiltration. Now the patient is awaiting chemotherapy without the need for secondary surgical review.

A 40 year old man known to have cryptorchidism, presented with acute intestinal obstruction. At laparotomy stricture of the distal ileum, distended small intestine with enlarged multiplemesenteric and pelvic nodes were found. Right hemicolectomy was done and biopsies of mesentericand pelvic lymph nodes proved the tumour to be malignant carcinoid tumour withmetastasis to lymph nodes. Biopsy of the atrophied right testis showed presence of seminalvesicles.KEY WORDS: carcinoid tumour, obstruction, small intestine, cryptorchidism

Lymphocytic colitis is a rare syndrome of watery diarrhoea, normal colonoscopy findings and mucosal inflammation. The patient does not present with bloody diarrhoea as there is no mucosal ulceration. A 30-year-old male presented with complaints of pain abdomen for 6 months. Ultrasound showed oedematous ileo-cecal junction with ileocolic lymphadenopathy which was managed with antibiotics. He presented again with symptoms of intestinal obstruction in the following month. On laparotomy, there was ileo-cecal thickening for which right hemicolectomy was done. The biopsy report came as lymphocytic colitis. This is a very rare presentation for a case of lymphocytic colitis.

Perineal hernia with bowel gangrene is uncommon but known complication of laparoscopic extralevator abdominoperineal excision (ELAPE). We present a rare case of closed loop small bowel obstruction with bowel gangrene secondary to an incarcerated perineal hernia that developed 7 years after an ELAPE. Intraoperatively, we found a definitive transition point due to adhesions in pelvis and a closed loop obstruction of the distal small bowel at different site with gangrenous intestine. She was managed successfully surgically with adhesiolysis and fixation of defect with biological mesh. Prevalence of perineal hernias will rise in future because of the increasing cases of ELAPE, in which no repair of pelvic floor is performed. The need of follow-up of these operations and more reporting of such cases are important in increasing awareness of these complications. Patients should be made aware of such complications and should seek urgent medical care.

A Rare case of heterotopic pancreatic tissue of ileum causing acute intestinal obstruction has been described with a brief review of literature. A 42 yr old male patient presented to the emergency department with features of acute intestinal obstruction. After evaluation patient was taken up for emergency laparotomy which revealed a band to be arising from ileum. Furthermore, there was a small growth in the ileal wall at the site of origin of the band. Hence resection of the growth was done and followed by ileoileal anastomosis. Later on, histopathological examination of the growth revealed it to be heterotopic pancreatic tissue. Heterotopic pancreatic tissue is often an incidental nding encountered in upper gastrointestinal tract during endoscopy and surgeries. But Symptomatic ectopic pancreas of ileum is relatively rare and they very rarely present with acute symptoms as in this case

Chronic intestinal pseudo-obstruction (CIPO) is a rare disorder characterised by recurrent symptoms and signs of intestinal obstruction without an underlying mechanical cause. Caecal volvulus remains a rare cause of intestinal obstruction that often requires operative intervention. We describe the previously unreported case of caecal volvulus occurring in an adult patient with CIPO, together with his subsequent management.
